PMP, is highly regarded and it is suction held you can wear for hours at a time, unlike those that attach w a noose.  If you want to try out an inexpensive version first, try the ESL40.

PMP has the rods and straps option—ESL40 strap only.

Soma correct, nice bit of kit, I've used one now for about 2.5 years, 3 sizes of cylinders so fits most, i also use the Soma erect which as a XL cylinder for the bigger girth.
