Don't know how to proceed with Vacuum extenders

Hi, I have plaque near the top below the head; both on the right and on the left side, as well as plaque near the base sending me in a 40 degree angle left during an erection.

I'm looking at the Vac3 or Vac4 extenders, but I'm a bit confused as to what size and what mm length to get.  

I have the Peyronie's Device for traction right now, and the noose and strap are constricting my penis so I'll need to look into what vacuum extensions are needed

The mm question for the Vac3 has to do with the width of the extension rods, not the length. I believe they offer 9 and 10 mm rods. This is important if you are using it to adapt another extension system to their system. My Andropenis used 9 mm rods but just to be sure, I ordered the 10 mm system so that the Andropenis rods would be sure to fit into the Vac3 extension piece (where you clip the vacuum to.
