I'm really sorry if this sounds like a dumb question, but having worn my PMP now for two weeks with roughly 3-4 hour sessions, I barely notice I'm wearing it. Obviously when I sit down or pick something up from the floor I can feel something But that's due to the rods and not the tension being applied. I'm conscious that I haven't got enough tension and I'm wasting my time so far with traction. I would guess that I'm at roughly 600g.

When I take it off I'm definitely much fuller so it must be doing something but should there be a constant feeling of the penis being stretched whilst wearing the PMP?

You should feel a tug while wearing it and not pain. If you feel that you are seeing improvement just keep doing what you are doing and eventually you will know if you need to increase the tension a bit more. My Dr's instruction on the PMP is to increase the tension by .5 cm every 3 weeks. I notice that when I increase the tension too much that it sets me back. Gentle tension, but I know some people would say increase it.

After 3 weeks of use you should be aiming for a tension of  600g or greater therefore you should be showing between 2 and 3 bars on the scale.

i am just around 500 g and i feel quite a lot of tension when the rods are extended to the max. i think its quite normal for you to feel comfortable after wearing it for so long and you can check how tense your penis actually is by feeling it at max tensions.

be careful not to overdo the tensions, as lower tension over a long period of time is more beneficial than higher tension and the risk of hurting yourself and having to take a break!

Also if you use gentle heat while in traction by either a heated rice sock or a heating pad that is set on low with some sort of towel protector helps with loosening things up.

600 gr are enough. You don't have to increase the tension after reaching 600 gr and feeling fine with it.
3 - 4 hours sessions are very long. I prefer 2 - 3 hours, because the breaks are as important as the wearing. The penis gets used to the device, but that doesn't mean to increase tension, because you add pieces to the rod automatically to hold the same tension over months.

If I'm honest, I am confused how to adjust the tension. I get it is to do with the screws, but should the screws be nearer the base for a higher tension and higher up to decrease the tension? Everyone talks about the bars showing but I always see 5 bars? Currently I'm somewhere in the middle to play it safe!

And does everyone wear the rod system with the penis facing upwards, ie in the direction of their tummy button? I have a dorsal curve so wondering if it might be beneficial to wear it downwards although I suspect that may both be uncomfortable and awkward.

I wear it both upwards and downward, downwards irritates my scrotum after like 30 minutes so i have to switch, i do it because being upwards for too long irritates the base of my penis too, though much less,
Being on high tension causes my penis to be numb pretty quick
Thing is that if i measure the tension when holding the device straight it shows a tension that changes when i put it upwards/downwards so im not sure which to measure it by.

So basically, higher tension can be counterproductive for me and can cause pain(where the scar is) as the penis goes numb

i work from home on a computer a lot so pull my pants down and lay it perpendicular to my stomach on my pants. looks funny but quite effective as it keeps it off my scrotum.

and sometimes i lift it up to give it a little change.

yeah i noticed that the bars change as well depending on how you are holding it!

If you are seeing 5 bars then you should increase the tension somewhat until you get a good tug - between 400 to 600 on the device. Look at the attached photo. I also wear the device upwards and then sometimes downward. If you have it on correctly and no numbness or pain you can wear it as long as
you can. I have personally worn it straight for about 6 hours with a small 10 minute break in between and then resumed with another 5 hours. I did not have any distortions or blisters etc... - but everyone is different.

Never been sure what is wearing it correctly as there are so many variables with the sluice/diapraghm/rings and never been sure if the glans should show through the sluice becsuse mine do sometimes( i wear the white/transparent one)
But definitely i know im doing something wrong and discomfort leads to worsening for me so i take it off frequently
Do you have numbness or pain if u wear it correctly by your definition if you wear it on high tension?

I also use the clear/white sluice and I can see if any portion of my glans is pulling out and I can feel it. So this is an indication that I don't have it in correctly. So take it off and put it on again. I have done this about 4 or 5 times once until it felt and looked right. As far as tension goes if I put a high degree of torque on it - or if I am sitting in my lazyboy I will feel a very very strong pulling sensation like right down through my perineum, but without actual pain. I must also say to everyone that I have had 2 double hernia operations on my right and left side. I am completely fiberglass steel belted in my groin area. Things down there are already pretty tense and tight. So I don't know if this hinders me or helps me?
