Topical Sildenafil for ED

Was looking at a study that is in phase 2 for sildenafil in cream form . The company says they want to make the product over the counter which could really help those who struggle to get insurance coverage for the pill form (like me). Additionally, there have been reports of fewer side effects from the cream which would also be positive. What do you guys think? Would this work for peyronies induced ed or do you think there would be problems with absorption due to the scar tissue?

I would certainly give it a try.  During the study the question of absorption should have been addressed, but maybe not for Peyronies.  Do you have more information on the study?

I don't know all the ingredients of this, but as Jimbruski alludes to, penetrating the dermis to reach the problem has long been the problem with topical treatments for Peyronies.

It seems to me that the drug would work no different than the pill form. If it can be absorbed into the blood stream, it's the same. The drug does not need to actually touch or hit the scarred areas to "work." However, I would love to see a study on it and see if they found higher concentrations in the penis, which I imagine they would. The other question is, does it even matter where it's concentrated or not. I suppose it might, in which case one could potentially achieve lower doses of the drug vs pill form and spare themselves from potential side effects. Interesting.

Ya, the clinical trials weren't focused on peyronies so hard to know.

There is still the problem of absorption. Without knowing the complete contents it's difficult to judge.

The other thing I would point out is, that after my surgery with Franklin Kuehhas, he prescribed me low dose Cialis - tadalafil. I mentioned using Sildenafil instead as the patent had expired and legally available generics were far cheaper. His response was no. The reason:  Sildenafil only stays in the system for a relatively limited period. His point was that Sildenafil is fine for sex, but for the purposes of maintaining increased blood flow over a far longer period, tadalafil is what's needed.

When the patent expires on Cialis, which I believe is this year, a similar cream containing tadalafil may become available.

I take sildenafil and here's what I can share:

Duration
- The label says: Lasts up to four hours.

- Experience: True. I find that I usually have to have intercourse within two hours in order for the maximum effect to be accessible to me personally. I'm looking for enough blood to flow to create sufficient hardness and for sufficient duration. Sildenafil does that.

- How it works: This drug is from the vasodilator class. It relaxes smooth muscle cells within vessel walls, in particular large veins, large arteries, and smaller arterioles throughout the body -- including the penis. The drug was originally developed to treat high pressure in the lungs. I suspect the discovery went something like, "Hey doc, look, I can get erections, again!" Wait. What? Boom, money maker.

- Opinion: I like that it does the job and is out of my system sooner than later. I take 40 mcg, and this is just enough. All ED drugs run the risk of creating an erection that lasts over four hours -- which if that happens you need to go to the emergency room so they can shove a needle in there for various strategies, but the outcome will be deflating the erection. When you're 20, you think that going all night is the bomb. Fact is, keeping it up that long, well, you might as well throw a bomb into the cavernosum. Sustained erections of four or more hours are bad for penile health -- per international medical knowledge. And, once you've found the PDS Forum you KNOW that penile health is worth gold. And, the last thing I want on this planet is for somebody to be shoving a needle you know where. Speaking of gold, the cost of ED drugs is outrageous. But, my membership at Costco allows me to obtain 90 sildenfil tabs, 20 mcg, for only $85 after insurance. Since I take two tabs, that means I can have sex for $1.89 per lovemaking session. If someone ever told me that one day I would be paying for sex I would have thought I had the last laugh.

- Funny story: I originally saw my general practitioner for a sildenafil prescription. He gave me a sample and said to call the office back if I liked it. When I called back, I spoke with the nurse and she said if I helped her write the prescription she'd get it right to the doctor for approval:

Me:  "Ok, then, how's about '40 mcg, as needed.'"
Nurse: "Yeah, but what if you want to have sex a couple of days in a row?"
Me: "As needed covers it."
Nurse: "Yeah, but what if you wanted it every day?"
Me: I couldn't see the difference, so I said, "Perfect."

Later that day, when I got to the pharmacy pickup window for my consult, I watched the pharmacy assistant grab the bottle, give it a glance while she was putting it in the bag, then she quickly pulled that bottle right back out and read the words, "Take daily. For sex." in big bold letters. No, those are not words you see everyday. Simplicity at its finest.
