Any insight would mean a lot to me.
« on: February 15, 2021, 07:38:23 PM »

Good evening.

I am new on these forums but I been “lurking” since November, when the onset of my symptoms began. Long story short I am a 30 year old male recently married to a extremely supportive woman. I have had lifelong Erectile Dysfunction and have been prescribed Cialis and Viagra in the past. One night in November I took 15 mg of cialis. The next night i took 50 mg of viagra and woke up with an approximate 20 degree bend to the left. Fearing the worst I made an urgent appointment with a local urologist whom “specializes” in Peyronies. Prior to my appointment and currently I have frequent urination, dribbling and a slight split stream towards the end of peeing when my flow slows down.

I had an ultrasound done about a month ago to which the urologist only injected me with a small dose of medication to avoid prapism. This resulted in an approximate 40% erection which he still did the ultrasound on. The results of this ultrasound were that there were no “palpable plaques” but that I had arterogenic erectile dysfunction, as my right cavernosal artery is barely functioning. He said I had 3 left to right perforators supplying my right artery. He basically said this is something that I “must’ve” had my whole life, despite my new 20 degree bend and new urine / urethra issues. I disagree and I think the lingering cialis / viagra combo must’ve injured me in some way..

He then scheduled me for a cystoscopy which yielded normal results. He seemed to blow off my worries and concerns of urethra trauma to “anxiety” and suggested pelvic floor physical therapy.. however, I am certain there is something else going on as my urine stream now sprays at about 2
O clock towards the end and I never had that prior to my new penis bending.

Sorry for the lengthy first post but this is testing my mental strength. I have a very supportive wife whom I don’t deserve and have stayed positive. With 50 mg viagra I can still have sex which is obviously helping me stay positive. I honestly can get over the bending and patronizes but the frequency of my urine and other urethra issues is concerning to me. Could this possibly be utheritis?

Thank you all in advanced it is greatly appreciated.

It should also be noted the when I asked my urologist about Pentox be snarkily replied “that’s not even a thing”.

I will let the experts chime in but I would imagine that you do not have Peyronie’s as the Urologist seemingly found your issue(s) and no scar tissue/plaques I am assuming as you/he said.

Do you suspect that combo of meds may have given you nocturnal priapism as you went to sleep, which caused damage and the bend?

I would most definitely get a second and third opinion if this Doctor is being dismissive.

I can tell you that I have dealt with psychogenic urinary retention and some deal with psychogenic incontinence.

The bladder is not completely autonomic like our breathing, it is in part, reportedly, connected largely to our emotions (so I have been told and experienced).  So stress, anxiety can play a role in some in regards to urine function.  Pelvic floor exercises did help me but I had the opposite issue, retention when psychologically anxious/stressed —everything just tightens up down there.

So you may want to look for a Pelvic Physiotherapist that deals with men.  They can do a physical exam and tell you if there are pelvic floor issues.

You may want to also keep a journal and monitor your stress levels and see if there is any connection with urine function at times of heightened anxiety or stress.

As for the bend and clinical findings, as I said, definitely get a second and third opinion.

I have the opposite issue that you are dealing with, a palatable bump but no bend (knock on wood), pain or other issues/symptoms.  Still waiting to see a Urologist.

Best of luck with things.

Thank you so much for your time Andy. Yes, I do suspect a nocturnal priapism, which was not painful and went away on its own. I also wonder if maybe sleeping on my stomach with an erection could’ve caused this. Do you think pelvic floor physical therapy will really help me? I think I’m having issues with ejaculation too. Not retrograde ejaculation, but rather my initial “bust” squirts out as normal then the rest just “seeps” out. I would think this has more to do with urethra issues as opposed to pelvic floor.

Thanks again!

Hey neguy92.... if your doctor told you that, then try to find somebody more knowledgeable on the condition. Also, are you just taking that to manage your Erectile Dysfunction? you should try cialis 5mg, and possibly use it with l-arginine. It's safe up to 2.5mg and there are medical papers on it. Using a VED and traction to help out with your curvature given that it's so minor may be of great benefit.

What do you want to do in terms of Erectile Dysfunction treatments?

I think but am not sure that Pfact meant to say L-Arginine is safe up to 2.5 grams.

Thank you for your detailed post.  That is what we prefer.  Be sure to read this carefully. https://www.peyroniesforum.net/index.php/topic,3180.0.html

The only thing I know that could make you bend during an erection is if one side either did not fill or the tissue did not stretch.  The side that did not stretch would cause a curve, just like putting a small piece of scotch tape on a balloon then blowing it up. If the tissue did not stretch, that means it is non-elastic scar tissue BY DEFINITION.  Ask your urologist what else could keep your cavernosa from stretching.  I am sure he will have no reasonable answer since there is none.

Thank you pfact and Hawk, it really means a lot to me.

Hawk I asked my urologist this exactly as my RIGHT artery is dysfunctional but my erection curves 20 degree to the LEFT. It would make more sense if my erectiom curved to the RIGHT as that side does not adequately inflate. He is adamant I have “no scar tissue” just because it did not show up on the DDU.

I have read the intro sticky you posted and have been in ALC, arginine, coqQ10 for about 3 months now. I am also in the process of trying to get my hands on pentox.

I am primarily concerned with urinary issues / frequency / dribbling as I have never experienced that before and this is new which I did not notice until I noticed my bending. Is there a chance that pentox and VED could help this?

Thanks again!

Trying to stay positive,

I am a bit puzzled by the urinary issues.  Typically I would suspect a stricture in your urethra, but not only would he have seen that with a scope, but the scope would also have been difficult if you had a stricture.  Other than Andy's interesting post, I have no other ideas on what is interrupting your urine stream.  I am not saying plaque on the tunica cannot do that, but it is NOT common to Peyronies Disease.

Question: do both sides of the penis get equally hard with an erection?

Thanks again Hawk. I’ve gotta say it seems those on these forums care more than my doctor did. Not that he wasn’t a decent human being, I just feel like I was always rushed out the door before I had a chance to ask all my questions.

Anyways, yes it’s frustrating. Going pee about 15-20 times a day had already gotten old. I plan on seeing a structure specialist to see what he has to say. Maybe it’s utheritis?

Yea, both sides of my penis DO get equally hard with erection, and especially so with viagra. Which makes me think if I truly had a mechanical issues with a “barely” functional right artery then no amount of viagra would even help sustain an erection.

Something else I’ve noticed since November is the bottom portion of my glans is somewhat blotchy with white spots; which leads me to believe that area is not receiving sufficient blood.

Against sorry for the long post but thanks for all that you do, it means more to me than you know.

Thanks again, Hawk. I’ve gotta say it seems those on these forums care more than my doctor did.

You can learn a valuable lesson early in life.  Doctors know far more than you or I, BUT where the system requires them to see 5 patients an hour, they do not get to ponder over your case long, and it is possible that you can know far more about your specific case spending 10 weeks studying and pondering it than a doctor can in 10 minutes.  The Dr. may also know more about your case, but if he does not have time to communicate with you, his knowledge is not worth that much.   So yes, in every ailment you will ever have; you will be best served if you become informed and an active partner with your doctor.  Finally, keep in mind that a general urologist treats dozens of diseases in men and women and male sexual function is only a small part of that.  Peyronies Disease is a smaller part yet and not very rewarding because there is not much they can do other than prescribe pentox and hope for the best.  Consider finding a Urologist that specializes in Peyronies Disease or a doctor who is a male sexual health specialist and you will get some better answers.

If both sides of your penis get equally hard, then we know that any bend is not caused by dramatically unequal erection in one side. That means the tissue on one side is not stretching as much, and that side is cinched down.  Only one thing can cause that.  The tissue on that side has less elastin, which is characteristic of scar tissue.  That is true of all scar tissue.  That is why burn victims have to go through painful stretching as they heal, or the scar tissue would draw their limbs and body up and limit their movement.

NEVER apologize for giving too large or detailed of a post.  That is what the forum is for.

Thanks again Hawk. Do you think my some miracle my urinary issues may get better with Pentox and possible heat therapy? I think NeoV had similar issues and I believe his have subsided with time.

Since no one really knows at this point what is even causing it, I think there is at least some chance it will get better on its own without any help.  That is not much to go on however.  You cannot assume because someone shared a similar symptom that they have the same thing and even if they do you cannot assume it will follow the same course or respond in The same way.

Thanks hawk! I have a new appointment with a urethra stricture specialist. Seems a little redundant since I’ve already had a cystocopy deemed “all clear” but I think a second opinion would be suiting, at least for my mental health. I do have red irritation and my “pee hole” is visible swollen. Maybe this guy can diagnose me based upon that?
