I had a lot of misunderstanding on this topic early on. I recently returned from Dr Trost's office for my third Xiaflex treatment. You can see in my post history that I have experienced with erections and ejaculations during the recovery treatment and learned a lot through trial and error.

During my second visit, I briefly touched on the topic and was told that "erections are good" as a natural way to stretch/model. But I did not press the topic.

During my most recent visit, I explored the topic more deeply. When they talk about abstinence, they want to prevent against blunt force trauma, such as can happen if you inadvertently pull out and miss the re-entry during sex. There's no doubt the penis' structure is physically weakened for a few weeks. Causing a "fracture" (what a needlessly scary term by the medical community) on a non-wrapped erect penis will lead to uncontrolled swelling due to the high blood pressure inside the penis. It's not a pretty situation, as I have found first hand. And since it's unlikely anyone is having sex while wrapped, it's best to just forget about penetration.

That leaves other types of erections/orgasms. First of all, nobody can control what happens during the night, so erections are happening regardless. So long as you are properly wrapped, any further internal stretching or tears, which is exactly what happens at the clinic post injection with the modeling, will lead to controlled swelling.

I was told that manual or oral stimulation is fine as soon as I felt up for it. I'd highly recommend doing it wrapped the first time (manual, vibrator, mind power, whatever works) so you can assess how you feel.

Finally, most doctors only do a few Xiaflex injections so they have little experience and even fewer post treatment feedback. Dr Trost and his staff perform multiple daily. Dr Trost himself authors and publishes protocols for others to follow. I trust his advice.

Piggybacking on what @richestorags said, I was over confident during my recent third treatment. Three full days after the second of two injections, during my daily re-wrapping, I wanted to see what it looked like erect to assess progress. Well, when I got to 9/10 hard, I felt the sharpest ripping pain that bent my knees while my dick doubled in size in four seconds. Absolutely terrifying. Luckily I had the wrapping material nearby, otherwise I might have ripped fore skin from the sudden swelling. It took all the concentration I had to hastily wrap with shaking hands, tremendous pain, and a bit of panic. I did this in the evening but was unable to sleep for the next 20 hrs due to the pain and it set my healing back at least ten days.

To be clear, the error was not the erection. It was the lack of wrapping. Wrapping and controlling the swelling and internal bleeding is key to the whole operation. Even a somewhat loose wrap will contain the swelling, which dramatically reduces pain, improves sleeping, and speeds up healing.

It makes a big difference because it's harder to wrap an uncircumcised penis due to the foreskin sheath. So it's harder to manage swelling. Also, the adapted wrapping technique then causes abrasion on that sensitive skin, leading to longer healing time. Finally, it's both painful and technically tricky to re-wrap at home because you need a third hand to retract the foreskin while you are swollen.

I can definitely relate to what you're going through. I was on Zoloft (Sertraline) for a long time, and while it helped with my depression, it completely wrecked my ability to get and maintain an erection. In the beginning, things were fine, but over time, it became impossible to stay hard, and my libido just disappeared. It was frustrating because, like you, when I wasn't on antidepressants, I struggled with even getting an erection at all.

The only thing that has consistently worked for me is Cialis (Tadalafil). It's been a lifesaver. Unlike Viagra, which lasts a few hours, Cialis stays in your system for a long time (up to 36 hours), so it gives me more confidence and takes the pressure off. It doesn't fix everything, but at least I don't feel completely powerless in the bedroom anymore.

As for switching antidepressants, Wellbutrin (Bupropion) is known for having the least sexual side effects, and some people even report an increase in libido. I've also heard that Viibryd can be unpredictable when it comes to sexual side effects—some people get crazy frequent erections early on, but long-term use seems to cause ED in others. Maybe it's worth talking to your doctor about switching to Wellbutrin or adding it to your current medication.

Have you tried Cialis or Viagra yet? I know your urologist said you're too young, but that honestly sounds dismissive. If the medication is causing the problem, you should be able to get something to counteract it. If your doctor won't prescribe it, there are other ways to get it from a reliable source.

You're definitely not alone in this. A lot of us dealing with depression and antidepressants struggle with ED—it sucks, but there are ways to manage it. Let me know if you want to talk more about it!

Hows your erectile quality? If you're having Ed or subpar erections... In my case I had funny bands running along the top of my penis. I can't remember if it was during erect or flaccid because I'm abstaining from months on end. In my case it was lacerations in the tunica in the tunica albuginea. I'm not saying thats what yours is, I'm just saying if erections are poor, there is healing in it. But the thing that strikes me as different is that you had no pain. Possibly my situation is different than yours. Please take care of yourself. God bless.

I have experience using the PMP and all yo need to do is to take some time off and things will heal. I also had some small like sores on my glans that healed. It just took some time in the chamber and your body does get used to it. I would be careful using any tape on your glans as it may stick when you try to take it of.

The supplements that you are taking all promote nitric oxide production and some will help in that area. I am puzzled about the cold therapy though? I do know when I was in pain I used heat therapy for a great amount of relief. I am glad that you have found a solution and wonder if others have used cold therapy instead? Please don't waste your $$ on shockwave as it really doesn't heal peyronies as many can attest to that.

This is a message of hope for all of those struggling with mild Peyronie's disease and or erectile disfunction

Via an accident with a Bathmate* penis pump several years ago- I developed chronic erectile dysfunction along with mild Peyronie's. I have tried and considered many different methods of treatment. I'm 36 years old.

I recently had what could be considered a mild flare up of the Peyronie's- so I decided to try icing my penis with a bag of frozen peas, then made the upgrade to an actual ice pack a day or so later (haha). All I have been doing is consistently putting an ice pack on my dick for about 5-10 minutes- 2 or 3 times a day on the spots I feel I need it. That's it.

 I also have been taking supplements for improved circulation and libido- but I do believe the cold has contributed the most healing. My results after about two week's time are as follows:

Improved skin texture on penis (very soft)
Much firmer erections
Hard flaccid nearly gone
No pain with sex
Significant improvement of premature ejaculation- along with increased sensitivity (this has truly felt like a miracle because I've struggled with premature ejaculation my whole sex life)

I also want to include that I have icing my taint which effects the prostate in a positive way I feel. While sexually active it's very common for our prostates to become inflamed (which can cause premature ejaculation) so I do believe that the cold has been taking down the inflammation in that area.

The supplements I've been taking are L-Citrulline (1500mg 2X day), Zinc pollinate, B6, CoQ10, D3 with K2, Pine pollen (one scoop from the bag added to a smoothie) and Beta Sitosterol (for prostate). I do believe the L-Citrulline has helped significantly also for erection quality

I can't say this will work for you as it has for me; and I'm not a doctor- but I do have extensive experience trying different supplements and I know my own body very well. This protocol is working for me and if this can help just one guy on this forum from losing their minds and getting their mojo back, well then- I'll be happy. I was nearly about to try the P-Shot and or shockwave therapy $$$$- but I'm so glad I decided to go this route first.

I bought a Restorex and have used it about 3 months. I find the base very sticky to the skin so the skin pulls tight unless you maneuver the Restorex and skin just right. Then the angles are limited to basically top, bottom, or the sides. I also find the length required to be able to capture the glans is pretty long. Anyone with less than 4.5 inches under glans when stretched out is going to have difficulty. And, the tilting mechanism pulls too much against the skin for the glans to stay clamped when you don't have the length.

Too me, the basic idea is perfect. Stretch then bend to one side opposite of the curvature.

If I had it to upgrade I would 3D print the base with a circular groove that fits a circular washer type base for the stretching mechanism. That way, the angle could be infinitely adjustable with just the loosening of a couple of screws. In the process, I would eliminate all of the painful pinch points in the process.

The stretching mechanism would be re-designed to be shorter for the smaller guys and with optional inserts for the big guys. Also, make it more reliable so it does not hang when trying to compress it. Presently, I can get to the 2nd notch and I insert a roll of gauze to enhance the bend effect.

Then there is the clamping latch. Move it to the other side so when you are working to adjust the unit your finger does not push down on the latch and release the glans.

If you're looking for a natural approach, I'd recommend checking out Bryan Johnson's supplementation protocol—specifically his earlier version, before he started selling his own supplements. You can still find it on his website. Many of the supplements he includes are focused on improving circulation, regeneration, and reducing inflammation. Since Bryan rigorously tests and filters for effectiveness, his list can be a great reference for identifying the most impactful options. I personally take some of them, including Astaxanthin, Luteolin, and Omega-3, among others.

That said, if you're at all open to considering medications, it's worth noting that not all antidepressants function the same way. For example, Wellbutrin doesn't cause sexual dysfunction—quite the opposite, in fact, as it can enhance libido. It also doesn't lead to dependency, which sets it apart from some other options.

Additionally, there's another intriguing compound worth considering that currently falls outside natural supplementation and conventional medication: methylene blue. It has impressive benefits for mitochondrial efficiency, neuroprotection, and antioxidant support, while also offering mood-enhancing effects. One of the reasons it improves mood is that it acts as an MAO inhibitor, increasing serotonin, dopamine, and norepinephrine levels. Unlike many antidepressants, it doesn't seem to cause emotional blunting or libido issues. I'm particularly excited to try it myself soon.
