Title: New, hourglass shape, not much help from dr, a few questions
Post by: neahcrow on June 14, 2015, 04:48:31 PM
Hello, I am a new member to the forum.  I was just confirmed by the urologist/"Peyronies expert" to have a mild case of Peyronies today.  I've read through the forum a bit and all around the internet before joining or even before visiting the urologist.

Some information about me, my experience so far:

I'm about to turn 30, live in Bay Area.  I do not remember ever having any kind of trauma, or hereditary predisposition.

About 2-3+ months ago I noticed I was having trouble getting erections at times or especially maintaining them, they didn't feel as strong either and I didn't have the same control.  I attributed this to condom use and anxiety with a newer girlfriend.  (For a long period with a serious ex-girlfriend I did not use condoms).  About 2-3 months ago during a practice run by myself I noticed for the first time an hourglass shape/indentation with my penis as I was starting to get aroused/get an erection.  What happens with me is basically an hourglass shape at first, as I get a firmer erection the tip of my penis will bend to the right and down; if I get a strong erection the hourglass shape "fills in/disappears" and I have a fairly straight erection- tilted to the right a bit.

I am able to have sex, although the power and control of erections doesn't seem to be my peak, my confidence had taken a hit, but now that I know what is going on there is less anxiety.  I do not have any pain.  Thankfully, I seem to have a milder case, my main concern is if I'm 2-3+ months in how much worse will it get over 12-18 months?  As I have noticed a progression.  Also:

I have read the suggestions for new members. My uroloist who is a "Peyronie's expert" frankly did not offer much help.  I came in with alot of knowledge already and he did not really say there was anything I could do for improvement or prevention.  Here was the response basically:  He suggested having a wait and see approach, and hoping it doesn't get worse and then:

There is absolutely no oral medication that would make this better. Vit E has placebo effect. Penile stretching devices have been shown to be beneficial but their use is cumbersome and time consuming. You can learn more about these in Google search under penile traction devices. Try avoiding any injury to the penis during intercourse, including force bending.

I've read the "firstt line of defense" suggestions on the website.  My doctor obviously does not think Pentox is effective.  I was looking for multiple opinions of anyone from this site of what I should try first to hopefully find the improvement or stop progression.  Especially anyone who has had the 'hourglass effect' in particular.  And any suggestions for stronger/more control erections with this effect in particular- sometimes I will get very aroused but I won't get a full erection due to it I think.  Its just inconsistent.  I am in a relationship and have begun to talk to my girlfriend about this.

EDIT: some other info- upon initial evalution my urologist didn't think I had Peyronie's  He didn't feel a plaque.  I showed him a picture and he confirmed it and said it must be located in the septum.

Hey neahcrow,

I have a lot of those symptoms as well. For me when I'm getting an erection my lower half will fill up with blood first and it will leave the top half looking skinny and at flaccid girth. And like you it tends to fill up after I have gotten a full erection. My bend downwards seemed to have straightened as well when I am fully erect.

I nor anyone can say if you are going to get better or worse as each case of peyronies seems to be different. In my case it started to get worse around the 4th month. Which is where I am right now. I disagree with the specialist in that he feels that there are no oral medications to improve your condition. I was taking pentox and felt a relief in pain and inflammation. Other members here have also beniffited from cialis and coQ10. They are certainly not cures for most but they do help in my opinion.

Traction ane VED are time consuming but they have shown great and even amazing results for many guys (VED even more so from what I have gathered on these forums). You can injure yourself with these devices, but if you learn how to use them properly it can be very beneficial. Certainly a lot less invasive than surgery. Although surgery is an option you should consider it as a last resort.

I'm of the opinion that "hourglassing" while semi-erect is extremely common in men with or without Peyronie's.  It's just parts of the corpus cavernosum filling up or draining at different rates.

I don't think Steveo that hourglass is normal thing.
A healthy penis will not have hourglass when semi erect. It will fill and drain same everywhere.
This is my opinion in any case.
neahcrow, your doctor is NOT a Peyronies specialist. You must start treatment as soon as you can so find a real specialist close to you here:

Or anywhere else. Just act as soon as you can.

Thanks for your input.  I was actually away with my girlfriend for a vacation and was able to perform fine- thankfully.  I do notice though even in the last few weeks the bend to the right, even when flaccid, but especially when semi-erect is more significant.

I have a question- What does Pentox do exactly?  Fortunately, I have not felt any pain so far.  From what I read Pentox is used for pain and also to improve blood flow.  Does taking Pentox help the shape/hourglass/bend at all or at least help stop it from getting worse?

I don't believe the hourglass deformity is any worse but the bend to the right I have has gotten worse over the last 2-4 weeks.  Noticeable when flaccid and semi-erect.

Pentox reduces the viscosity of blood, allowing to flow more freely. It also, apparently, can reduce plaque load. How it achieves that, I'm not sure. It has been shown to reduce calcification of plaque.

Don't expect Pentox to correct any curvature, you will need VED or traction to try and achieve that.

In my case, Pentox and low dose Cialis reduced greatly the calcification of my two plaques.
Pentox makes the red blood cells flexible, so yes, as Jonbinspain said, reduces viscosity of blood.
It enables the red cells to carry the oxygen to the extremities.

I appreciate the input.  I talked to my doctor and he said he would write a prescription for Pentox if I wanted to go that route.

I have a question- I've read about the side effects, but has anyone here personally had any side effects?  I try to take as little medicine as possible because I always seem to suffer from some kind of side effect.

Since a decent number of people seem to think that this helps I would like to try this medication if there are no major side effects.  Hopefully, it will help my mild ED symptoms and possibly help the plaque I have.

This topic will be closed as an introductory topic, please discuss specific subjects on the adequate boards.

Regarding Pentox side effects, myself had very bad, everything is written as side effects.
This is the reason that you have to go in the way bellow (my opinion in any case):
* Take care that it will be the slow release one
* Start with one 400 mg in the morning with meal, always take Pentox with meal.
* Increase after a week or so to 2*400 mg when the side effects subsided.
* Increase to 3*400 mg per day after an other week, take the third one not late that 6 PM.
