when you say pain during ejaculation, where is the pain? is it where your scar tissue is, is it within the urethra, or rather on the underside between your testicles and the anus where the prostate sits?

if the latter two are the case this could be pelvic floor dysfucntion. it can also explain pain during an erection, pain during urination, erectile dysfunction

orgasm/ejaculation is largely impacted by the bulbospongiousis muscle, the feeling you get from it is actually this muscle contracting. when there is pain during an orgasm, pain when urinating it is often not an infection but rather an overactive cramped muscle caused by either anxiety or physical stuff like masturbating too often in a cramped forced position

I've had pain before directly in my scar tissue. Are you clenching your pelvic floor when you ejaculate? That would be the cause, by doing that, it clenches your penis, which you don't want to do. According to my doc, clenching is one way to hurt yourself. Be mindful of it - you will have to clench a bit to orgasm, but don't clench the entire time leading up to it. Also, maybe look into pelvic floor physical therapy to help loosen everything up. If it is your pelvic floor, this could help loosen everything up a bit.
