So its been about 4 years in on my peyronies.  Started with erection pain which led to a dorsal upcurve. (pic1)  After 3 xiaflex injection, the curve was more manageable. and I was at peace with the disease and my penis. (Pic2)  Last year, I noticed an indentation on the rear left of my penis, next thing (pic3) i'm hour glassed and now an inch shy.  The lightening struck twice on me!  The hourglass is not that big of a deal but I want my missing inch back before it all happened!  Currently I'm at a solid 5.  I used to be 6, it makes a difference!  Any advice for the hourglass?

How about you guys? Ever have to go through a 2nd round?

Was that hourglassing gradual? Or did it just happen all of a sudden? The 2nd photo looks like great results compared to the first. I'm pretty sure xiaflex does not do much for hkurglassing.

We're you pretty active sexually after the 2nd picture?

the 2nd round seemed to com on pretty quick.  No pain at all.  Yes i was sexually active, also (and still do) masturbate several times a week.  Not so much sex since losing the inch.  Pretty self conscious.

How quickly did this occur? Do you notice any bumps/ridges/indents along the side of your penile shaft where the hourglassing occurs? How much girth did you lose at the site of the hourglassing? Length should not have been affected, but girth significantly. Did you measure?

as to the girth at the base, i was 5 around before now at the base im about 4.5 but at the widest part i'm a bit over 5, so a bit thicker up top and thinner at the base.  I did see an indent at the beginning. no bumps. its gone now. see pic below. 

A note on pics.  Guys, we're adults.  Get over your shyness on the pics, as they say it tells a thousand words.  I've probably taken a hundred pics by now to chart my progress or lack thereof.

The hourglassing doesn't seem noticeable in that pic. And I know you are self conscious losing an inch. But I been 5 inches my whole life and looks like I'm slowly losing some length too.

No the dent started it then the hourglass followed.  Hey 'im not knocking 5. that's actually average. like everyone else here, was just used to more.  I think they key is to just accept and not obsess.  Tell me about what's going on with yours.

The xiaflex helped my bend but i cant see it doing anything for my narrowing.

Hey bud luck. Is your issues more the erectile dysfunction? Or the narrowing and dents? Because to the untrained eye it looks pretty good. Does your gf notice or say it feels different?

No problems getting hard.  It actually gained a half inch on girth but lost a bit on girth towards the base.  She likes the thicker part lol.

She said that doesn't notice the difference, but I do. I just losted penis volume, it creates unestability in the penis when is getting hard, once is erected  it stabilize, I don't have the hinge effect when is erected. For the last two weeks my nocturnal erections are weak or non-existent, maybe is due to hormones? Stress? . My erections overall are worse for the last two weeks.

First of all - thanks for sharing the pics.  I need to get some up here. It just seems like they help us to know we're not alone when you see evidence of other guys who look very similar.

Second - Yeah, I'm on my second injury.  First one was a pretty severe bend to the left. Practically a 60 degree curve.  I had it down to below 25 degrees, to the point where Dr. Trost said in a video consultation that Xiaflex wouldn't be an option but he'd help me get a RestoreX if I wanted.  Decided to keep manual traction and my supplements.  Then about 3 months ago I noticed a new plaque on the right side, actually pretty high up where cock meets pubic bone.  And am now twisted to the right. Not a new bend per sé, but a definite twist.  When I'm erect you can see the whole glans twisted to the right and my urethral opening ("piss slit") no longer up and down, but now diagonal.  Hate it.

And I totally get what you say about lost length but increased girth. Seems like the law of conservation of matter would mean that if you lose an inch in length that same penile mass/volume now has to exist in a shorter tube, so the tube gets wider. At least that's what makes sense in my mind.

Here is something else since the hourglass set in.  My penis is weaker on the left where it is more pronounced.  I can easily bend it when hard to the left but not to the right.  I started sleeping commando because a night erection hurt getting bent the wrong way.

Yeah, I've noticed that there's a difference between the "hinge defect" and the "hourglassing." With my hinge it felt like a literal hinge that my shaft would bend to the left easily. Made me scared for penetration, like it was so unstable it would buckle and / or "break" again at any minute.  The hourglassing seemed like more of a visual defect - not fun to look at, but didn't seem to cause much instability.

Sorry to hear this.. I saw your earlier feed and felt so optimistic about your result w/ Xiaflex.  I have some narrowing too and that's so frustrating with the lack of things to fix it. Are you going to livr with it or try surgery? A little viagra helps me a little on the narrowed area for stiffness

I'm not on any program but wide open to suggestions.  I saw where castor oil could help and began applying it.  Really didn't do anything.

Any thoughts on getting an implant so that you don't have to deal with the endless cycle of Peyronies flares and the consequential depression that follows after?

Hourglassing is the tissue not expanding normally in that area.  It's not easy to fix but you should definitely give VED a try to help at least try and push blood to those areas to make the tissue have blood flow.

What treatment did you use to achieve the last pictures result? Looks good.

OP, have you considered that your newfound girth at the glans end is distorting your perception of an indentation?  For instance, I "suffer" from something a medical journal calls "Circumferential Acquired Macropenis" whereby the tunica loses elasticity and girth expands over the years. I have noted that twice in the evolution of this condition, where I have deep circumflex veins in the penis, there would be an indentation when erect, as the tissue around the vein wasn't expanding (or losing elasticity) at the same rate as the rest of the tunica around it. When I first saw the first "indentation" I freaked that my Peyronies Disease had returned after years, but over more than a year, that section filled out and caught up with the surrounding tissue.

I'm just wondering if your girth expansion isn't similar, and if you're noting an "indentation" that isn't necessarily Peyronies Disease, but just the border of expanded vs. unexpanded tissue.

Just a thought.
