Can traction cause hourglassing ?

Hi all,

In 2025 I've been using RestoreX for 7 months and gained 34° curve (from 47° to 13°) and length. After 3,5 months I saw that an hourglassing was forming under the glans all around the penis (except ventral part). So not only at the plaque place (which is only left side under glans).

Two questions :
1) What exactly causes an hourglassing effect? What is it exactly ?
2) Can traction cause hourglassing ?

I'm asking because I'm thinking about giving the RestoreX a next 3 months round.

Took me a bit to understand what you were saying. You have experienced a 34 degree improvement which is great. Your current curve of 13 degrees could even be considered "normal" most of us have a slight curve of some sort.

To address your questions, hour glassing happens when a band of scarring or plaque forms all the way around the shaft of your penis. So when you get an erection you actually have a band around your shaft that doesn't expand/stretch the way it used to which results in an hourglass appearance.

Traction is supposed to help hour glassing, not cause it. It is my understanding that VED therapy is even more effective for hour glassing then traction.

Me also(less than you but i have also a second problem with semi hourglass at the base right)

2.You corrected a lot this bend but you developed the an upward curve

I am doing restorex  30 minutes per day for 2-3 months mostly straight mostly without bury the white lines.Only last week i increased tension that I cannot see white lines and started 10 minutes counter bending downwards but very soft.I didnt manage to bury the lines in counter bending i think i am very far from this point.Stopped cialis after a few days testing  because of strong tinnitus.
I Stopped before one month all other (propolis-cq10-L citrulline-L arginine) cause i have too strong and very long in time night erections.
My erections boosted when i started restorex and i am in a point that i am afraid that they last too long every night.I will visit doctor for this

Based in your story and my story i am trying to explain what is happening
I beleive   that this(left bend-becomes upwards bend+ hourglass) is a remodelling.
In the beggining my plaque was palpapbe  mosttly at the left side just behind the glans
Now its more palpalbe at the top behind the glans
I dont know if all these are good or bad trying to have a conclusion

Forgot to say that i didnt regain length yet, probably because i didnt bury the white linesunti a few days ago

How long dit it take you to correct upwards curve?
Hourglassing becomes worst or it is stable?

Thanks to both of you for your answers.

I actually have kind of difficulties to understand my case...lol.

At the very beginning I only felt a plaque at left side under glans. Thanks to traction it softened/disappeared and my curve went away.

It is true that in that meantime an up curve was appearing after disappearing with traction but I don't know if that is due because I was still in the acute phase or because it was a remodelling of the plaque due to traction.

But indeed: hourglass was slowly appearing all around the penis under the glans.
Does that mean that there was an unpalpable plaque since the very beginning all around (so even right), that remodelled because of the traction? Today I can't still palpe anything, but the leftovers of my original plaque (left side).

So... is an hourglass only due to a plaque ?

Hi alex_be,

I did not yet use RestoreX and only used manual modeling. What I can see is that the progress of the disease was not a clear Active Phase then Chronic Stable Phase for me, but more nuanced.

First I felt the Plaque and Pain. Then the plaque became bigger. I think this was the proliferation of fibroblasts and inflammation of the initial trauma, etc. c.a. 6-12 Months

Then my glans shifted up into a slight curve and started to hourglass (bottleneck) and shorten my penis. I think this is when the fibroblasts differentiate into myofibroblasts that constrict. It took c.a. 6 additional months for full constriction.

At that point I think my body said, "ok good enough, the danger of this would is resolved. Ready to release the worker and clean up at much is necessary.".

Then the hourglassing became progressively less noticeable over a course of 18 additional months, and the plaque changed its density a bit. I think this is when the tissue remodels into final scar tissue, e.g. the collagen matrix reaches its final form and the my fibroblasts become dormant or even die off. I think in this phase the plaque also might calcify for some patients and for others not. In my case it never did, but again maybe it will over time. I have a US scheduled next week so let's see.

This progression follows what I have read how other hypertrophic scars develop in the body. So I don't think that Peyronie's is special anymore, but simply a scar formation inside the penis. The severity of the scar depends on the biology of the patient similar like some people get thin scars, some get hypertrophic scars and some even get keloids.

Interestingly I only have a dorsal plaque, but it created an hourglass shape. The plaque is palpable and confirmed by two urologists, but it was never visible on an ultrasound. My assumption is, that as I have quite some girth in the middle of my penis (oval shape) the girth counteracted or hides the bend and therefore the curve appears more as a hourglassing instead of a clear curve). You can DM me if you are interested then I can share some pictures.

So the clear Active / Chronic Phase Model is a very simplified view and all your examinations could simply be still part of the natural progression of the disease. I think the Active / Chronic phase model makes sense for a physician when he first sees a patient (do we expect larger changes of curvature before surgery), but it is incomplete or too simplified when looking at it from a patients lifetime perspective.

Also I wonder the long term effects of RestoreX. e.g. I did not yet see many reports from patients that use RestoreX for years or use RestoreX then stop using it for years, etc. Hopefully the medical professionals will come with long term follow up studies at some time so we learn if traction is a permanent treatment or not. e.g. on related conditions Dupuytren and Morbus Lederhose stretching is not really working long term.

Did you already check the new hourglass deformity with a Urologist e.g. with Ultrasound? Maybe e.g. part of the plaque got calcified or something different happened just via the natural biological progression.
