Dr. Trost Xiaflex Protocol and Followup Instructions

Hi all,

For the folks that have seen/are seeing Dr. Trost for Xiaflex, can you comment on his protocol and after instructions?

(36chambers was going to scan/post these as given from the office, and received permission to do so, but hasn't been on since his last injection.)

I'm about to have Xiaflex (on Wednesday) with a local doctor. He has a lot of experience (involved in the original study and doing it ever since) and seems up on current studies including what Dr. Trost is doing. Dr. Trost went into some good detail answering Sebtp1973's question at: https://www.peyroniesforum.net/index.php/topic,18581.msg157078.html#msg157078

Dr. Trost even summarized as follows:

- Doing 2 CCH injections no more than 1 day apart
- Restorex starting on day 2 (or as soon as feasible)
- Use of 0.9 mg of CCH
- Dilution of CCH to 0.7 ml
- Administering the injections to about 15-20 sites (making sure to remain only within the tunica and making sure to address the septum as well)
- Creation of an erection with the first of each series with medicine administered to the point of max curve
- In-office aggressive modeling on day 2 (and day 3 with the 3rd and 4th series if they're needed)
- Sildenafil 20-100 mg or Tadalafil 5-10 mg nightly beginning day 1 and continuing until 6 weeks post the final treatment
- Wrapping with gauze / coban to minimize the extent of bruising from 4-7 days (depending on specific scenario)

I have to hope that my doctor injects correctly and accurately but on my end, I can do the wrapping, take cialis, and start using Restorex on day 2. Is there anything else you guys recommend? I will listen to my doctor but I do want to mimic Dr. Trost's protocol as closely as possible.

Noticed less morning, and less strong, erections 2020
Noticed dents/groove/hourglassing/hinge near base 2021.
Urologist had me start Pentox, Vit E, Cialis, RestoreX.
I also started VED and all supplements in the guide.

The wrapping in and of itself do not contribute to Trost's superior results. Trost injects a lot more Xiaflex than the established protocols, which would cause massive swelling and bruising. The wrapping prevents bruising and swelling. Wrapping allows Trost to inject a lot of Xiaflex without turning your penis into a purple eggplant. I doubt your doctor will inject this much Xiaflex.  
57 yrs old.
Pre-Trost treatment, 110 composite curve (75 up, 35 left)
Post-Trost xiaflex treatment, 0-15 degree curve.

Interesting. I see now that the standard dosage is 0.58 mg and per Dr. Trost's summary he is doing:

- Use of 0.9 mg of CCH
- Dilution of CCH to 0.7 ml

If my doctor isn't already doing this successfully, I wouldn't want to be his first. I will run all this by him though.

Does Dr. Trost recommend the regular Restorex protocol starting on day 2? That is, 15 straight and 15 angled or something different?

Anything else he or his office recommend?

Hi Dorian. I'm in week five of post injections with Trost.
You pretty much have all the right info. It would be nice if your doctor would confer verbally with Dr Trost.
 The only other thing is the manual pulling and counter bending. He wants you to pull pretty hard. He pulled it hard enough after the second injection until it popped/tore. I heard/felt it. Of course I didn't feel any pain. Manual pulling/modeling was tough. But, you do it as best you can. Once a day at least. It was really hard doing manual counter bending. Too sore. But did the best I could. Eventually it got to where I could do the hard pulling for 30 seconds each. Straight and counter.
I sent Dr. Trost several post injection emails to make sure everything was normal. His assurance was perfect.
Super happy with my results. Hope it sticks long term.

Curious who you doctor is. Location.  
age 62. Onset 2010. 30deg to the left
Did ved therapy for 6 mo. Some help. Lived with it.
In 2020, left bend increased 50-60 
Jan 2023 Did some Xiaflex shots w/ Dr. Trost.
Now, slight curve with hourglassing. hardly noticeable.
Pleased with results.

Dent-n-curve

Bumping this thread looking for some feedback about resuming Restorex.  I had my first go-round with Xiaflex this past Tuesday and Wednesday.  

My doc's suggestion was to resume Restorex after 5 days ..  and as I'm reading here, Dr. Trost is suggesting Restorex after 2 days.

I've begun gently stretching by hand and today tried a little bit of "pre stretch" using the Restorex (just moving it up a tiny bit on the extension rods, no spring release, to hold the equivalent of a gentler hand stretch for a little longer).  Both things felt okay except for some loud stinging pains around the injection sites.

Monday would be the 5th day after the last of my injections.  Hopefully those pains will be less by then, but are they something to expect and endure if they're still occurring to any extent?  

Would love to hear from those of you who have been through it (or if Dr Trost himself happens to be looking)

Peyronies began mid 2022 as pain, dent, curve toward dent, then waisting/hinging into upward curve. Xiaflex w Dr. Carrion, round 4 completed 12/23. Some improvement, but still more curved than penis 1.0. Maintaining w Pentox, Cialis, + Restorex.
