I take it while fasting too. In the beginning I would not recommend this seeing as your stomach and intestines will get very upset. After a while you can try it out and see how your body responds.

Depends largely on age, with increasing age gives more gastrointestinal side effects.

I started taking only 2 pills with meals (breakfast and lunch).
It took me some weeks until I was able to take the 3rd pill (the dinner one) without issues. In my case not because of stomach issues but like anxiety issues or difficulty to sleep.

Nowadays I can take on empty stomach with no issues. Sometimes I do intermittent fasting and take the night one without dinner and have no issues.

By the way, I've been taking pentox for 1 year. Anybody knows if there is a maximum period of time straight one can take pentox?

There no maximum period (as suggested by my Urologist) but some doctors I've seen have stated that - if it doesn't give much benefit after the first year - it most likely wont do much. Others suggest it can stop progression of the disease and recommend staying on it.
