As I have the financing for a semi-rigid implant surgery to be made by my local urologist, I would like to ask the opinion of the forum members regarding the timing.
I have made measurements using the VED because stretching the flaccid penis with my very hard plaque that runs from the bottom to the glans it is just impossible.
I made a drawing (attached) that shows the bends side view and top view.
The length measured in the VED is 130mm, but from side view the opposite side of the plaque is 142mm and from top view is 140mm.
My first question is:
If during the implant surgery the doctor will make incisions of the plaques to make the plaques sides flexible, the outcome will be longer implant in the range of 140mm or will remain 130mm or even shorter?
Second question:
In the last few months with the oral treatment and the VED the plaques feel somehow softer and my erections have improved also. Should I give more time to the oral treatment and VED, maybe will get better results from the implant length and girth wise?
I will appreciate any opinion, suggestion and remark.
I know the forum members are not surgeons, but I would like the forum opinion as I think is the only place I will get not biased answers.

I am not sure I exactly understand your plaque and why you cannot do a BPSFL but in general I would work on the meds and VED for at least a month beyond when I felt all improvement stopped.  You do not want to proceed if you are still making progress.  

Next, I will leave it to you about the type of implant but I would think I would only get a semi rigid implant if I thought I could never get a 3 piece implant AND I could not get an erection with the VED sufficient to have intercourse.

After using VED the plaques are more soft and then I can do BPSFL. The length come out more or less the same as in the VED.
If I try to make the BPSFL before the VED, it come out some 1/2" shorter because when I am stretching the penis it hurts.

My main point with erections is that is sufficient for penetration but lasting short time. I think I will continue with my actual treatment per your advice because have some improvement and I would like to have maximum length/girth before the implant.

Regarding the semi rigid implant, it is the only one that I can finance right now if I will make it here and I don't think in the near future (at least three years) I will have enough to make the three piece in the US. My doctor here has done some 60 semi rigid implants, is a simple operation and cost around $ 7,000. He have done also same number of incision with grafting for Peyronies.
The 3 piece will be $ 12,000 plus pre and post operation doctor expenses, traveling and stay in the US, it mean total of at least
$ 20,000.
Maybe in a few years time I will replace the semi rigid with the 3 piece if I will be able financially.

What you thing about the length in my drawing? The incision of the plaques will enable stretching of 1/2" more than the actual?
Or I am completely wrong in the way I am thinking.

That's a lot of money James.  Aren't there implant surgeons (3 piece) closer to you the U.S. ?  

Also, when you say erections don't last, is that also true when Ou take Viagra and l-arginine?

In my area Singapore is considered the best, Thailand the second. Have just two problems with both places (3 medical centers from each):
* The prices I get are $33,000 to $39,000 if no complications!!! (from US, $12,000 and $20,000)
* To my question how many surgeries the surgeon makes per year or the total number he have made I get no answers.
I will call this week AMS representative here in Indonesia, I get the phone number from Jack and ask them who they recommend an pay a visit to the recommended doctor.
An other problem that have here is that the appreciation they are giving for any treatment including surgery is X and the bill more or less 2X. It happened to me three times, heard the same from friends.

Regarding the erections, they don't last even with 100mg Viagra and 2000mg L-Arginine, but have some improvement in the last two months. Sometimes the erection is not full also, maybe 80%.

I tried again today to make the BPSFL.
It come out 125mm, the VED shows 130, sometime even 136.
The problem with the BPSFL is that the cord like plaque running from the base (inside my body) to inside the glans is so hard that can't really stretch my penis. :(

If the "cord" stretches tight to its limits and prevents you from further stretching your penis, then that is your BPSFL.  Those same fibers/tissue yould prevent stretching with an implant unless they are excised and replaced with a graft or dealt with in some other way.  If that tissue is excised and replaced then calculating the gain you might experience is more complex.

My theory is that if this "cord" will be gone, the penis will stretch much more. This "cord" is hard like a tendon or a bone. :(
Hope my theory is right. :(

I just saw this post james.  I am not sure what exactly what we are talking about here.  The "chord" is likely plaque.  If it was "gone" you would be normal IF by "gone" you mead returned to elastic normal tissue.  An implant however will not correct the plaque other than it might put some stretch or tension on it and somewhat correct the curve.  Even deflated an implant acts like 24 hour a day light traction because your penis can never retract (turtle effect).

What I am saying is that the plaque runs all along my penis from the base up to the glans and makes a banana shape because this side can't stretch with an erection, with VED or flaccid.
It is calcified and strong like a bone, lost all the elasticity and can't stretch it, doesn't mather how much I will pull.
So if it will be excised or incised with many small cuts, the BPSFL should show a much longer penis. This is my theory, hope it is correct and then I can get longer penis with an implant that the length now.
What is your opinion Hawk?
Some other forum members, have opinion on the subject?

James,  is the plaque really hard "like a bone" or was that an exaggeration ?  When you are flaccid does the other side contract and the side with the plaque stay longer because it cannot contract as much?

The plaque is more like a tendon. Can bend in flaccid to some degree (still very stiff), but can't stretch.
In flaccid, the bottom side shrinks, so the penis is strait more or less, both sides the same length.
In cold water the penis shrinks and the plaque side stay longer than the other side.
