Hello,

Today I stumbled upon this Forum and think that it's incredibly great how everyone helps each other.
I've already read a few topics  but i am still not sure about my condition.

About a year ago i noticed a light indentation at the right side of my penis when semi-erect. I didn't think much about it, but about 4 months ago (after my second circumcision) i noticed that the indentation was also below, and on the left of the penis, forming the "hourglass"-shape. At that time i also stopped having night-erections and think that my normal erections aren't as "hard" as they used to be. As i said, the hourglass-shape appears only in semi-erect state.
Can someone tell me if that is Peyronies, or if it can develop further? And if the absence of night-erections can worsen it?
I already visited a urologist and he couldn't find any plaque.

Hi Mark,

Nighttime erections are important for penile health. Have you been checked for other health problems that might cause loss of erections? If not, I would get a general check up on your overall health.

As for Peyronie's, a definitive diagnosis might need to be done with a specially-trained urologist. Some plaques are too small to feel during examination and may require an ultrasound to find the plaque. I had a flaccid ultra sound done a few years back and it is a painless and quick procedure. If they do diagnose Peyronie's disease, I suggest getting on pentox and possible daily cialis. Both have been shown in studies to reduce inflammation and prevent the build up of scar tissue. Perhaps one of our members from your region can recommend a good specialist urologist near you. I am in the United States so I am only familiar with the big names over here.

Thank a lot for the fast reply!
I will try to do a general check up as soon as possible. I talked with my urologist about the nighttime erections, but he didn't think that it matered. I just think that it's weird that i stopped having them right after the circumcision. Before that i had them every night...

There are all sorts of penis issues that are NOT Peyronies Disease. And there are multitude various manifestations for Peyronies Disease. We cannot tell you from this forum if you have it. A professional needs to do that. However, based upon what you have said, get thee to a professional!!  It sure sounds suspiciously like it. Could it just be trauma from the circumcision? Sure. Could it be that the trauma of the circumcision triggered Peyronies Disease? If your conditions were right, absolutely! The absence of plaque that is not felt does not clear you. Can't feel mine either. Please get a professional opinion. Then get busy doing something about it. This forum is your best resource and your best friend. Good luck.

I will try to find a specialist as soon as possible. My only fear is that my condition will worsen in the meantime, because of the absence of the nighttime erections.

I found the resources this site offers unrivaled in dealing with Peyronie's. We're all brothers in this F^@$!ng war against Peyronie's.

I put a great deal of emphasis on the idea that testosterone plays a significant role in Peyronie's. From what I've gathered regarding my own personal Peyronie's nightmare, the drop in my testosterone levels precipitated the Peyronie's and ED.

My recommendation would be to get a full blood work done especially identifying testosterone levels. If the results are that your testosterone level is below what is considered the 95% percentile, you should consider begging the doctor to put you on testosterone replacement and possibly Clomid and Anastrozole to bring down estrogen levels.

In the meantime, it can't hurt to add things like the following to help raise testosterone and nitric oxide levels (improve blood flow):

Hi Matt,

I will certainly do a blood check after the holidays. I hope it's the testosterone, because now i'm worrying about something much worse: the absence of the nighttime erection started just after the circumcision, and it could be that, during the procedure, the perineal nerve was damaged. I read about it, and it basically says that, if damaged, you can't get nighttime erections, but can still get psychologically-induced erections (just what i have)....

Matt, i didn't quite understand this. Do you mean that your libido is now worse because of the supplements? Sorry that I'm asking.

For the nocturnal erections, I suggest you get Cialis Daily as soon as you can. I had no night erections at all. I started Cialis 5mg and I wake up with wood every morning now. Some guys require more than 5mg, but you can increase until you find your balance. Any doc can give you Cialis.
Btw, I may be missing it, but what is your age? That may affect the approach you take to all this.

Also go to the Erection Dysfunction board and read about Trazodone.  I am willing to bet a miniscule dose of 25 mg (1/2 or the smallest tablet) will give you NTE's and even though it is prescription it costs about 2 cents a dose.

Hi,

My age is 20.
I will try to get one of these (Cialis, etc), but i suppose i will have to wait until next week or so to get an apointment at a normal urologist.
I have a question: will the nighttime erections eventually return when i stop taking the meds?

There is no way we can know if NTE's will return if you stop the meds.  We don't even know why they stopped.  Maybe they will return if you do nothing.  Maybe they will not return even with the meds.  I am sure you can see how impossible that is for us to answer.

Sorry I wasn't clear.

Since I started taking the supplements in the list I posted, my libido went nuts. I'm hornier now than any time in my life and I'm 49. If it weren't for the venous leakage, this would be a magical time in my life.

When I added the testosterone, it further increased my libido.

I agree that adding 5 mg. Cialis is a good thing to do. You can get a 30 day free trial at the Cialis website and then use a discount card to keep the monthly cost to about $120/mo., assuming you have no insurance to cover it.

Lastly, I agree with Hawk that Trazodone is worth trying and because of the very low dose and not using it every night, you shouldn't have any habit forming issues. However, it is an anti-depressant of sorts and should be evaluated as an option by you and your doctor. I think all insurance companies cover it and it is dirt cheap.

Hello, merry christmas and thanks for the replies!

I think this is possibly the worse time to notice this disease... Because of the holidays i can't really get to a doctor or to a farmacy.
I am now thinking about what medications i will use. First i will try to get pentox and cialis from my urologist. But i am not sure about the other: COQ10, l-arginine,l-carnitine, etc.

What do you think is the best combination?

My advice is:
Pentox, Cialis, CoQ10, Acetyl L-Carnitine, L-Arginine.
If you can, add the VED

Instead of Arginine, may I suggest you check into Citrulline Malate?

Citrulline will break into your system more effectively than Arginine. Also, Citrulline won't make you sleepy like Arginine will.

I don't mean to sound obnoxious about this but for those of you have have exhausted your Arginine supply and are not seriously considering Citrulline then you are short changing yourself.

Hi guys.

So, today i got all the things i need (except the VED). I told my urologist about the benefits of Pentox and Cialis and he prescribed them right away. So I have the Pentox, Cialis (5mg) which was incredibly expensive, COQ10, l-arginine and l-carnetine.
What daily dosage would you recommend for me? Is 2x 600mg Pentox enough?

The standard dose for Pentox is one 400 mg tablet 3X daily, WITH FOOD.

As per Dr. Lue's recommendation, I take 400mg. 6 times a day with food. But only do that if you have been prescribed that much.

I think your doctor might have prescribed the Extended Release version of the Pentox but I'm not sure so 2 times a day I believe is appropriate for the ER med. You need to double-check with your doctor and/or pharmacy on that.

Also, I told you before, go to the Cialis website, you can download a PDF for a free month of 5 mg. Cialis. Then, you can get a free prescription card at http://freedrugcard.us/. Use this card to get a discount on any future Cialis prescriptions. In my area, I can buy 5 mg. Cialis for about $120/month using that discount card. You can also use that website to look up the pricing for any medication that you have to buy outside of your insurance coverage.

Also, did you get your testosterone levels checked? If yes, what was the result? If no, what the hell is wrong with you? :)

Lastly, I told you to look into the Citrulline Malate instead of the Arginine, did you? If not, what the hell is wrong with you? :)

My understanding is that all Pentoxifylline is of the extended release formulation.

There is a twice a day formula. It's a more expensive. It's 600 mg. and it's called "Controlled Release".

What does extended release mean? Is is it worse than the 3x400?

Mark4, please don't quote me but it should work exactly like 400 mg. 3 times a day.

Also, your testosterone might be fine but it's worth checking. My philosophy on testosterone levels for Peyronie's patients is that it should be at the 95% percentile level because of our condition. Which means, it needs to be high. I'm basing my opinion on the study that was done regarding Peyronie's and low testosterone. What I'm stating is not conclusive but as long as it does no harm, I'm amping my testosterone as high as I can get it. And, I'm going to add HGH to my program as well.

I've been on testosterone therapy for 2 weeks and I'm loving every moment of it. :)

Okay, I think i will begin with the following daily dosage:

2x 600mg Pentox
2x l-arginine
2x l-carnitine
2-3x COQ10 (I'm not sure yet)
5mg Cialis (maybe less, depending on what happens)

If someone thinks that i should change something feel free to say it :)

One little thing: I asked my urologist whether the circumcision could be the cause of the lack of nighttime-erections, and he said that that couldn't be, because the nerve would still function above the cut.

I have used two types sustained release (ie regular pentox) and extended release (purchased from river).

The extended release are much thicker in size and can be taken with no food, no problem.  The regular/sustained release seem to have a much shorter half life and have more side effects.  I would recommend the extended release when you have the option.

Hi guys, just wantedd to do a quick update on my condition.

I had an appointment at my urologist to make an ultrasound.
Well, i was there thinking the worst, but as it turns out, he didn't find anything.
He looked for scar tissue, etc but couldn't see anything. I asked him about it and he said that if there was anything he would see it immediately, he even looked at the underside and directly above the shaft to see if it was in the septum, but it wasn't.
He also said that my blood flow is excellent.
I got my blood checked (will have the results in a week) and he made me an appointment for an andrological consultation, where a neurologist and other doctors can look at it.
He told me to keep on taking the Pentox and Cialis.
I can't say that I'm relieved, but at least he ruled one thing out.

Hi,

Thanks for the kind words.
I also think that it's good news, but I'm still pretty nervous about my condition. I constantly ask myself if the doc could have missed it on the ultrasound, or if it's something with the nerves,... One thing i know for sure is that in the last months, and especially the last week, the form of my penis changed, and it's weird because Peyronies would explain everything...
I think I will chat with my uro about it when i get the results of the test.
