Peyroines is highly variable in its course. Some guys have mild cases, some severe. I have a photo in my phone, which  I got off the internet, and the guy had a 180 degree bend. Can you imagine a 180 degree bend? Holy crap. I thankfully never had that.

And some guys improve naturally with time anyway. Mine improved a lot with no treatment over 5 years. But, I still want to get Xiaflex going to see what further improvements I can have because Dr Lue recommended it. And some traction to to get at least some of my length back.

I read the summary and it seems to say some, but less, improve the w hourglassing and length than curvature.  Did I read it incorrectly?  Guess I did, Trost says hourglassing not effectively treated w xiaflex and that there has been some success w traction.

I don't understand why it woudktn have an effect on hourglass deformity

Got it.  I'd try traction, VED, pentox and low dose nightly generic cialis all to remodel the plaque to healthy tissue to decrease the hourglassing.

To peter123 and others, the reason Xiaflex is said not to work with hourglass indentations is that those with hourglass indentations were specifically excluded from the clinical trial for FDA approval, as were those with calcified plaque, and 1-2 other reasons. So, bottom line is we have no idea if Xiaflex works with hourglass indentations. I had hourglass indentation and sever hinging that makes sex impossible. I don't have ED but the base of my penis has no blood when the rest of it is hard and it just flops back and forth, and I risk worse injury using it. After 3 series of Xiaflex, each much worse complication than the last, the hourglass is much smaller, the hinging is less, and I can finally have sex if I hold the base of my penis for a while to make sure it doesn't just bend at the remaining hinge. So...i think the jury is still out on whether Xiaflex works for hour glassing indentation. Seems to be working for me, but after 6 injections i am better but still a world away from where I was last year...

So an update from my post above 2 months ago in September. After 3 series of Xiaflex injections, I decided to wait on a 4th series of injections due to the increasing complications with each series of injections (bruising was worse, pain lingering longer each time, but no where near the hematoma problems some have had). After two months the plaque seems the same size, but the indentations continue to get smaller and I finally have enough rigidity at the base of the penis to have sex, albeit pretty carefully. So, perhaps this might have healed on its own, but I suspect it is the Xiaflex, perhaps in combination with 25 mg Cialis and Co-Q10 and vitamin E. I have not used traction or Vacuum, and only have done a little manual "modeling" as my urologist didn't think a few minutes of modeling each day would help. I did get myself hard each day for at least 30 minutes (when my penis was no longer hurting from Xiaflex injection). I am fortunate to have a wife who enjoyed helping with that part of my "self-therapy" :). So, update is Xiaflex seems to be working and I will wait longer to see if I get the 4th series or not.
