Title: Very confused about what might be.
Post by: AndreasB on January 11, 2023, 01:14:19 AM
Hello everyone. I am 33yo from Romania.
I will start  from now to 2014.
A few days ago after a rough sex session I got pain and penis was hurting afterwards even sometimes in flaccid state. For two days I couldn't get erection maybe because of the pain. Yesterday I managed to get an erection without touching the penis while I was giving cunnilingus. The penis head seemed smaller, and the shaft in the upper side seemed a bit narrower

It is a nightmare because it reminds me of 2014 when I had quite similar situation
Now, my penis has been for the past years curbed right 25 degrees and it got up to 37 upwards.
However, after 2015 this situation improved a lot, and I happily got married and got 2 children.
Now, back in 2014 I went to some urologists, the most attentive one told me I have congenital curvature. The ultrasound got some strange tissue on the dorsolateral?! left side, 0.74/0.35 cm. The doctor back then said it is strange since my penis is curved rightwards and upward-rightwards and I feel the pain and the inflammation on the left dorsolateral side.

Yesterday I saw an urologist (medical doctor, very experienced University professor, but not on Peyronie itself) and he told me just to rest that I have no plague. He did me a flaccid Ultrasound and said everything is normal and that I should rest for a couple of days without sex or masturbation.

Today I am seeing an urologist specialised in erectile disfunction and peyronies surgery. Not so many options in Romania.
My penis feels a bit inflammated even when flaccid

Now the question is, what this could be? Pain and inflammation on left-dorsal side but bends upwards and rightwards? And why ultrasound now shows nothing, not even what it showed 9 years ago?

I am attaching photos showing where the pain feels and inflamation

I honestly have no idea. It is bending to the right so in Peyronies that means the damage is on the right side but they found nothing on ultrasound so it is weird.

I think your dick looks symetrically good only the head looks in a weird twisted shape. I wonder if they could correct that with surgery or maybe it does not bother you?

I don't see anything unusual on the left side. And the curving to the right looks more like a congenital curve to me, not Peyronies.

PM me if you have questions.

Thank you. What concerns me the most is the upwards curve which seems to get a bit worse or at least doesn't allow much bloodflow to the penis head. I will attach a photo. But since the pain is new (I'm on the 4th day) I avoid sex or masturbation

Hey andreasB,

I had an injury a couple months ago. Since you say it's been only a few days and you can still get erect I will tell you most urologists will recommend observations for 3-6 months. Stop with the rough sexual activity on your penis until it is healed you can still have sex just be careful. In the meantime I think these may help you:

Traction

PDE5 (viagra, cialis, etc) helps with healing

Pentox (400mg 3x daily)

Antioxidants ( I recommend MitoQ it is more bioavailable than regular coq10 and reversed blood vessel age by 15-20 years)

Hello, thank you, I have seen the second urologist who also performs surgery for peyronies in Romania. He confirmed the first urologist saying there is no Peyronie and if it was, I should notice it after 3 months.
He says that it's a trauma due to sex and I should avoid sex for 1 month and nothing else to do.
I have started 3 days ago preventive already Colchicine(managed to get it without prescription in Romania) and Vitamin E. Maybe Pentox is better?

You might want to try some heat therapy as this has helped me with healing and pain relief.  You can try various supplements as it has helped some and others not.  I would cease your sexual activities and let things heal. Pain is an indication that something is not right whether it is from an injury or peyronies or nerve damage. You pics look normal for someone with a congenital curve.

Should I trust these two urologists? They are the top ones in this small country (Romania) and one of them performs also surgery for Peyronies.
Both says nothing to worry about and just stop sex for 1 month but I feel I need to take some medication

Do they specialize in mens sexual health or are they just general urologists?

Is that shape not bothersome with sex? Looks like it would be real bothersome penetrating with a twisted head like that. Has your penis always had this twisted and bent shape?

Hello, Trapper. First one is a Professor at Bucharest State Medicine Institute, second one is a doctor specialized in sexual health who also performs surgery for Peyronies.

Spooning position is the best, but my girlfriend has insisted lately on doggy which is kinda problematic for me
Twisted shape happened after the pain 4 days ago, but I had the same thing 8 years ago and it improved. Now it happened again

I am getting confused now Andreas. Was your dick in the past straight? Or did it get this shape after trauma? If your penis was in the past straight and now it looks like this then you 100% have Peyronies.

If your dick always had this shape then it is a congenital curve aka your natural curve since birth.

Thank you for this question it helps a lot to understand. My dick was always curved rightwards and somehow upwards, I even got meseasureed 37 degrees angle upwards back in 2014 when I had a similar situation with pain it started twisting. But after a while it got better. I took some Pentox back then.  So after this episode it is a bit more twisted but two top urologists say they cannot find anything nor by touching nor by ultrasound. Although I can feel the inflammated tissue on the left-dorsal side. This is very confusing indeed. Second urologist says it should heal normaly and only after a few months we can say whether the injury transformed into Peyronie. But he things nothing to worry about.
BTW Colchicine is a good preventive option or should I switch to Pentox instead?

Okay, I understand now.

Hopefully someone else can respond the question about those two medications.

I've never had either of them so can't give an answer.

Update - tried to get an erection this morning just to check hands free, but I only managed to get half-erection. During full erection the upward angle is way smaller than during half erection, is this strange, bad or good?

Also I checked more thoroughly feel a small lump right now right under penis head in the left-dorsal side, is it possible that two top urologists and one ultrasound have missed that?
Also, is it possible for this to be a penile fracture and two urologists to miss it?
The two urologists put diagnosis "penile trauma"

Also, I have noticed that if I start getting hard, this vein looks very strange. Pic attached

Regarding the upward angle........  Does it curve upward, or is it straight and points upward? The reason I am asking is for some reason some men are confused that their erection should point straight out parallel to the ground. This is not true. There is clear data that indicates the average for mean is to have an erection that is pointing between 30-60 degrees upward when fully erect.   /

From my point of view it points upward. And a bit twisted
