Jan 12 Plication - here are my results so far
« on: January 23, 2015, 12:05:14 PM »

Hi there. I am a 36 year old man who has had a downward curvature my entire adult life. I suspect it was not caused by damage as I don't recall any trauma in my youth that would have caused this later. This curvature was present drom puberty onward. Erections were not painful, but did create difficulty in having intercourse. I did not have my curvature measured, but would estimate it at somewhere around 50+ degrees. My wife and I found ways to make things work, but obviously there was discomfort on both sides and "optimal results" were not always attained.

My surgery took place on Jan 12. It was day surgery that took place first thing in the afternoon. Full degloving and sutures in place to straighten stuff out. As I was being rolled out and into the recovery room my surgeon came by and said everything went well and things look good. I was home and resting by about 8:00 or so that eve.

The next two days consisted of laying on the couch watching TV, playing video games, and just generally doing nothing. I was taking the prescribed antibiotics (5 day course) and T3s for pain. I left the dressing on for two days (as instructed). So far so good at this point.

Day 3 I removed dressing and had my first post-op shower. Still lots of bruising everywhere - no surprise. No problem. Stitches look fine, little sign of significant healing at this point. Head of penis is still completely numb. This doesn't concern me as I've read from others here that this can persist for quite some time. I was cicumcised prior, so no issues there with that.

I believe it was the night of day 3 where I had my first (and second) nocturnal erection. Wow pain. I must say, this is among the most painful things I've experienced. Any movement magnified the pain, so the goal was simply to manipulate the blankets and my sweatpants so they were not putting any stress/pressure on my erection. I was unable to move around enough to pull blankets back, turn on a light, etc and take a peek at what was going on. Erection dissipated on its own within probably 5 minutes or so. I was well and fully awake though. Took significant time before I could fall back asleep.

The next few nights basically where the same. An erection or two overnight causing pretty severe pain - enough that I was unable to check and see how things looked. At this point I have had at least one erection every night since the initial. I'm encouraged that my little buddy is eager and wants to get back in action, I'm not completely happy about being woken every night excruciating pain. I would rather this than zero erections and full nights of sleep.

I would notice that in the morning once I would get up (get out of bed, not get up as in an erection) and take a look good that I would have fresh blood/scabbing around the incision. I guess during my erections the incission was being stressed and essentially being pulled on. Makes sense. No surprise there. All my stitches were holding, and the incision was looking better day by day so I let it ride.

Bruising dissipated probably somewhere around day 5. It was probably around day 5 or 6 that I was able to sneak my first peek during my nightly wake up. I kept my phone within reach one night, and when I woke I grabbed it, put on the flashlight app, and flipped back my blankets and lifted my waistband on my sweatpants. Smiling back at me was an almost completely straight (but still very very painful) erection! Exciting!

Fast forward a few days more, incision is healing very nicely at most spots, there's one section that is healing slower and is prone to more irritaion from rubbing on underwear, but its coming along. It is not infected. The stitches have not dissolved yet, and its approaching the point where they are more irritating than the discomfort of the incision itself. That's how it feels anyways.

Still getting erections nightly (often more than once). They still hurt very much - though I think maybe a little bit less compared to last week. Its difficult to gauge for sure. I called my wife in to peek at one the other day - she seemed pretty excited! I think it was straighter than she expected. We've been sleeping in two beds during the early part my recovery, plus she had a bad cold I wasn't interested in picking up. She commented that things didn't look much shorter to her at all - there's still way enough to work with.

If I measured along the top of my penis (my long side - of course a guy is gonna measure the long side) I was over 7 inches. I haven't measured post-op, but i'd be surprised if I lost more than an inch. Can't complain about that! I've also only peeked at erections that aren't fully full. There's been no hands on or anything along those lines to fully stiffen and inflate things as is usually done prior to use.

As is the case with everyone, I am excited to give things a test drive. I often find my thoughts wandering to sex and related activities, this causes a partial erection (if I can redirect my thoughts quick enough, or a fuller erection if I can't). No signs of any damage that will lead to Erectile Dysfunction (at least not yet).

At this point everything is great and moving along more or less how I would have expected.

I do have a few Qs for others who've been through this.

1) my head is still numb, I am getting a small amount of sensation along the ridge on one side. Too much sensation actually. Its very sensitive, not necessarily pain, just ultra sensitive in a small area. I'm taking this as a positive. I've seen some people say it takes many weeks/months for proper sensation to return. Curious what others can offer from their results.

2) I can feel the internal stitches when I handle and inspect myself. It seems like there is one set of stitches along the top that is centered, and then a few sections kind of along the sides, not quite fully on the side, but partly (if that makes sense). The set that is on the top has a bit of a bulge, it feels like the knot of whatever exactly they did there. The side sections don't have this. I didn't have any left/right curvature before, so likely the stitching on the sides was minimal in comparison - it almost feels like it was put in as some type of support stitching just to reinforce. I'm curious if this bulgle/knot/whatever is something others have encountered, and did it go away in time? I'm not worried about this procedure being noticeable later down the road. My wife knows what has happened, and really that's the only person who is gonna get any hands on time with my gear. I know some guys (understandibly) are concerned with a surgery that is undetectable later - this is not my concern here.

3) What time frames have others had to deal with as far as pain during erections? I have not been cleared for intercourse yet (obviously). My follow up appointment isn't until March. I have been told no intercourse before my post-op appointment. Obviously everyone here has varying degrees of correction done, varied rates of healing, and varied levels of pain tolerance. When did erections cease to be painful for anyone else?

4) Sex is off the list until mid march, but I'm pretty sure I'll be ready to have a little bit of some "me time" before that where I can give things a test and see if everything still fires. Depending how/when that goes maybe I'd follow up with my wife and look at some gentle handjobs or blowjobs. Have others found that they were able to take part in manual or oral stimulation significantly earlier than full intercourse?

Looking forward to hearing from those who are ahead of me (or done) in their recovery.

I'm optimistic for what is coming next!

MisterBee

I can't answer to your questions as I had no surgery, but I would like to thank you for the post.
Could you give us the surgeon information? We don't have too many reports from Canada.
Wish you continue the successful recovery. :)
Please continue to update us.

Nocturnal erections continue - 3 last night (first time having 3). Each resulted in me being awake for approx 1 hour between the erection and falling back asleep. It leads to not getting a good nights rest, but I guess that's how it goes. The pain is certainly less than it was, though it is still very very significant.

Everything seems to be healing good, stitches are still very irritating and kinda prickly. That's a short term thing that is easy to tolerate/ignore.

MisterBee

Your nocturnal erections is a sign that your surgeon made a good job and didn't harm the nerves.
I f you don't want to post the doctor name, send me a PM.

I would like to touch base with my surgeon and see if she is comfortable with me sharing her name here before I do so. I'm fine with doing so and will shoot an email over to her office on Monday and see what he feelings are on that.

I'm for sure taking all these erections as a positive sign - as painful as they are.

You didn't say what procedure you had, so ours might be different. I had pain for 3-4 weeks post op. No intercourse until 6 weeks post op. Also had the very (overly) sensitive ridge. Over time this returned to normal as well. I've always said healing is a long slow process and not to rush it. After 6+ months for me I would say things were back to normal.

Figured I would give another update as another handful of days have passed. Stitches along incision healing nicely. Stitches are for sure providing more irritation than the actual incision at this point. This is much as someone might expect 2+ weeks after surgery.

I have been woken by (significantly less) painful nocturnal erections maybe only 50% of the time over the past 4 or 5 nights. I suspect I am still having them pretty much every night, but am managing to sleep through them, or at least only wake up lightly enough that I end up going back to sleep more or less immediately afterwards and managing to stay groggy enough to not remember them in the morning.

Erections that I am getting are still looking good, looking straight. For sure less painful than in the first couple weeks. Very much still uncomfortable - I wouldn't even entertain the idea of using an erection at this point. It's nice to see one, but certainly when they arrive I want them to leave as quickly as possible.

Don't expect much in the way of significant changes or updates - it may be some weeks before post more updates. If something "bad" happens I'll keep people in the loop so they can either compare to their own results, offer me advice/suggestions, or just use the info as they make (difficult) choices for their situation moving forward.

My post-op meeting with my Dr. isn't until mid March, I may not post again at all between now and then, depending on how things go. I will be looking to get more detail about what exactly was done during my procedure. I don't at this point know exactly what type of surgery I had. I know it was a Nesbit - but I'm not clear if that is a category of surgeries, or a specific tecchnique. For example, do the 16 or 24 dot methods classify as nesbit, or are they their own thing? I guess if it's all terminology then a Dr. could really be doing one thing, but referring to it by another name.

Glad to hear the nocturnal erections are becoming less painful. It's quite a relief when that subsides!

About the question at the end of your post: if I'm not mistaken, the Nesbit procedure refers to excising portions of the tunica and stitching the edges together. This was the 'original' technique, but gave too many unsatisfactory outcomes, so surgeons starting modifying Nesbit's technique just to rely on sutures. These version involve just shaving/scoring the tunica (Kuehhas, Levine?) or not cutting it whatsoever (Lue, Morey, etc).

The 16/24-dot version you mentioned is Dr. Lue's technique for arranging the sutures to minimize tension. (16 and 24 dots are precisely the same technique; 16 dots is the norm, but 24 are used for exceptionally long penises and/or severe curves.)

All the above are 'plication' techniques, which shorten the long side, as opposed to grafting techniques, which lengthen the short side.

Anyway, please let us know if the surgeon clarifies which technique she used. Best wishes as you recover, and keep us updated!
