Plaque in Glans Penis (certainly not your average Peyronies)

M62:  I developed a mass in my glans penis 2 years ago.  This mass was 3~4mm in width and 5 mm in longitudinal direction.  No trauma or precipitating factors.  No deformity or pain in the penis.  Urethral meatus and urethra normal, shaft of penis normal, testes normal.

8 mos ago, that mass became inflamed and painful to the touch.  In the flaccid state, there is a slight deformity of the dorsum of penis just proximal to Coronal Sulcus.  Palpable firmness over 2 cm of the dorsal vein.  In the erect state, the dorsal vein is acting as a tether while the Corpus Spongiosum continues to push the bottom of the glans and the glans tilts upward.  Think of a horseback rider pulling hard on the reins and the horses head lifts up.  Ejaculation is extremely painful and the pain persists for 5 mins.

I am told you cannot put a needle into the glans or it will bleed and not stop.  That means surgery is out of the question.  I have seen two urologist and both call it a Peyronies variant, neither have never seen anything like this before and neither know what to do.

I think my only option is therapeutic ultrasound or shockwave therapy but based on what I am reading, neither have much success.

Any thoughts would be greatly appreciated.
