Doctor recommending plication

Hey friends,

Had another urology appointment and the doctor is now leaning towards recommending plication on my penis. I had great success with RestoreX and took curve from about 35 degrees to maybe 15 (some days better, one side better than other too. Straightest I measured was 8 degrees I think). The curve itself is not very bothersome except that it hinges badly. I don't have ED really but I am on daily Cialis to make it harder because of how easily it can hinge still. Cialis makes it ROCK hard but still able to hinge at times.

Anyway, I'm nervous for surgery. Doctor thinks it would improve my sex life because I won't be thinking about it and I shouldn't have much length less due to curve not being awful. But any surgery is scary, let alone one on the penis.

Anyone have any thoughts or recommendations? Happy with or regret plication?
Early 30s. Downward curve for as long as I can remember. Peyronies from injury as teen. Started restorex/cialis June 30, 2023, seeing fantastic results.

Before I would think about a surgery I would give traction therapy (with a FIRST generation device) a chance. In this forum are some reports of hinge effect improvements.

A very small study supports this treatment for hinge effect:
"The first published study on PTT was published by Levine and colleagues in 2008.21 A pilot study was performed on 11 men with long standing Peyronies Disease (mean 29 months) utilizing the FastSize penile extender (Fastsize Medical, Allso Viejo, CA, USA). The patients were instructed to use traction for 2 h a day increasing the duration to 8 h a day, with the extender rods lengthened 0.5 cm every 2 weeks for 6 months. Reported outcomes included changes in EPC, stretched penile length (SPL), and penile girth. Additionally, erectile and sexual function were assessed using the IIEF-5 questionnaire. A total of 10 men completed the study. All 10 men reported subjective improvements in penile curvature. Objective EPC decreased by a mean 33% (range 10–45°), increased penile length (0.5–2.5 cm), and increased erect penile girth (0.5–1 cm). Hinge effect resolved in 4/4 men as well. IIEF increased at least 4 points in 50% of patients (mean IIEF-5 score increased from 18 to 23.6). Importantly no adverse events were reported such as decreased penile sensation, worsening ED or skin injury."

If you have time, patience and discipline to do this treatment for
(i) a couple of month,
(ii) every day,
(iii) long hours (much longer than with Restorex) and
(iv) with a rod system (not belt)
you may be able to avoid a surgery. (In case you still need a surgery you may are in a better position due to some increase in length.)
Early 40s, no ED
02 2024 injury
04 2024 50 degree upward, start Cialis 5mg daily, start penile modeling (daily 2x3min)
05 2024 start PMP daily ~5 hours, start LI-ESWT (8 weekly treatments), PRP (P-Shot), start Vitamin E daily

I would not suggest surgery unless you have difficulty with sex. It WILL shorten it some and that will bother you more than the curve. I would get another DR. Anybody that suggests  surgery for you is suspect. I am going to post my 4 rules for treatment on your link.
