Title: can someone please help understand what this means
Post by: ocding on June 30, 2015, 09:57:54 PM
Hello guys, I finally seen Dr Levine earlier this morning and had a penile Doppler ultrasound. I'm happy to say that I do not have Peyronies disease. However I'm confused about what my Doppler results are saying. I tried remembering as many questions as I could but he seems to be a VERY be busy Dr and moved fast. so I'm still kind of left in the dark about a few things, and it was self pay Because I dont have insurance. I was injected with 0.15 mcg trimix and did not get a full hard on, only a semi. He even left the room and told me to start using my hand and imagination and that he would be back in a few minutes. I still could only get some what hard with stimulation only.

This is what it/he ended up saying,
Procedure:
penile duplex ultrasound was performed for erectile dysfunction. 0.15 mcg trimix was injected. measured deep cavernous arterial diameters. left pre-injection 0.9 mm, right pre-injection 0.9 mm, left post-injection 0.9 mm, and write post-injection 1.1 mm
Post stimulation response showed left peak systolic velocity 8.8 cm / sec, right peak systolic velocity 19.0 cm / sec, left end diastolic velocity 1.7 cm / sec, right end diastolic velocity 4.8 cm / sec, left resistive index 0.81 and right resistive index 0.75.
Procedure response pulsatility 2-3 but and. Erectile response maximum rigidity 7, phenyl and 600. curve at max extension ventral 20- he states when fully erect no ventral curve degrees. Findings include no calcification, no corporal fibrosis, no narrowing / indentation, no hinge effect. Compared to home erection today's erection is worse.
Discussion / summary:
" I believe this man has venous leak assoc with excessive adrenergic tone. There were no structural abnormalities. I rec nitely cialis 5mg with booster prn and referred him to Dr Hartlage here for sex therapy now or after one to two months of cialis as pharmacologic rehab. He was reassured that he does not have Peyronies Disease."

Can someone please help me with what venous leak assoc with excessive adrenergic tone means? Does that mean a venous leak due to a psychological problem or a physical one?

The good news is that you don't have Peyronies :)
Regarding venous leak, I don't think it can be psychological.
In my opinion, his proposal to take daily 5 mg Cialis for pharmacological rehab is because he believes it may help.
I was trying the Cialis for two months and see the results.

it may refer to constriction of muscle tissue surrounding the urethra in response to excess adrenaline...not certain, and not clearly documented where I can find it.  relax...

Ocding;

Read this, I think it explains most of why you were prescribed Cialis.

Thanks for the link man, even though I have a hard time understanding most of those big words. I just dont know if this is something I'm going to have for life and am always going to need pills or if taking the pills just for short term fixes this. :-[ this feels really depressing.

Also James, you say you took cialis for 2 months and seen the results, what do you mean exactly? Do you mean after 2 months on cialis your erection strength came back without having to take cialis? I'm only 28 and dont want to have to take these pills for the rest of my life just to have sex or get an erection. I understand with you it's different because peyronies.

But is there anyone else who has this type of excessive adrenergic tone venous leak? Can cialis used as rehab eventually bring your erections back to normal without cialis or ed drugs? Can l-arginine and ved help or will ved make it worse? I basically dont know what to do here and what to think.

First regarding what to do, in my opinion just follow Dr. Levine instructions. He is considered one of the best Peyronies specialist.
I am taking 4 to 5 mg Cialis every day to promote blood flow and to try to have some night/morning erections, maybe for life, but I am 67+. I need Viagra 50 or 100 mg for intercourse.
To your question:
QuoteCan cialis used as rehab eventually bring your erections back to normal without cialis or ed drugs?
The answer is yes. This is the reason Dr. Levine decided to prescribe you the daily 5 mg Cialis.

Those values are so low in terms of normal flow velocity... :| I am also curious about the cialis for 2 months, but do it. I will also try cialis in the upcoming months. Still have to find a nice uro though.

I dont understand, what do you mean they are so low? This is so confusing. I'm guessing if the excessive adrenergic tone goes away then so does the leak?
