Title: Possible VED injury in 27 year-old - prescribed by Consultant urologist
Post by: Federer1981 on August 01, 2020, 09:23:01 AM
I am a 27 year-old Doctor in UK who has suffered what seems to be severe injury from a medical grade VED I was prescribed by a Consultant back in December. My erections were very good at this point (although could be affected by stress/anxiety at times) but he felt he could feel some narrowing at the base even though he never confirmed any plaque or fibrosis on scan. Therefore he prescribed cialis 5mg once a day a night and advised using a VED once a day for 10 minutes to help stretch out the narrowing and firmness at the base.

The way in which he advised using the pump was:
- pump up to the max size you can tolerate; some discomfort is ok but never pain
- hold at max for no longer than 2 seconds then release vacuum
- start the process again and aim to cycle through 10 full erections (or however many fit in to the 10 minute session)
- 3 pumps every 10 seconds, no faster than this
- check for bruising/petechiae

Now checking this forum it seems like what he had advised me was incorrect, and in fact too aggressive. I followed his advice in that I never held at max erection for longer than 1-2 seconds, I felt a slight discomfort/stretching sensation but I wouldn't say I ever felt pain, my sessions never went over 10 minutes, I never had superficial bruising/petechiae, I never pumped more than 3x/10 seconds.

Just for more information, I was using the Somaerect device from Augusta Medical Devices which is a medical grade VED and therefore does not have a vacuum gauge. He never mentioned anything about vacuum levels, and both him and the pump technicians assured me it was completely safe with no scope for damaging yourself.

Interestingly, I used the pump alongside cialis 5mg for around 2 and a half weeks in December/January and everything seemed fine and I was having some of the best erections I can ever remember. Therefore, I stopped using the pump at that point and carried on. It was only when the Consultant called me for a follow-up in April that he advised I should still be using the pump daily until I saw him in clinic again. Therefore, I went back to the pump for 8 days over a 10 day period at the end of April and it was then that I noticed my EQ suddenly plummet. I was living with my girlfriend at that time and having sex several times a day so I don't know if that was potentially the issue compared to the pump use in Dec/Jan when I was not sexually active. Maybe it was too much in conjunction with the pump.

The symptoms since that use in April have been:
- some soreness R-side of shaft after stopping use of the pump (this resolved relatively quickly in just a few days)
- no spontaneous erections - I was getting these all the time before, at times even from just seeing my girlfriend or holding her hand
- No nocturnal/morning erections generally, and if I do get them, 50% of normal and very short-lived
- contracted, tense flaccid which feels hard
- cold glans. Poorly engorged glans and corpus spongiosum with erections
- very difficult to get erection even alone - takes 1-2 minutes of stimulation to even get to something resembling erection and I have to be sitting or lying down - no chance if standing up
- erections feel less rigid and more unstable at the base
- seem to have lost girth when erect
- lose erection within 10-20 seconds without constant stimulation
- small brown pin-prick mark on left side of glans

My questions are really:
- has anyone experienced similar, and if so, what can be done to try and recover?
- I don't understand why 2 and a half weeks of pumping in Dec/Jan caused no issues but 8 days in April has led to this? What would the mechanism of pump damage even be as I never had any bruising, petechiae, pain or significant oedema?
- I have been taking cialis 5mg daily, l-citrulline 1500mg, ginger 500mg, vitamin E 400 IU, vitamin D 1000 IU and muira puama 500mg but not much has improved - in fact I would say things are getting worse
- I have been looking at proteolytic enzyme supplements such as serrapetidase and nattokinase to try and break down scar tissue/fibrosis as I am worried this is what I may have
- I have an appointment with a private urologist on Monday 3rd August and hopefully will be able to get a scan (previous urologist I saw in July just told me it was all psychological and there is no way the pump could have caused damage)

Thanks, I appreciate any help that can be suggested!

Unless you felt some sort of intense sting or tearing pain, I don't see what you could have done. If you injured yourself with VED, I believe you would be aware at the time. When ever I have had any form of injury it usually feels like a sharp stabbing feeling following by inflamation.

Thanks smashin! Have you always recovered from injuries previously and got back to good erectile function?

I never really had tearing pain or intense sting - maybe once I felt like a sting just below the glans but it was very mild and lasted a second or so.

Hi so many protocols for using a VED, I have used the Soms VED for over 2.5 years, told so many different protocols, my Uro told me to pump to max hold for 10 seconds then release, repeat for 15 minutes, the Area manage of iMedicare pump in groups of 3 until to reach max release and repeat, many on this site, so i think its upto the individuals  to find what works best, i see you use the Soma erect unit, now this has the XL cylinder, so unless you have a large girth i think the Somscorrect unit would be a better start.

Hi, yeah definitely so many protocols. That is what seems slightly worrying as what I was told has differed from what many others have been advised. Nevertheless, like you say, maybe it is a matter of finding what works for an individual.

Which protocol have you used for the 2.5 years of VED therapy? Have you ever noticed a drop off in erection quality (EQ)?

Hi, I have being using many different protocols but i found that pumping up to max hold for 10 seconds, release and repeat for 15 minute session.
I have problems with my erection, i take pills, VED and cock rings, get some good erection but don't last long.
Link to the damage i got with wrong VED.

Hi again, and by "max" do you mean to the point where it would be painful/uncomfortable to introduce more vacuum/size?

Yes that the max (the point where it would be painful/uncomfortable to introduce more vacuum/size?) when i start my 15 minute session after 2 cycles my penis is about 6.5 long, by the end of the 15 minutes i'm at max erection size which is 7 inch, that's it will not gain any more length than this, do not try and go beyond your max penis size.

Federer, it seems your doc was following the protocol in the Raheem study.  Not much research w VED as traction.  Here's a good summary: https://cuaj.ca/index.php/journal/article/download/6205/4365/
With no signs of pain of damage I doubt the Erectile Dysfunction was caused by VED.  What about a complete Erectile Dysfunction work up by the specialist?  Hormone panel, erect ultrasound, etc.?

Thanks again, that makes sense! So did you put measurement gradations on the pump to accurately measure the size or did you just go by appearance? I just kind of went by appearance and assumed it looked about my erect size (6inches) but I never accurately measured so can't be sure it wasn't slightly over

I imagine he was as he is also a London Doctor and probably familiar with Mr Raheem. Thanks very much for that summary, I hadn't come across that before (I thought I had seen most if not all).

It is just strange that the erectile function remained so good after first VED use in Dec/Jan/Feb and then suddenly dropped off in May after using it for just 8 days at end of April. Maybe I am putting too much importance on the use of the pump and it has become a self-fulfilling prophecy. Furthermore, I don't really understand the mechanism of damage it would cause. 8 days use and the timeframe of a few months would seem extreme to have caused fibrosis. I will definitely ask the Doctor on Monday about hormone panels and USS/MRI as he sees fit - I'll keep the thread updated with any outcomes. I am also hoping that taking a rest from things, focusing on other areas of life and leaving my penis alone will be beneficial - hopefully at the age of 27 things will return back to baseline!

Hi,

No worries. Just head this advise from me. It seems that when you suspect peyronies, you are always pushing and pushing to get back to the penis you had when you were 19. If you have a mild injury in the past, it will probably recover fine. If you keep pushing your limits with VER or manual tracking, you will just keep going and going until you eventually do some damage.

It sounds to me, you are going to be fine. You will recover if it was just mild sting yes, as I have in the past. Generally the more pain, the more of a pain it will be to correct with traction or VED.

Please take it from me, just take it easy, don't even hit past 95% of what you believe your limits are. Be careful and I am sure you will be in good shape again.

With regards to a sudden and/or severe drop in erection quality, this will usually occur immediately after an injury whether it be mild or severe. If have had it in the past. It could take anywhere from a day to 2 weeks to sort itself out. So just be patient in that time and don't go crazy with VED. Put your mind at rest, if it goes on for a much longer timeframe, then that would probably be the time to look further into things. Cialis should help you get back to shape.
All the best.

Hi, what i did and do, is fit the rubber protection ring, then with a felt tip pen i mark the cylinder from 3 to 7 inch  from the base.

open link and scroll down and you will see the markings on my cylinder, i only mark at 7 inch  now.

I suffered a similar thing after overpumping with VED for long periods of time
