Peyronie's at crus of penis, or not Peyronie's at all?

Hi. I'm new here.
Have been browsing the forum in the last two days. Very grateful to the people who set this up and everyone who contributes.

I'm 36, living with my girlfriend. I hurt myself masturbating about 7 months ago. Since then I've been in pain (flaccid and erect), my erections are less rigid (softer than before mostly on the right side), my erect penis is thinner, shorter, exits the body at a differerent angle than before, and with much less glans engorgement.

Early Doppler ultrasound revealed no problem, but just over a month ago I had another Doppler ultrasound (referred by Prof. David Ralph, London) which revealed a thrombosed deep dorsal vein. There is no blood flow in the proximal third of the DDV. Instead, the DDV now connects to a superficial vein on the right side of the penis, and empties the penis from blood through there. Prof. Ralph says that the proximal third of the DDV is gone, so there's no point in taking anticoagulants at this stage.

Prof. Ralph said the I have no Peyronie's and no venous leak (based on palpation, doppler ultrasound, and an NPT test), and prescribed Tadalafil. During the ultrasound test, 20 mcg of Caverject induced only a very short-lasting erection, but Prof. Ralph says that it's fine.

I then had an online appointment with Dr. Francois Eid for a second opinion. Dr Eid said that such changes in the shape of the penis can only be explained by Peyronie's disease where the scar is forming throughout the shaft, and in the crus of the penis (inside the body). He says that Doppler ultrasound can miss Peyronie's scarring of this type and in these locations. He suggests waiting another year, and seeing a urologist for palpation once in 3 months. He told me about traction therapy but actually said he's not sure if it's helpful at all, or maybe even harmful (at least in my case).
He suggested anti-inflammatory medication and heating up the penis to reduced the pain.

I'm wondering if anyone here has experience with:
A. Peyronie's not detected in a Doppler ultrasound (is it usually detected in later stages of the disease? is an MRI better at detecting it?).
B. Peyronie's scarring in the crus of the penis.
(I tried the search tool of the forums, but I get too many results with the word "crush" when searching for "crus", so it's hard to find relevant results).
C. Thrombosis of the deep dorsal vein (I'm getting the impression that it's rare).

Wishing you all good health!

Here are some more details about my case if anyone is interested:

1. 8 months ago I hurt myself after prolonged (40 minutes) masturbation. I used my hand to straigten my penis when ejaculating (I have a very slight congenital downward curve), and I think this movement injured some penile veins.
2. For 48 hours my flaccid penis was much smaller than usual.
3. After that, for 3 weeks I had extremely painful nocturnal erections at the distal third of the shaft. Sometimes with swelling (but normal color).
4. Then I took Etodalac 600 mg (anti-inflammatory) for 7 days and the pain was gone (already after 2 days). My flaccid penis returned to the normal size and shape.
5. I tried masturbating again and it was so-so rigidity-wise, but I saw it as an improvement.
6. The day after, the shape of my penis (flaccid and erect) changed and it remains like this until now.
The change is at the angle where the penis exits the body, and narrowing, mostly of the right half of the penis. Mostly at the base.
A bit of shaft length is lost too, and I have a smaller glans during erections.
7. At the same time I started having pain in the right side of the perineum.
8. Doppler ultrasound with 10 mcg of PGE1 revealed no problem.

6 months later:

9. I had another Doppler ultrasound with 20 mcg of Caverject. It showed a thrombosed deep dorsal vein. There is no blood flow in the proximal third of the deep dorsal vein. Instead the deep dorsal vein connects to a superficial vein on the right.

10. I'm still in pain most of the time when flaccid. My erections are thinner, softer, a bit shorter, with a relatively small glans. The pain is at the distal part of my deep dorsal vein (which is palpable and hard now, both when flaccid and erect), and on the right side close to the base (this area used to be quite thick - thicker than the left side, and now it's bit thinner than the left side and painful).
Age 36. Pain and ED since April 2022.
A blood clot destroyed part of my deep dorsal vein (verified with Doppler US) and caused a hematoma.
Cialis helps.
Not yet clear if I have Peyronie's, but Dr Eid thinks I probably do. Now "wait and see".

Hi Bela,

Two things.

1.  You can eliminate words when you search by typing the minus sign.  So for example, if you search as

  crus -crush -crusty

you will get all the results for crus and there are many.

2.  Just checking but if Bela Szabo is your real name, please change your name on the forum.  We don't use our real names.

Best regards,
PK
Age 64, Peyronie's history 4 years, left side hourglass, 20-degree bend to left, no ED

Hi PK,

Thanks for the search tip. It is very useful!

No, it's not my real name. But I changed it anyway.

Best wishes,
K
