Can an implant resolve issues with previous failed surgery

Hello all.  For those that don't know my story, I had a failed incision and grafting surgery nearly 3 years ago to correct a 90 degree bend.  I am not left with a 45 degree bend, some ED, and loss of length of around 2 inches.  All of which make it near impossible, and certainly not enjoyable to have intercourse.

I have tried tadalafil, which doesn't have a huge impact.  My current specialist team keep trying to get me to try injections, but the pain of the erection was terrible, so I am reluctant to try again.

I'm currently having a flare up of constant dull flaccid pain, which I can only assume is from either the graft or plaque pressing on something.

I am starting to think that an implant might be the only option, but not sure if it would be worth considering if it didn't go some way to restore some lost length and alleviate some discomfort.

Is it possible for a surgeon to excise the previous graft and remaining plaque, whilst fitting an implant? I wish I had gone straight for an implant at the time, but the surgeon recomended otherwise.

The short answer is yes it is usually possible for an implant to resolve prior failed surgery.  Exactly how that is done varies by the surgeon.  He will decide if he needs to excise any graft or plaque or whether he can manually mold the penis.  Usually, even a 20-degree bend makes intercourse very possible, and likely an implant alone (especially a TITAN) with no other procedure would correct you to that.

I notice that your age is not in your signature line.  How old are you?

I wonder if you could use a vacuum erectile device (VED) to help restore your length?

I don't know, but wonder it the stretching would help alleviate anything?

Regarding VED, I did have fairly good results with VED, but stopped when I had a skin anchoring procedure and partial ligament separation to try and stop extreme retraction, which caused discomfort from the opening of the urethra rubbing on my underwear etc.  unfortunately the anchor failed, as something snapped under tension when sitting.  During the operation a buffer was placed to stop the ligament for reattaching.  The placement of the buffer makes it uncomfortable when trying to get a good seal for the vacuum to work effectively.

MarkDS, considering your age and from what I have read, I would have gone straight to the implant. At nears 50 years of age, men naturally don't have the erections they had in their teens or twenties. An implant will be the best solution and your age is the ideal time to get one.

Now considering you have lost 2 inches, it would be worth atleast considering or looking into or talking to a doctor doing tunica expansion procedure (TEP). This allows the doctor to increase the length of the penis while installing an implant without grafting by making staggered tiny cuts into you tunica. Your tunica then naturally grows and heals to cover the defect, causing an average increase in length of greater than 1 inch. Please note, that this is relatively new procedure and very few doctors around the world are trained at the time to do this. You should also note that with a procedure like this, the risk is slightly elevated but if it succeeds satisfaction rates are very high.
Tartao on this forum who is around 28 years old got an implant with the TEP procedure and recovered all of his previous length and is very happy.

TEP sounds interesting, but very scary.

I was 46 at the time of the original surgery.  I have always had good erections, which is why the surgeon did not recommend an implant.  However, I think due to the severity of the curve, another surgeon may have thought differently.

MarkDS , it's supposed to be more reliable and safer than grafting, since no external graft is used. It reduces operation time since no graft is needed which reduces infection rate in theory. Hence this new technique has been devised. Only big drawback is that most urologist who would have been trained in grafting might nit do TEP yet and lack expertise. Moreovoer, You have already had the scarier grafting. Read tartao 's  journal on this forum about his experience with TEP.

Good news ! I just checked thanks to your post. It seems that TEP is catching on with the doctors in USA. There is a video on YouTube doctor doing TEP with implant in the us just posted in 2021.
