All alone here fighting with Peyronies Disease

I am 42 in age. Have Peyronies Disease for last 2 years. I find no uro knows well about this condition in my country. Sometimes I feel very hopeless as no one can really help me here. My first uro started treatment with verapamil injections and got 2 injections. But 2nd uro stoopped saying that I must not put myself for further injury with needles.  

My problem is hourglass in my tool with small plaque. Sometimes their is pain with sex but often it is painful without sexual activity. Over the time pain was less but no improvemet in indenture. I think verapamil helped me to stop the disease progression and I sould have continued that .

I wanted to re-start injections. But last 1 month ago after the injection I got back inflamation and pain. This time my uro used thicker needle insteed of insuline one. This stupid why make this horrible mistake. I notice some upward curve developed for the first time.Actually the uro I am going, he seems to be don't understand what to do now. Just give me Vitamin E and Edecra 50 mg ( as on when needed)as I have develped moderate ED.

Now I am at a loss! Should I continue verpamil? If any one can suggest me what should I do? Should I start PAV?

It depends on which problem you are trying to correct. You mention only slight curvature. If this does not interfere with sex then I would not risk the injections. You mention indenture. Even though we all want our penis' to look perfect.... are you concerned about the look, or again are you able to perform sexually.

The main issue that you point out is the pain. For sure you need to address the pain. And the most common recommendation here is that of a prescribed drug called Pentox or Trental. This seems to have helped several men on this  forum with the pain. For the "indenture" I would suggest you look at the subjects here on using traction or using a VED for therapy to help or improve penis health.

As far as the PAV...  I've already mentioned the "P" Pentox and that this could help with the pain. The "A" is for Arginine or L'Arginine which can be purchased without a prescription and this will help to improve penis blood flow. The "V" is for erection improvement drugs like Viagra, Cialis, Levitra, etc. While they do improve blood flow, you didn't mention if you had any ED problems. If you are able to achieve good strong erections, I'm not sure adding Viagra or Cialis is necessary.

I will second lwillisjr regarding his advice. If you can perform sexually, you don't need any drastic approach.

My own daily treatment is:
Pentox 2*400mg (some taking 3*400mg)
L-Arginine 2*1000mg
Ubiquinol 2*100mg
5mg daily Cialis (It may be low dose Viagra or Levitra also)

VED or traction may help with the curvature but it takes time, is a very slow process.

As for my condition you said very right that I need to address my pain first. I shall try your suggestion. I have developed moderate ED for last one year and this is after one year of Peyronies Disease. I want to use Edegra ( same as Viagra) and want to know is that make me dependent for erection?

I have collected some integrated argine 1200mg  with lysine 900mg and ornithine 1000mg in one serve. Is that OK, can I take. Here I can't find only arginine.

As for VED I can find what they are saying for penis enlargement only, can't say about Peyronies Disease use. Can I use that one.

The only thing I find here in Indonesia are mixtures of L-Arginine with other supplements for body builders.

lysine 900mg and ornithine 1000mg

I don't know them.
I have ordered L-Arginine 1000mg and Ubiquinol from iHerb, get it in short time and no problems with the customs. L-Arginine is cheap.
The only problem was the it was shipped with DHL and they charged me locally a big amount in addition to the shipment already paid.

Pentox I have ordered from https://www.riverpharmacy.ca/drug/pentoxifylline, including the customs it was 15% from the price here in a pharmacy.
I don't know the low in your country regarding import of medicines and supplements, I had no problems here in Indonesia.
I get it by parcel post, no additional charges.

Regarding VED, here is a "sex toy" and can't find the medical grade. I am using a simple one, be aware that the pump have release button. I am following the single cylinder protocol posted on the board, be careful not to overpump

Thanks for your reply. I understand what you said about VED, I may consider the local one with care as can't get better here in Bangladesh. The worse part my Peyronies Disease fighting I happen to stay in Bangladesh, a country like out  side of the world!!
Other wise it could be different. My condition was not so bad, it was the uros who made me what I was not with the disease ifself

Anyway, I can't even import medicine due to payment issue, I can't pay online. But fortunately, petoxifiline is available here and getting prescription is easy.

I shall visit Singapore in Oct, do you think I can get arginine there? As you are for long time in this research, you may help me in getting information for lysine and ornithene, so far I found they are used for bodybuilding. I am just afraid if it cause any harm to Peyronies Disease.

1. Calcification of the plaque it means the plaque became calcified and "strong like bone" A qualified doctor can see  using ultrasound
2. Basically, those medications that I am taking together with VED suppose to reduce the plaque and increase blood flow
3. I am taking 5mg daily Cialis to have some morning erections. Regarding Viagra, I begin with 25mg, up to 50mg and now 100mg helps very little bit. It may be because my venous leakage getting worst.
4. I don't have answer because I will stop taking just after implant.
5. Indentation or dent is like a place that the penis getting a sharp turn to some direction. Curvature is a gradual curve in some direction. Hourglass is like from the name, some place that the penis get a shape like that.

Hope somebody with better English than me can jump in with the explanations if someting not clear.

Regarding your item #4. I think most doctors would only recommend low doses of Viagra or Cialis is you have problems achieving erections. If you can achieve a good erection then I don't think Viagra or Cialis would be necessary.

I have mild to moderate ED as I have mentioned. Sometimes I can have good erection and have sex with satisfaction, but sometimes I can't maintain it as long as I need.My uro suggest Edgra 50 Mg (same as viagra) before intercourse. Idea is that it give me firm erection all time and prevent bending due to lack of full erection.

I also want to consider having  low dose of such medicine daily basis as I read it is helpful to maintain penile health.

But my concern is that will make me dependent in long time use? My question is simple, shall I face any problem for erection if I stop this medicine after using  long time.

In my case, as I mentioned before the medications effect gone down during time until today even 100mg Viagra don't help.
I don't know if it is because venous leakage or just the drug effectiveness became less because of using it for a long time.
We are using medications for ED because we have ED and usually ED is progressing during time.

I have read posts of people that for them the daily low dosage of Viagra or Cialis reduced they ED.

But my concern is that will make me dependent in long time use? My question is simple, shall I face any problem for erection if I stop this medicine after using  long time.

To your question, very difficult to have a concrete answer how it will be for you because as I have mentioned above, have testimonies on both directions.

This in indeed a good answer as is based on practical experience.

I have showed my URO yesterday the doc I downloaded from this site about Pentoxifiline and other combination drug. It seems he first time to hear this but he was very interested and Rx me pentoxil 400mg X 3 and L-arginine 1000 mg daily. Actually I could get pentoxil (same as Pentox) without prescription and I got one of my friend to bring arginine for me. I just wanted to educate my URO so that he can help other patients, I heard he had few other patients whom he was using vitamine E, Verapamil and steroid injections. I also requested to provied my phone no. to other patient if they are interested to talk to me. If someone come to me I shall ask them to register to this site if they know English. ( I am sorry for my bad English as I am not used to it in daily life).

On the other hand, I am trying to get appointment for ultrasound test to see if my plaque is calcified. Thank you James for giving me this idea.

Hope to start pentoxil and Arginine withing couple of days. My understing if even I am unlucky to have calcified plaque I need to continue this , James pls let me have your opinion.

As for  low dose Viagra and VED ( medical VED is not available here) I am still studying, I would really appreciate more input for this issues.

In my opinion Pentox and L-Arginine is good for blood circulation and energy, this is the reason to take them and doesn't matter if the plaque is calcified.
As I have wrote you, I am using a simple not medical VED but it must be with manual pump and quick release button.
Have to be carefull with VED and not to continue pumping if you feel pain, or even small discomfort.
Read the single cylinder protocol for more details.

I begin with 12.5mg Viagra (cutting 100mg in , increase it to 16.67 (cutting 100mg in 6) and now 4 or 5mg Cialis (cutting the 20mg in 4 or 5). I changed to Cialis because I have learned on the forum that Cialis may help also to dissolve the plaque.
Both make me some nocturnal and morning erections.

I am noticing now that I have lost atleast one and half inch length of my tool and may be little bit of grith . I was not aware of this, actually I was not so serious about overall situation until last two month ago while I had the 2nd injury.

After I discovered this site, I am undertanding my actual situation. I am really greatful to all who initiated this geart effort and who are presently helping us all. I am so much inspired that I now want to help other patient in my country Bangladesh where I suppose many many patient suffering from Peyronies Disease.

Anyway, as for my condition I shall  keep posting about the changes I may get through my oral medication and as well as the status of the plaque after urtrasound.

I have dicied to start pentoxifyline and arginine from this night.

FYI I have started pentoxihyline for three days now only 400mg daily and in 2 days I shall increase it 2X 400mg. Then I shall add arginine package as I mentioned.

I had very good morning erection on 2nd day of pentoxifyline. Is this giving me some inflamed feeling in my tool throughout the day?

Smart to begin with 400mg Pentox daily before increasing to two.
It seems that the Pentox really helping you, happy to read that.
I have increased to two after a week when I see that no side effects. I am taking it in the middle of my breakfast and the middle of the dinner, not late than 6:00 PM.

In my case, I used Viagra 25mg, after that 50mg and by the end 100mg.
Today, even the 100mg not helping too much. But it may be because the venous leakage get worst during that time. I don't know.
In my next appointment with my uro I will ask him to check out the problem.

I know of no studies, and I believe there is NO evidence, that Viagra use causes any dependence on the drug.  There are of course situations where a man has a condition that causes him to start Viagra and the condition continues to get worse.  This might make it look like he is building up a tolerance to Viagra but it is the underlying condition that is progressing not a dependence on the drug or building up a tolerance to the drug.

I need suggestion as I am very upset now!

Yesterday I had USG and report finds following:

"Small dense echogenic structure representing fibrous plaque ( 11 x & X 3mm) is seen in the fascial ( tunica albuginea ) layer covering the corpora cavernosa in the distal and mid dorsal aspect of the penis slightly fight to the mid line. Evidence of tiny calcific foci are seen with in the lesion. A similar smaller dense echogenic structure pepresenting fibrous plaque (6x5x1 mm) is seen in the fascial ( tunica albuginea) layer covering the corpora cavernosa in the distal dorsal aspect of the penis slightly left to the mid line.

The penile architecture is otherwise well maintained.

Both the corpora cavernous and the corpus spongiosa are normal in dimension and echotexrure. The investing fascial layer in rest of the penis in uniform. The normal anatomy of the urethra, penile bulb and the glans are maintained. The vascular structures of the flaccid penis are normal."

Until this USG I knew I had only one plaque . But I don't understand why another plaque developed after 2 years of my Peyronies Disease?! :' I don't think I had any injury at the place of 2nd plaque. First plaque is now bigger than earlier ( in first USG 2 years ago it was 10x4X2mm). I can understand this is due to further injury caused by my uro during injection 3 months back.

Now most sad point is that old plaque got calcified in small area ( report says tiny calcific foci)

I am now on Pentoxifylline 400mg X 2 times, Arginine 12000 daily.

I have good morning erection. Now pain is less.

1.What should I do now considering plaque is bigger with tiny calcification and gifted    a new small plaque

2.This calcification will spread to all the plaque over time?

3.These medicine will help to arrest the calcification?

4. Should I start low dose daily calis so that it may help?

I think a good treatment for one plaque is still a good treatment for multiple plaques.  The number plaques does not change treatment.  The only thing that would indicate a change from pentox is if after a time, you or your doctor conclude you are one of the very small minority that can actually get worse from pentox.  I say you should continue on and monitor your condition.  Unless you see a noticeable worsening, you are on the right path.

2. It is impossible to predict the progression of an individual's Peyronies Disease

3. It has the best chance or arresting the calcification but odds only relate to large numbers or groups.  Such odds do little to indicate what the result will be with an individual.

4. I do not think low dose Cialis is a bad idea.  It is not just for erections but may help arrest the progress of the Peyronies Disease.  th

There seems to be few if any downside to taking low dose Cialis.

I forget to mention in previous posts:
Be aware that Pentox is a blood thinner and is suppressing the immune system.
If you are planning a surgery for example, you should stop taking Pentox a week before the surgery. This is my doctor advice.
If you have some viral infection like flue, you should stop taking Pentox to heal faster. In any case, it works for me. I have now flue, and I stopped Pentox until the flue will pass.

There is an academic study in the literature which only mentions dissolution of a calcified plaque in one patient with ultrasonographic evidence, though one cannot make scientific claims depending solely on one experience. Moreover, some double-blind placebo controlled studies manifest that calcification burden might decrease with medication.

There is an overall general tendency of calcification of the plaque over time, but as Hawk indicates, it is unpredictable in individual cases. Some plaques remain fibrotic for years, some of them quickly becomes calcified. Some people suggest lowering daily intake of calcium and adjusting Vitamin D and K levels in order to maintain the calcium level in blood in appropriate levels. I personally believe that these two vitamins are important, but I do not think that daily calcium intake of body is that vitally important in calcification process.

Regarding your third question, Iranian study demonstrates the impact of Pentox on stabilizing or reducing calcium burden on the plaque. Of course, it is a general scientific statement. We should be aware of the fact that individual cases may vary.

I respect you comments and posts.... but not sure I agree with this one. I agree that Pentox is a blood thinner, but I have never read that it suppresses the immune system. Any blood thinner is stopped before surgery due to the fact it reduces the clotting capability of blood.....  and this is obviously a risk for any surgery.

My personal experience. First time I had flue after beginning with 2*400 Pentox per day I had it for two weeks already with no improvement. After reading the posts on PENTOX - Side effects, stopped Pentox and in one week I was OK. From then, each time I have some infection problem I stop taking Pentox.
From the other side, I don't find nothing by the Internet searching for Pentox as immune system suppressor.
I was also hopping that Pentox will reduce my white blood cell count that is very high (some on the forum experienced reduced white blood cell count from Pentox) but until now has no effect, or maybe stopped the increase.

Thanks for your suggestion about Pentox. Its very important to be careful which only can bring something positive. I experienced some  lowering blood pressure  may be for being one week with Pentox.  Or it the affect of Arginine, I don't know.

I am having Amlodipine 5mg and Hydrochlorothiazide bp 12.5 mg +Losartan potassium usp 50 mg ( these are compound in one tab) daily for hypertension.And I was maintaining 120/80 blood pressure for long time with these medicines. Just today I find it 110/70. I may need to drop some medicine after checking with doctor. I shall be rather happy if I can reduce some.

My concern is that Cialis may further reduce the blood pressue, right?

Any one here has such experience with blood pressue having Pentox and/or Arginine and/or Cialis ?

Viagra, Cialis, Levitra are lowering blood pressure, this is one of the warnings on the package. You should monitor how the low dose of Cialis affecting your blood pressure. I suppose much less than the 20mg.
My blood pressure is ranging between 110/70 to 120/80 with no medications. Sometime my blood pressure with 100mg Viagra is down to 100/60, but just sometimes.
I think you should monitor your blood pressure daily (if you can) and to see what happening. You can't take any conclusion/decision based on one test. It may be not consistent and may be some other reason. If I will monitor bellow or in the range of 100/60, I was going to talk with the doctor, but if you are concerned, go talk with him, but take some more samples, maybe 2 or 3 times a day for one week then he can get some opinion based on his experience.

As I posted earlier my response to pentoxypilline is good as I have noticed good NTE. I am not sure but some softness in plaque may be there, hope in long time it will show some positive result. I am taking this in half way to taking meal as suggested by James.

But regret to say while I increase pentox to 2X400mg daily I experience strong gastrointestinal discomfort. I am also having esomiprazol 2X20mg with pentox. So I am forced to reduce to 400mg pentox daily from yesterday. I need to watch the condition.

1. Can I have any benefit of having pentox only 400mg daily ?

2. Is there any way I can increase the tolerance to pentox?

3. In long time use tolerance level will increase?

3. I like to add CoQ10 2X100 mg daily as an alternative of redusing penox dose. But I found in internet that it reduces the body's response to blood thinner medicine. So will it reduce the efficacy of Pentox?

I begin with Pentox 400mg and after one week I have added 400mg more. I had gastrointestinal discomfort, but after a month it ended. Maybe you should take 400mg daily for longer time before increasing the dosage.
I don't know esomiprazol, maybe someone on the forum can answer for that.
I don't think that CoQ10 is an alternative to Pentox and I don't think that in my case CoQ10 (I am taking Ubiquinol, the refined version of CoQ10) reduced the efficacy of Pentox. I hope that someone with more knowledge than mine will jump in.

Do you use Warfarine?
Do you have diabetes?
If not, I don't think there is a problem. They are not talking about Pentox or Aspirin. But I am not a doctor so you may ask a doctor what is his opinion.

Thank you for your clarification. I understand the issue fully. I am not taking warfarine and don't have diabetes either.

I want to add that now I am able to take Pentox 2 time daily. My doctor gave me esomiprazol/omiprazol 20 mg and Sucralfate USP 1000mg both 2 times. These are helping to maintain appropriate condition in stomach so that I can tolerate Pentox well.

With PAV I am having some feelings of erection all the day. I think this is good, isn't it?

A hard erection all day long can cause damage to the penis tissues!!!

Its remain soft while not in full erection but  not coming to complete normal flaccid position either. I feel it is fillled with more blood  always.

I want to post the current status of my Peyronies Disease condition as on following medication for last 1.5 month.

1. Trental 400mg*2 with Omiprojol 20mg and  Sucralfate USP 1000mg.( to maintain stomach condition, for this I can somehow tolerate 2 times Trental with various discomfort of side effects )
2. Cialis 4mg *1
3.  Arginine 1000 ( with Lysine and ornithine as compound)

Good side is most of the I have no pain in my tool in flaccid and erect condition. I am having many times NTE and sometimes day time also. I am not sure but seems like plaques going to be softer.  But  having some problem in having sex which I shall explain in later post.

Bad sides are :
I am feeling very week sometimes as if I never take any food for long time.  I have lost my appetite significantly with high degree of Nashua and insomnia. Sometimes I feel burning sensation in face and some kind of palpitation in hands, legs or other parts of body. But overall side effects still I am tolerating and want to continue as I believe I am benefited with these drugs.

On the other hand I have went for routine blood check yesterday. All tests are ok. Only WBC decreases by 15 % after taking theses medicines. It is still very much within the range.

Now I need some suggestions as under;

1. Which medicine making feel very week? what can I do for that?

2. Which medicine giving me other side effects?

3. Should I concern about WBC count ? What can I do?

Hope to have some valuable insights.

I can just talk from my experience.
First, you should take Pentox with food. It is a must, otherwise the side effects are really bad, for me in any case. Maybe you will not need the medications that you are taking to tolerate Pentox if you will take it with food. Just a suggestion.
The weakens you are feeling, again in my opinion are not from the Pentox, Cialis and Arginine. It may be from the other medications/supplements or from the fact you are not eating enough. Pentox can cause depression for some people.
Myself I have lost 17Kg in one year without making any diet. It is not from the medications, it is from the stress and my relations with my wife in the last year.
It is good that your blood check up is fine. I don't know what is WBC but if if still in the range it may be OK. If WBC are White Blood Cells you should keep a watch on it as it is one of the Pentox side effects (I mean to lower the white blood cells, I pray God that it will do it for me as mine are very high, more than double of the normal).
If your hemoglobin is low, you may feel very tired. I had mine at 9,000 that is anemic and felt very tired all the time, now thanks God it is 12,000, still bellow the normal, but feel much less tired.
What is your sugar blood result? It may hold a clue for some problems.

I am taking Pentox with food as you advised earlier. I don't think my problems are from other medicines because I am taking those for may years. May be less food is the cause for weakness as I can't take proper amount of food due to less appetite, Nausea, stomach discomfort etc. I have lost 5 kg in last 2 months. I am 5'9" with now 64 kg only.

You are right that BC stands for Waite blood cell. My hemoglobin and sugar level are fine. Besides all bad sufferings my pain with tool gone, good NTE and softer plaque I am happy !!

You may find from my earlier posts below I have hypertension, I am also suffering from arthritis and asthma. I need to add that I have heavy nasal congestion for one week.  You may say see me as a mobile hospital . But I am sure my present bad sufferings have nothing to do with medicines I am taking for my old diseases.

Still I am fighting in many fronts with new enemy Peyronies Disease while no proper doctor here in this corner of the earth. I am getting help from this forum members only and here from my wonderful wife who says what is the problem so what we are together

You must see a doctor and to increase your weight. Have large boned bodies as mine and narrow boned bodies, but even with narrow bones body you are too light. I am also 5'9", 75kg now and fighting to be back on 77kg.

You are right. I need to increase my weight. But I have high cholesterol with hypertension  and without medicine can't be controlled. Umm this is another dept. in my mobile hospital I totally forgot about. So on principle it is risky to get fat.

I don't know what doctor can do, its really complicated. I shall discuss with him.

I would like to update my condition as below:

1. Continuing PAC  with much gastrointestinal problem.
2. Old plaque on the right side of the tool seem to be softer and bit smaller may be.
3. New plaque on left side seems bigger and harder creating  more visible curvature.
4. Have better cortroll over ED by using Cialis 20 mg before sex.
5. I have always been suffering from Premature Ejaculation, after I develop Peyronies Disease this become more frustrating.

I appreciate suggestions for PE.

I don't understand how Peyronies disease can have anything to do with premature ejaculation. The two are completely unreleated. PE I believe is more related to the time elapsed between sexual encounters and your personally abilty to mentally delay your ejaculatory response.
