Priapism from Pentoxifylline and Cialis

Hi, I am a 28-year-old based in the UK under urology for some intermittent psychological ED and firmness/narrowing at penile base. Generally however, my erectile function has been good and I could generate full erections alone or with a partner on demand allowing multiple episodes of sexual activity within 24 hours. My question is, has anyone experienced priapism (prolonged, unwanted erection) whilst taking pentoxifylline 400mg three times a day and cialis 5mg once a day?

I was on this regimen from my urologist, and started to wake up at night/in the morning with very rigid, full-size erections that would tend to persist if I remained lying in bed. Once I got up and either went to the toilet or stood up for a few minutes, the erection would subside over the course of maximum 5 minutes back to flaccid state. I never experienced any obvious pain and never required medical intervention to return the erections to flaccid. This happened about 10 times over the course of about a month and a half. Unfortunately, I have no idea as to the duration of these episodes as I was asleep prior to waking (therefore I worry the erections could have been persisting for a while in my sleep and causing potential damage from ischaemic/low-flow priapism).

Since these episodes, I have subsequently lost essentially all erectile function. With a lot of work, I can generate something resembling an erection when I am alone, but it is by no means a 100% erection. Furthermore, I need to be sat down and if I stop stimulation it will fade away quickly. My flaccid penis feels less 'elastic' and just not as I remember it. Furthermore, I have no nocturnal or morning erections anymore and this has been the case for about a month and a half.

Has anyone else on the pentoxifylline + PDE5i (viagra or cialis once daily) combination had a similar problem and, if so, has there been any improvement?
Furthermore, does the description of the overnight episodes seem to fit with low-flow/ischaemic priapism, or would there typically be pain and more difficulty in relieving the erection once waking?
I am awaiting results of a penile MRI and USS (both done without injection in flaccid state due to worry over priapism from Consultant) but the initial informal feedback suggests they have seen nothing overtly abnormal.

If you can get full hard erections without Cialis and it seems you might be on the verge of priapism, I would either cut your dose in half or eliminate it and tale L-Arginine.

I haven't developed priapism as you have described it, but always after using sildenafil (generic Viagra), I would have what I'd call 75% erections for most of the rest of the night, which was very concerning. I switched to 5mg tadalafil (generic Cialis), daily, but didn't get the desired effect. I did some experimenting and found that (on days when sexual activity was anticipated) if I took one in early afternoon and another a few hours later, that all was well and I didn't have prolonged erections all night.
Three years since symptoms and diagnosis. 70° upward curve. After four cycles, improved to 55°, but up to 90° 6 months later. December '23, one treatment cycle by Dr. Trost. Curve reduced to approximately 15° after that one cycle.

I seem to be in the minority here but when my Peyronies was first diagnosed back in March of this year, I was immediately put on 5mg Cialis. It gave me uncomfortably strong nighttime erections and burning sensations slow began to develop after that point. Even after reducing down to 2.5mg and then again to 1.25mg.

So despite Cialis being touted as the go-to medication, with the most amount of evidence for helping, in my case it wasnt the case. Maybe because my bloodflow was perfectly fine?

Anyway, I switched to Pentoxifyline in June. I know there isnt much hard data around it really helping but the absence of cialis reduced the pain. Due to how it works, I very much doubt it would be responsible for a priapism.

Interesting you mention pentox. It also helps with erections. I personally felt bad whilst on the medication.... Lightheaded mostly... And never tried it with cialis...

It is not for everyone. This happened to me as well. I even mentioned it to one of the urologists if this might have been what made my Peyronies begin. He said most likely no. What happened was that I woke up with an erection as hard as steel. Never been that hard in my life. It did not go down. Took around 30 minutes for it to go down. Entire penis shaft was sore for roughly 3 days after.

I googled this and stumbled upon threads on reddit and other forums where I have found that the same exact thing happened to other guys as well.

Now here's the thing. During my episode it took around 30 mins for the erection to go down so at first I thought okay, it was definitely not a priapism, But then I thought, who knows for what amount of time it was that hard during the sleep?

That's a question I have been asking myself for 5 years now. What if I was that hard for over 4 hours in the sleep and it was infact a priapism episode? I have avoided Cialis because of this. Sildenafil works better for me and the risk of priapism is much lower due to it being out of the system much faster that cialis. I can't stop thinking about if this episode was what gave me Peyronies to begin with.
