Doppler normal - why cant I get hard?

I have plaque mid-shaft. During doppler they said it's not the worst they have seen in terms of size. My glans didn't fill up too much during the test but I did get quite hard after they gave me a second dose. PSV and EDV all came in at normal values ruling out arterial insufficiency and venous leak.

So why are my erections soft? Is the plaque blocking the flow to the end of my penis? Because that is what it looks and feels like. Also, my libido is almost zero. Does Peyronie's affect mind/penis arousal too?

I'm both happy and at the same time depressed that my blood flow is normal but that I still remain without an answer for this problem.

Yes,
Our mind and mental status probably has the most impact on the ability to achieve and maintain an erection. Peyronies makes us constantly aware that we "have a problem". And this certainly causes anxiety and impacts libido and erection quality.

What Willis said. Completely true. First of all it's good news that you got those results from the US because now you can rule out those possible things as a cause.

Anxiety, mental status. As willis perfectly pointed out so true is that we constantly fixate about having a disease that's affected our penis, we do not feel normal anymore and many keep comparing to what life was before getting this disease. This whole loop of anxiety and depression only worsens the Erectile Dysfunction and most likely even the disease in the long run. Personally my Erectile Dysfunction has progressed worse and worse over the years because of the extra depression the disease caused.

I am sure you can relate to this that some days you magically have almost zero issues getting and staying hard while many other days you feel as if your on the verge of complete impotence.

If you can relate to this then that is a very strong indicator the Erectile Dysfunction is mostly psychological. I say mostly because of course our scar tissue prevents the penis to expand fully as it used to prior to getting hurt so everyone with peyronies has some form of Erectile Dysfunction because of scarred sites not engorging with blood as they used to.

Without the aid of pills I really cant get a normal one anymore, but yes every once and a while I will get one that feels close to normal I guess. And yes I am severely depressed over this. It ruined my relationship with a beautiful woman. We had the best chemistry and were very active and then this disease kicked in and I lost my mind over it.

Would it be reasonable to conclude that if I could break up that plaque that it would help my blood flow better to the end. Because that is what it feels like a lot of times. It's firm on the bottom half and then after the plaque the top end and glans arent as strong. But sometimes my glans does feel pretty firm. I cant future this out. So frustrating.

Should I try verapamil to break up the plaque?

Be very careful with injections, injections can cause more scar tissue

I don't know how to explain this. You are assuming that the plaque is causing your Erectile Dysfunction, and that is not true. Men with Peyronies plaque still get erections, and that is what causes the penis to curve. Erections are triggered by nerve impulses from your brain to your penis. This is why drugs like Cialis don't create an erection during the whole period the Cialis in your blood stream. You still need the mental signals from your brain to start the erection process.

Makes logical sense to me that when an area inside the penis is damaged/scarred and does not expand properly it's only natural to get Erectile Dysfunction as a result and that Erectile Dysfunction can in different patients be almost unoticable while in other patients it gets more troublesome.

It is true what you say about the signals from the brain and nerve endings etc but take me for example with a dented area on the right side of shaft close to base, I can literally visually see the area not filling out like the rest of the penis while fully erect, this causes tilting to the right side of about 20 degrees and instability as the penis feels like its almost loose and wobbly and moves from side to side incredibly easy while fully hard.

Prior to getting that dent, at maximum erection I was ALWAYS rock hard with full rigidity/stability

So I'd say in different cases the disease can defenitely cause advanced Erectile Dysfunction.

I got the P-shot and didn't help, if the P-shot was helpful all the Urologists will use it, so I wouldn't recommend it, but maybe will work for you.

You get an erection when the corpora chambers fill with blood. Scarring from Peyronies forms on the Tunica. While it is true that the Tunica does not expand in the area of scarring, this is what causes the curve associated with Peyronies. Think of it like a piece of tape put somewhere on a partially inflated balloon. It doesn't stretch where the tape is. But the balloon gets its air from another location. And the blood that fills the corpora chambers comes from a different set of arteries and veins. Plaque on the Tunica may impede blood flow in that area of the Tunica, but this is not the source of blood flow that fills the corpora chambers to create an erection.

I stand by my statement that Peyronies scarring does NOT cause Erectile Dysfunction. The cause of the Erectile Dysfunction is for a different reason. And if you assume it is from Peyronies, you are wasting time in trying to treat the Erectile Dysfunction.

Mental health can take a toll on your erection, even before my diagnosis, stresses of life.

After diagnosis I was on the messed up side I didn’t know what to expect.  The first four months I didn’t have this forum, I stumbled on it by accident from googling so much!  This place helped me with everyone’s honesty and experiences.  I learned to keep myself busy.  Music, video games, washing the car…

Right now my erection feels softer, probably about 85%.  I don’t think it’s the Peyronies Disease though, after being my own boss for over 18yrs, sold the business, now looking for a job.  It’s tough to find something that will pay.  I’m just assuming this is the reason, I hope.  Next uro visit is January.

So Willis, if your theory is it isn't the scar tissue that is impeding the blood flow and my test results according to the doctor - one of the best in the US for Sexual Medicine - indicate "normal arterial inflow to the penis and no detectable vascular leak", then why do I not get full hard-ons?

You think it is mental? Like my depression?

I have to say I have smoked cigarettes quite a bit in the last 6 months, which I have stopped doing recently due to stress with this and other things in my life. Do you think it is the smoking?

Yes.
Speaking for myself, when I first learned I had Peyronies, I suddenly was having Erectile Dysfunction issues. It was because I was focusing so much on it. I think when I started to get an erection, my brain was afraid to see what it was going to look like. Or how bad the curve was. Like many have stated on here your mental health has a lot to do with the ability to achieve a full erection. Everything can work physically, but if your brain isn't in the game, an erection isn't going to happen. I don't know how else to say it.

Sounds like you are pretty much describing me here though my Erectile Dysfunction issues started later down the line.
