Hello -

First off, thank you all for the information in this forum. It is so helpful.

This is my first time ever posting. I've had Peyronie's-like symptoms for the past two years, which I believe were either caused by 1) post-hernia repair nerve damage affecting erection quality which led to trauma 2) pelvic floor dysfunction also leading to poor erections/trauma 3) damage from a cock ring or 4) genetics due to grandfather having duputyren's

Anyway, my case is a little different as I have little to no curve, but have had substantial narrowing and hourglassing over the past two years. No plaques have been identified through ultrasounds (flaccid and erect), but I have had 2 urologists identify a hardness that could be a plaque in physical examinations. Other urologists (I think I've seen six woof) have said everything is fine.

I have fair erection quality but not what it once was. And have probably lost an inch in length and a 1-2 inch in girth.

My concern is that I have tried low dose cialis, very conservative VED use and even a heating pad to attempt to help sretch the plaque and regain girth and length. However almost everything I've done has led to increased pain and what I perceive as worse off narrowing. My thought may be the pelvic floor issues prevent the tissue from expanding due to lack of blood flow, and then an "artificial erection" puts too much pressure on everything.

Could I just have a penis that is extremely sensitive to trauma? All of this has caused severe anxiety and as a gay man, the need for a firm erection is even more important with penetrative anal sex.

So I guess my ultimate question is - what are the best treatments for narrowing/hourglassing and pain with no significant curve?

I've always had some pain which could be identified as from the pelvic floor, but I've had two 9 month or so more significant pain eras and all the narrowing makes me think it can't just be from the pelvic floor.

Any advice is greatly appreciated. Thanks for your time.

Welcome to the site miafranc and so many questions.  You've found a supportive and informative place to ask questions!

Miafranc, many on this forum and peyronies specialist seem comfortable starting right away with:  VED, esp for hourglassing; traction for shortening; heat (hothands is convenient) for softening the plaque; along w cialis, pentox and antioxidants also for softening the plaque.

Somanyquestions, finding the right MD seems crucial so it seems you're on the right track!
PxD 2 yrs 9/16.  Failed all treatment. 9/11/18: excision, grafting & implant Dr Karpman MtnView Ca, AMS CX 18cm + 3-1cm RTEs.
Pump failed.  2/11/20 Dr Karpman installed Titan 22cm +1cm RTE.

I guess this is my concern with what you've mentioned - it's a paragraph I posted originally (although I know it was a super long post)

I would look into drugs like pentox and acetyl l-carnitine. I know pentox is supposed to help with the pain.
Also, measure the width of your penis (circumference) so you can see if any changes are taking place.

When I take a 20mg cialis it provides an erection that seems to irritate everything. My answer, yes I think good erections could aggravate your peyronie's. Not much you can do because strong erections are what you need. VED is what people here will recommend for narrowing.

I spend a lot of time researching penile implants because it fixes everything and men have a lot of satisfaction with them when implanted by a world class surgeon.
