Title: Helping the PDS survey broken?

I get a 404 error after I click the submit button in the survey at Giving Assistance to the Peyronie Disease Society (http://www.peyroniessociety.org/survey_help.htm):

Temporary bug?

Tried also, got error also.
Maybe the survey is not active anymore

Interesting, I have not ever looked into these since I started helping Hawk as admin and I didn't even know these existed. I focus 100% on the forum and not so much on the general web site. But I can look into these.
