Hello,

I just signed up to the site and had some general questions.  I am 45 =
years old, in good health and was diagnosed with Peyronies Disease in July 2013 with no =
pain and moderate curvature. My main reason for visiting the urologist =
was to treat ED and the Peyronies Disease was discovered on my first visit. I have been =
taking Trimax and Edex injections to achieve an erection, but have only =
achieved 75% hardness compared to what I used to get, and worse, once I =
penetrate the vagina it seems to almost immediately dissipate. Is this a =
common occurrence with ED, Peyronies Disease or both? Also, I have been married seven =
years, but have noticed a gradual decline in my attraction to my wife =
well before ED was a problem. How much does ED affect libido? Even when =
I achieve a modest erection I am not excited to have sex with my wife or =
anyone else, for that matter. I am confused as to which came first and =
am hoping someone on the forum has some insight that could help. I =
love my wife very much and she loves me, but a sexless marriage is not =
going to work for us and I am running out of time.

have you had your testosterone levels checked?  low T levels can contribute to decreased libido.  low T could also contribute to poorer quality erections.  It could be that the libido and erection issues are unrelated to Peyronie's.  Peyronie's can be scary but don't panic.  This forum has a wealth of information and a lot of supportive people.  Educate yourself, ask questions.  There are many things you can do right now to lessen the damage.  Check out the forums on Vacuum erection devices, nutritional supplements and recommended urologists.  Search the web for urologists in your region and look for one that has experience with Peyronie's.  Some urologist's web sites don't even mention Peyronie's in the individual doctor's profiles.  You may need to call their offices and ask who on staff has experience.

Arcadia- I agree with pretzel, get your T levels checked. It is not uncommon for men in their 40's, to have a brief time when they are not turned on by their wives. But as you said a sexless marriage is not going to work for either of you, so figure it out. Study this forum about VED. If injections are not good enough, use the VED and a cock ring and you'll stay hard as long as you want to.Regards Inkhorn

Yes, I have had the testosterone levels checked over the last 1 1/2 years. There was a temporary spike in libido early on when I was on a double-dose of Androgel, but that was dangerously high. For the last six months my T levels have been normal without Androgel and, honestly, there was no noticeable difference in my libido or energy level throughout the treatment, other than the spike. Can either of you speak to my issue with losing an injection-produced erection upon penetration? Does that sound physiological of psychological? Should the injections produce rock-hard erections by themselves?

If you have bad venous leakage like me, even TriMix or QuadMix won't do the trick all by themselves. That's my situation. I use a cock ring along with the injection and that does the trick. Here's what I do in order to maximize my penis size for sex/masturbation:

(1) I use a VED for 5 minutes.
(2) I slip on a cock ring.
(3) I inject QuadMix. (Inject at the 2 o'clock location. It works better than the 3 o'clock location)

I also take Viagra and/or Cialis 30 minutes before the VED. I don't know if my erections are getting better but last night I only used Viagra and Cialis then the VED & cock ring and I was able to maintain a solid erection for quite a while.

It's an annoying process but that's what I have to do in order to make things work. Plus, that combination makes my penis even bigger than it was before Peyronie's.

Thanks for the advice. A few questions. Clock positions? Do you mean as I look at my penis? Is is better to take injections on the side with or without Peyronies Disease? Did I understand that you take Viagra and Cialis and the injection? Are there no side effects from using so much medication? I am going to my uroligist this week to get a VED demo.

All your symptoms point to venous leakage. As it gets worse the pills and then the injections do not help.  

As for testosterone it will give you a temporary boost in libido but it will not last.  The other benefits are still worth keeping a check on our testosterone level.  Some urologist only check total testosterone you need both total and free checked. Your free should be around 60% of the lab range to be effective.  

When you see the doctor talk to him about venous leakage and see what he recommends. The first step is a VED with a constriction ring.

The clock positions would be, 12:00 is at the top of your penis. 2:00 is slightly to the right side of your penis. 3:00 would be directly into the right side of your penis, level with the floor.

As far as the injections themselves are concerned, you should definitely have the approval of your urologist before doing injections. Some people get Peyronie's from the injections so you have to be as sure as possible that this won't happen to you. Also, I don't think injecting directly into plaque is advised.

Yes, I had been taking Viagra, Cialis and the injections but my erection quality has improved so I don't see needing to take all 3 as necessary anymore. There are no side effects for me other than a stuffy nose caused by the Viagra/Cialis. My venous leakage insures that I won't be getting Priapism anytime soon.

And as Jack mentioned, make sure your testosterone is at the appropriate level for your age. Frankly, for me, the higher, the better.

I will double check my total and free testosterone levels tomorrow when I visit my urologist. Several questions: I am testing a VED tomorrow, as well. What questions should I ask? Does the device and a cock ring prevent the blood from escaping due to venous leakage? Also, more and more frequently, when I am able to masturbate and ejaculate, the great feeling that i used to have is gone. In fact, there is next to no feeling. Is this a factor of hardness in your experience? Is hardness directly related to feeling at climax, or is this some other problem? And last, is low libido a factor of ED or a separate issue? I sometimes wonder if ED makes me lose confidence, which leads to low libido, which leads to ED, etc....

A cock ring will help greatly in dealing with venous leakage. The best cock ring I've seen is here: http://www.timmmedical.com/cart/index.php?act=viewCat&catId=6. It would be either item #1611 or #1613 depending on which size is best for you.

Like I said, pump your penis with the VED 30 minutes after you've taken Viagra/Cialis then slip on the cock ring.

If you decide to do the injection as well, then your injection site should be close to the base of your penis, right before you get to the cock ring.

Additionally, there's something I do that a few of the other guys here would scream at me for doing but it sounds as if your leakage is as bad as mine so I'll share this info with you with the understanding that I do not recommend it unless your doctor says it's OK or you can be certain that you're not having any issues with this technique. Here's what I do: I put on the cock ring before I start using the VED. The reason I do this is because I can trap more blood in my penis with the cock ring on versus slipping it on after the VED pump session (I find doing 5 minutes to be optimal for sex or masturbation. Longer than that gives me temporary impotence). So while I'm pumping, the cock ring is trapping almost all the blood versus without the cock ring on and pumping, there's a far less amount of blood being trapped. So at the end of the 5 minutes, I don't need to slip on the cock ring losing some of the blood I had already trapped. In addition to the great benefit of getting a tremendous erection, the extra captured blood floods the scarred areas and should help remodel the scarring.

As for your libido issue, not only should you have your T-Level checked but you should get a comprehensive blood panel done. Get these checked out:

Testosterone Free
Testosterone Total
Thyroid Panels (T3, T4, TSH)
IGF-1
CMP Profile
Lipid Profile
CBC w/Diff profile
PSA
C-Reactive Protein
DHEA-sulfate
LH
Estrogen

This will give us an idea of whether your libido issue is physical or psychological.


Here's a list of the supplements you may wish to consider:

These supplements are designed to boost testosterone and nitric oxide. They will naturally raise your testosterone. Within a short period of time, you should feel a surge in testosterone that should raise your libido, give you firmer erections and could very reasonably have a positive impact on your Peyronie's. There are a number of other supplements that can help as well, i.e. Ubiquinol, but for me, these are the core ones.

D-Aspartic Acid

Produced in the pituitary gland and the testicles, this amino acid (also known as D-aspartate) stimulates the release of luteinizing hormone, which boosts testosterone production and acts on Leydig cells (special cells in the testicles) to increase testosterone production as well as the production of sperm. One study, published in a 2009 issue of the journal Reproductive Biology and Endocrinology, found that subjects who had taken a D-aspartate supplement daily for 12 days increased their testosterone levels by more than 40 percent.

Suggested use: Take 3 grams in the morning


Tribulus Terrestris

This herb, also known as puncturevine, grows in North America and Asia and has been used for centuries to treat sexual dysfunction. The active compound in Tribulus is a saponin called protodioscin, which increases the release of luteinizing hormone from the pituitary gland, thereby boosting testosterone production by the testicles. But protodioscin has also been found to boost immune function, which is important because higher testosterone levels often suppress the immune system. By taking Tribulus, you can boost testosterone levels without increasing your risk of getting sick and missing gym time.

Suggested use: 500 to 1,000 milligrams 30 to 60 minutes before sexual activity and/or workouts. Use for 6-8 weeks, then go off it for 4 weeks before cycling back on again.


Fenugreek

Native to southern Europe, this herb is often used in cooking. It boosts testosterone levels because it contains high amounts of furostanolic saponins, similar to those found in Tribulus terrestris. These phytochemicals boost test production by increasing luteinizing-hormone secretion. One study, published in February 2011 in the journal Phytotherapy Research, showed that healthy young men taking a fenugreek
supplement for six weeks experienced a boost in their sex drive, sexual performance and energy levels. The researchers concluded that this was because of a positive effect on the guys' testosterone levels. Another study, published in the October 2010 issue of the Journal of the International Society of Sports Nutrition, reported that trained men taking fenugreek for eight weeks almost doubled their increase in bench-press and leg-press strength, as compared to those taking a placebo, and they also lost a significant amount of body fat, while those taking the placebo did not.

Suggested use: 500 to 1,000 milligrams an hour before sexual activity and/or 30 to 60 minutes before workouts. I use Solaray Fenugreek Extract -- 350 mg.


Damiana

Also known as Turnera diffusa, damiana is native to Mexico and Central and South America, where, for centuries, it has been used as an aphrodisiac. Recent research from the University of Mississippi reported that damiana contains the active ingredients pinocembrin and acacetin, which are potent inhibitors of the aromatase enzyme. Aromatase converts testosterone into estrogen, so by blunting it, damiana keeps testosterone levels higher.

Suggested use: 50 to 500 milligrams 30 to 60 minutes before sexual activity and/or workouts. I use Solaray Damiana Leaves -- 370 mg


Long Jack (Tongkat Ali)


Raises testosterone by stimulating Leydig cells. Second, it frees up testosterone in the bloodstream. Third, reduces levels of the catabolic hormone cortisol.

Suggested use: 100-300 mg of eurycoma per dose and take it in the morning, 30 minutes before workouts and before bed. I use Long Jack PowerMax 200 -- 200 mg


Forskolin

Directly stimulates Leydig cells. Activates the enzyme, adenylate cycclase, which stimulates testosterone production and fat loss. In a study by the University of Kansas (Lawrence), subjects taking forskolin for 12 weeks experienced higher testosterone levels and greater fat loss than those taking a placebo.

Suggest use: Look for products that supply 20-50 mg of forskolin and take it 2-3 times per day before meals. I use Forskolin 50.


Vitamin D3

Vitamin D is produced in the body from cholesterol, as is testosterone, it follows that higher vitamin D levels can encourage higher testosterone levels in men. In fact, Austrian scientists from the Medical University of Graz reported that men with higher blood levels of vitamin D had significantly higher levels of testosterone.

Suggested use: 5,000 IU per day, taken with meals, to maximize all D's benefits.


Zinc

Zinc is very important for the production of natural testosterone because Zinc prevents testosterone from being converted into estrogen (the female hormone) by making the enzyme aromatase not work plus Zinc itself turns estrogen into testosterone and Zinc helps produce healthier sperm and higher sperm counts so actually low levels of zinc can cause low testosterone levels. Foods high in Zinc include oysters (a natural aphrodisiac), liver, seafood, poultry, nuts & seeds.

Suggested use: 100mg of Zinc daily.


Omega-3 Fish Oil

Fish oil has been shown to lower SHBG and increase production of Luteinizing Hormone (the hormone responsible for triggering the testes to produce T).

Suggested use: I use Mega EFA® Omega-3 EPA & DHA Fish Oil -- 2,126 mg per serving


Citrulline

This amino acid is so closely related to arginine that in the body, it is actually converted into arginine. However, citrulline may boost Nitric Oxide (NO) levels even better than its cousin because cells in the digestive tract are always starving for arginine, and they tend to grab it before it can get into the bloodstream and be converted into NO. The intestines are not interested in citrulline in the slightest, however, which means that it passes straight through them and into the bloodstream. One study, published in 2008 in the British Journal of Clinical Pharmacology, found that when subjects took equal amounts of citrulline and arginine, the citrulline increased levels of arginine and

Suggested use: 3 to 5 grams 30 to 60 minutes before sexual activity and workouts.


Glycine ProPionyl-l-carnitine

GPLC is a patented compound that combines the amino acid glycine with a form of carnitine, which has been modified to provide the body an extra source of energy and to assist with uptake by muscle tissue. Glycine and carnitine are known to boost levels of nitric oxide in the body, most likely by increasing the activity of nitric-oxide synthase, the enzyme that creates NO. In fact, a study published in December 2007 in the Journal of the International Society of Sports Nutrition reported that trained male weightlifters supplementing with GPLC for four weeks had 30 percent higher NO levels than when they were given a placebo.

Suggested use: 3 to 4.5 grams 30 to 60 minutes before sexual activity and workouts. I use GPLC Glycine Propionyl L-Carnitine HCl-GlycoCarn 1000 mg PLC per serving


PycnoGenol

Extracted from the French maritime pine tree, this patented flavonoid is a potent antioxidant. That means that one way that it can boost NO levels is by neutralizing the free radicals that break down NO. But research shows that Pycnogenol also boosts NO levels by increasing the activity of nitric-oxide synthase, allowing for more arginine to be converted into NO and keeping levels of NO higher for longer.

Suggested use: 50 to 100 milligrams 30 to 60 minutes before sexual activity and workouts. I use Pycnogenol -- 100 mg


Horny Goat Weed


Also known as "Epimedium", but its common name is pretty fitting — horny goat weed has been used for centuries as an aphrodisiac. But it was only in the last decade that scientists figured out exactly how it works. Researchers from the University of Milan discovered that horny goat weed contains a phytochemical called icariin that inhibits phosphodiesterase-5, an enzyme that normally breaks down NO. By inhibiting this enzyme, horny goat weed helps the body maintain higher NO levels for longer. Because this is the precise mechanism by which drugs like Viagra and Cialis work to boost erections, horny goat weed can truly be called herbal Viagra.

I would also recommend that you add Human Chorionic Gonadotropin (HCG) and/or Human Growth Hormone (HGH) to your supplementation protocol. The HCG will tell your testicles to produce more testosterone. It will definitely make you horny and aggressive. The HGH will also increase your libido and help wound healing including remodeling your penile scars.

I will talk to my urologist today and reference your information. I was looking at a Pentox discussion and came across a post of yours regarding Dr. Lue. Did you see him? If so, what treatment plan did he offer? It seems that you and I both have the same problem with venous leakage and I am considering seeing him for a second opinion.

Dr. Lue recommended 2,400 mg/day of Pentox and 30 minutes every day of VED exercises. He'll be doing Xiaflex injections starting in March.
