My surgeon plans on 2 separate surgeries to correct my Peyronies and ED. First, he is going to perform plaque excision, grafting and insert a malleable device. After 3-6 months he plans on removing the malleable device and size and insert an inflatable device. Has anyone had this done? I wasn't expecting 2 separate procedures, but his explanation about restoring/gaining back length and width seems to make sense. I would appreciate feedback. Thanks

You leave a bit of history out. I had plaque about 2 inches up from my pelvis. It was size of my thumbnail. Hourglass as well. My doc did nothing but put a titan in and do a bit of modeling.

After titan was put in, no more peyronies. No more hourglass. No more plaque.

My surgeon told me the titan took care of it. Period.

Lots of others on here with the same story.

You might want another opinion. I did not find my implant surgeon till doc 4.
Dealt with ED, Peyronies, & venous leak for 3 yrs.

Removed Titan and put in LGX 18 cm plus 2 RTE's, 20 cm total.

I had a 90 degree curve and heavy plaque. I could still get an OK erection but it wasn't good for anything. Just did it all in one two weeks ago. I wouldn't be thrilled to be looking at another procedure months afterward, but I think I am a sissy compared to some of the guys on here. Just not a doctor fan. If it were me, I would not preclude it, depending on your trust level, but I'd absolutely want to check in with a second Doc.
60 Years Old, healthy and young for age, diagnosed 3 years ago. Verapamil by Levine, recommended for either plication or graft, but surgery cancelled. Abandoned hope for 2 years. Implanted AMS 700 CX October 2020. So far, so good.

First of all, doctors don't do everything the same way and 2nd, in order for any of the members to offer you an accurate opinion, the would have to have the exact same problem... same side plaque in the same place.  If you are happy with what your doctor has told you, you should go with that... if not, keep asking questions until you're satisfied.  My problems were not the same as yours but I've had 6 or 7 surgeries and i'm still not done. Trust your surgeon. :)

I had excision and grafting and implant in one surgery.  If he can insert th e malleable why not the inflatable.  I'd get at least an opinion from a high volume implant doc.  
PxD 2 yrs 9/16.  Failed all treatment. 9/11/18: excision, grafting & implant Dr Karpman MtnView Ca, AMS CX 18cm + 3-1cm RTEs.
Pump failed.  2/11/20 Dr Karpman installed Titan 22cm +1cm RTE.

Does your size is the same as before Peyronie?
Surgeons say an implant can't restore girth and length, so a reconstructive surgery is needed before the implant is put. Both procedures can be done on the same day.
1993 born. Brazil.
Peyronies since Sep 2019.
Penis developed curvature, lots of plaque, loss of size and axial rigidity. Severe peyronie.
Mar/21 TEP + Titan Touch 20 cm + 1.5 RTE.

Thanks for the responses. I will ask more questions on Thursday. One reason he did give for the separate procedure revolved around restoring/gaining length. The malleable implant serves as a type of full time traction device. He says the second surgery will give him a better chance at using a larger implant than if I just went right to the inflatable implant. I will update after I see him on Thursday.

Another question. What amount of pain should I expect after the surgery?

Dear Tpat,

I had a nesbit surgery to correct a bend, hinge, and had a graft before my implant. DON"T DO IT.
Although it corrected the bend and hinge, it shortened my penis by almost 2". With my implant, I have recovered about an inch.
If I had a chance to do it again, I would definitely go directly to the implant.

I had excision and grafting and lost 1/2 inch. Hardly noticed the difference and still very happy with it.

THERE IS NO ONE ANSWER FOR EVERYONE ON THIS TOPIC. A lot depends on your particular situation, amount of curvature, your doctor, etc. etc. etc.
Developed peyronies 2007 - 70 degree dorsal curve
Traction/MEDs/Injections/Surgery 2008 16 years Peyronies free now

I agree with the notion that everyone is different and therefore the solution for could/would be different. In my case my surgeon (Dr Eid) discussed excision and grafting for the significant calcified plaque that I had and he felt strongly about putting the implant in and letting it due its job and straighten my curve over time.  He did not want to do any excision or grafting due the potential downsides of nerve damage and higher chance of infection.

Others on the board have had success with excision and grafting so my experience is just a data point of one.  I would ask about potential complications of the excision/grafting process vs going straight to implant.

I am happy I had excision and grafting.  It removed a lot of calcified scar tissue and straightened me out.  Ultimately I ended up with an implant anyways, but I am glad none of that calcified tissue is in me anymore.

Also, I never really measured myself prior to getting peyronie's except maybe as a teenager, haha and cannot remember what size I was.  I would say I maybe lost 1/2 an inch total from start of symptoms to now.  The grafting gave me back some length, and cycling the implant I may get some more back, but if not I am happy with what I got now.
