Norm your info is not complete is not that basic. There are several techniques used inserted in 5 basic approaches:
- Excision and Graft
- Incision and Graft (Example: Egydio Technique)
- Excision and plication (Examples: Stage Technique; Modified Nesbitt; Original Nesbitt,..)
- Incision and plication (Yachia Technique, Levine TAP Technique,...)
- Nonincisional and Nonexcisional plication (Corpora Rotation Technique; Tom Lue 16 Dot technique,...)
Only the first two are lenghtening approaches, the others are shortening approaches (not counting the rotation technique). They can use absorbable and non absorbable sutures, they can opt to deglove or not deglove. Besides this you have to take in consideration whats the type of curvature (
Dorsal,
Ventral, Biplanar, complex including shaft rotation, if it is Peyronies,
congenital, degree...). Why? Correction a curvature on the
dorsal side is not the same as correcting through the
ventral side. Ventrally you have the Corpus Spongosium and Urethra, dorsally you have the most important Neurovascular bundle. The question of degloving the penis can actually comprise a miriad of reasons, like the type of approach, particular case, aesthetical, full access to penile shaft,......
The STAGE technique can be considered a modified Nesbitt Technique, but honestly i would consider it a bit more than that. Whats the diference?:
- Nesbitt Technique (bigger tunica elipses are excised, can go to 1cm width until curvature corrected. Both tunica inner and outer layers are implicated. This creates a point of maximum tensile force specially during recovery)
- STAGE Technique (Small eliptical excisions are made (3x2mm) until cuvature is corrected distributing the tensile forces. A major benefit is added. This technique only excises the outer layer not rupturing completely the
tunica albuginea, in some way is similar to Dr. Levine TAP Technique, actually i had the oppurtunity to discuss this with him.)
There are a lot of more details but would be to much text :-) I have to be honest i took in consideration all these details and tried to view them in my personal case.
