I go to a leading Peyronies Urologist, and I currently have a moderate case of Peyronies that is causing hourglassing at mid shaft. Basically he advises against surgery for hourglassing (grafting) unless you cannot have sex at all. In his opinion, there is so much risk and possible adverse affects to grafting, that you only do it if you can't have sex at all in your current state. That way if it does not help, you did not lose anything. In his words, grafting causes scar tissue as well, so it can possibly not have any corrective effect, but can have downside effect.

Are you having any pain during erection or in flaccid state? and is your urine flow affected in any way? Other than hourglassing do you have any other deformities?

Some pain during erection. Not really much flaccid, or any other issues/deformities other than hourglassing.
