Yes, it's possible to achieve orgasm other ways. My urologist (Dr Lue) says that gentle masturbation and oral sex is acceptable. I would not risk oral sex personally, but based on that advice, I assume that it's not the internal process of orgasm that risks penile fracture, but the road to achieving orgasm, i.e. physical contact with the penis? My last injection was about 10 days ago, so it's been about two weeks total without relief. Is there any real risk of injury from climax if not stimulating the penis directly; perhaps my girlfriend gently rubbing just below it? (this is prob the longest I've abstained in decades and it probably wouldn't take much!)

Where did you hear that you could not have orgasm after Xiaflex injections?? The purpose of the injections is to get Xiaflex into the scarred area and hopefully it will soften the plaque and "release the curve". I've had injections by Dr. Levine and was only ever told to take it easy for a few days after each injection. You should be fine.

Don't get confused by posts regarding post-surgery. Post-surgery is a completely different scenario.

The makers of Xiaflex advise against any sexual activity for 4 weeks following the second injection of each cycle. My urologist also warned against it for risk of penile fracture, which is pretty serious. He gave me a printout with after care instructions that indicates gentle masturbation and oral sex are acceptable, while warning against anything else. In general, the consensus of opinion that I've culled from these forums (specifically this Xiaflex subforum) seems to suggest that any sexual activity is extremely risky until at least 3-4 weeks post injection. I've even read that sustained erections are not good for the health of your penis while it's in recovery due to thinning/softening of the tunica albuginea.

From Xiaflex website:

"Penile fracture (corporal rupture) or other serious injury to the penis. Receiving an injection of XIAFLEX may cause damage to the tubes in your penis called the corpora. After treatment with XIAFLEX, one of these tubes may break during an erection. This is called a corporal rupture or penile fracture. This could require surgery to fix the damaged area. Damage to your penis might not get better after a corporal rupture.
After treatment with XIAFLEX, blood vessels in your penis may also break, causing blood to collect under the skin (hematoma). This could require a procedure to drain the blood from under the skin. If a hematoma appears, skin and soft tissue necrosis (death of skin cells) may develop in that area, which could require surgery.
Symptoms of corporal rupture or other serious injury to your penis may include:

a popping sound or sensation in an erect penis
sudden loss of the ability to maintain an erection
pain in your penis
purple bruising and swelling of your penis
difficulty urinating or blood in the urine
Call your healthcare provider right away if you have any of the symptoms of corporal rupture or serious injury to the penis listed above.

I had a lot of misunderstanding on this topic early on. I recently returned from Dr Trost's office for my third Xiaflex treatment. You can see in my post history that I have experienced with erections and ejaculations during the recovery treatment and learned a lot through trial and error.

During my second visit, I briefly touched on the topic and was told that "erections are good" as a natural way to stretch/model. But I did not press the topic.

During my most recent visit, I explored the topic more deeply. When they talk about abstinence, they want to prevent against blunt force trauma, such as can happen if you inadvertently pull out and miss the re-entry during sex. There's no doubt the penis' structure is physically weakened for a few weeks. Causing a "fracture" (what a needlessly scary term by the medical community) on a non-wrapped erect penis will lead to uncontrolled swelling due to the high blood pressure inside the penis. It's not a pretty situation, as I have found first hand. And since it's unlikely anyone is having sex while wrapped, it's best to just forget about penetration.

That leaves other types of erections/orgasms. First of all, nobody can control what happens during the night, so erections are happening regardless. So long as you are properly wrapped, any further internal stretching or tears, which is exactly what happens at the clinic post injection with the modeling, will lead to controlled swelling.

I was told that manual or oral stimulation is fine as soon as I felt up for it. I'd highly recommend doing it wrapped the first time (manual, vibrator, mind power, whatever works) so you can assess how you feel.

Finally, most doctors only do a few Xiaflex injections so they have little experience and even fewer post treatment feedback. Dr Trost and his staff perform multiple daily. Dr Trost himself authors and publishes protocols for others to follow. I trust his advice.
