Have had good improvements recently in EQ. Still having some hinge effect at the base though.

Can anyone tell me of there is a method to get hinge at base to stabilize?

Injury Jan 2020. Was using cock ring. Sudden loss of erection and it flopped to the side.
Erection Quality has been bad since
Uro prescribed Cialis.  Worked for a few days. Now seems to have stopped

VED helped with mine a bit - others found traction helped... Some found no result.

If you have a twist - have a look at user Monty's post he seemed to have fixed twist  with ved

I have the "hinge" effect and traction made it worse. I asked Dr Trost about using the VED and he doesn't recommend it, he said that the VED can make the "hinge" worse
My first symptoms started early in 2019
I tried Traction device, Pentofixiline, Q10, TRT, L-Argenine, cialis
I have narrowing/dent/hinge on the left side of my shaft
My ED is getting worse
Had a PRP shot Aug 2021

Is hinge a twist from the base? Can someone describe what it actually is please

The hinge effect is when the shaft tends to buckle/bend in the middle like a hinge.  It is usually caused by a significant dent in one or both sides (often called an hourglass deformity).

I also have a hinge effect at the base. My penis is bent upwards and the plaque is dorsal. But the hinge effect is at the base on the underside of the penis. I am also thinking of trying a conical pump. Or even with a special pump which you can even wear at night.


Plaque Diagnosed in October 2022.
No treatments so far.

The weird thing to me is that I do have some hinging going on, but generally my penis is straight. I feel like a lot of those on this forum struggle with both curvature AND the hinge effect. If it wasn't for the indentation on my penis which causes the hinging, I think I would be just fine. The curvature isn't an issue. My goal is to get the plaque to go away, trying everything possible that you see in my signature. I read it's common that plaque size would decrease with time, and if that's so, the indentation should also improve right?

I had hinge that prevented sex. Large plague at base caused hourglass dents. Three rounds of Xiaflex and two years later, hourglass dents and hinge are gone, penis is 2 inches shorter (7.5 down to 5.5) and I have a 15 degree curve upward higher up on my penis that I did not have before, but the 30 degree curve at the base of my penis is gone. Go figure. I have no idea if Xiaflex really is what helped or if it was just time. I did not do traction or VED. I miss my longer penis, but my wife is fine with it - now that I no longer have the hinge.
62YO, hourglass indent/hinging at base 30deg bend up 3cmx2cm hard plaque at dorsal base. No ED. 3 rounds Xiaflex decreased plaque/indent/hinge/bend. 1 year later, penis went from 7.5 to 5.5 in, but only 15 degree bend and no hinging.

The fact that xiaflex worked potentially to some extent is definitely reassuring. Nice to hear a success story from it, and good to hear of a hinge effect case resolving. I honestly think if I was not doing VED right now my length would be decreasing as well. To me it makes sense that if the penis is not stretching, the plaque will continue shorten the penis. Needless to say, I am going to try to counter that with the VED.

Dissolving the plaque with Xiaflex does not cure hourglass, according to Dr Trost, because the underlying erectile tissue is lost in the dent. I'm not clear why traction helps, however.

The weird thing is, my dent isn't even where the plaque is, but I do feel like the dent is a result of the plaque being right next to it.

Far be it from me to disagree with Dr. Trost, but the Xiaflex trial specifically excluded men with hourglass and other less common Peyronies abnormalities. So I do not believe there is any scientific evidence for or against Xiaflex helping hourglass and hinge. The "evidence" will only be antidotal until a clinical trial is done on hourglass, and that will never happen. All I can say, is there was a direct correspondence to using Xiaflex and the plaque at the base of my penis significantly decreasing in size, and then the hinge effect went away. I went from not being able to have sex at all, to having "close to normal sex." Whether it is was the Xiaflex or just "time to heal", I will never know, but the time frame of the injections, significant decrease in plaque size, and then getting better, indicates that Xiaflex played a major role in my improvement. So, I would not discount the possibility that Xiaflex can help with hourglass/hinging.

This may sound like a stupid question, but is there greater risk to length loss to those with a longer penis'?

I've seen a few people say they've lost 2inches etc but they're always above average to begin with. 7.5 to 5.5 is still an average penis by the end of it which is good, but if someone starts at 5.5 and went down to 3.5, that's almost a medically certified micro penis? I've not seen that seem to happen to anyone? So do you think it's possible the bigger guys have more to lose so do etc or is there a point where the penis just can't get shorter on itself?

I hope that isn't as stupid as it partly sounds in my head?

I don't think it is a stupid question at all, I was wondering the same thing, being the guy who went from 7.5 to 5.5. And believe me (and my wife! :), a 5.5 inch penis that works is 100% better than a 7.5 inch penis that does not work...

Hey Shane,

We're using the VED or getting daily erections prior to Xiaflex? Just curious if you think you could have prevented the loss in length in retrospect. Or did the loss come as you started Xiaflex? From what I recall you're not supposed to use VED while undergoing Xiaflex right?

I did not use VED or traction before or after Xiaflex. The loss began at least one year after Xiaflex injections and the hourglass/hinge was nearly gone. I also began a new 15 degree curve much further up the penis as it was shortening (n addition to the loss of length due to the curve). I suppose VED and/or traction may have helped. I may start that.

I'd def give it a shot. Imagine like any muscle injury in the body. Usually rehab can help you regain functionality, range of motion, etc. Not sure if that's the same with the penis but worth a shot for sure. The VED certainly is helping me keep my length. A very interesting test would be what I'd look like now if I did nothing at all, versus my current regimen.

Just wanna say thanks for giving some hope, Shane! Truly happy for you that you can at least be sexually active now with the hinging gone. I'm single and I haven't attempted sex, only foreplay just because I'd rather be safe for now and not risk any injury until I feel the hinge is more stable.

So how many injections did it take before you noticed the hourglass going away? And when the hourglass went away, did the plaque go away at the same time (which caused the hourglass improvement)? Would love to hear any notable phases you went through from your hourglass at its worst, until the first sign of improvement.

Also, I know you said you think the injections MAY have helped, but also that time itself could have done it. Which do you think is more likely from your gut feeling? And did time itself prior to injections do anything for you?

I read all these studies about how pentox, heat, time, etc can lead to plaque dissolving. I would assume that if my plaque dissolved, even a little, the indentation/hourglass would be gone too. What are your thoughts on that? I mean, the dent is just a result of the plaque right? From what I understand the dents can in fact be recovered once the plaque goes away.
