Objectives: To describe a patient with Peyronies Disease, whose persistently painful erections and psychologic distress were eliminated by a novel treatment involving transdermal estradiol. This report also aims to present a novel treatment for patients with paraphilias.


Results: Treatment with transdermal estradiol decreased the patient's sex drive and virtually eliminated his erections with no undesirable side effects. The patient and his wife still enjoy sexual relations without the need for penile erection.


why are they not researching how to eliminate the pain without making him completely impotent?

Wow - pretty radical.  However, you seem to have missed that the man suffered from paraphilias, which are frequent, intense, sexually arousing fantasies or behaviors that involve inanimate objects, children or nonconsenting adults, or suffering or humiliation of oneself or the partner.  So it's no surprise that he wanted to quell his sexual drive.  The Ottawa University study doesn't mention it, but it's quite possible that if he had paraphilias he could have been a sex offender, or someone who was pathologically driven to relieve himself sexually in inappropriate, distressing, or illegal ways.
 If he was a paraphiliac sex offender, the fact that he also had Peyronies Disease is irrelevant to the study.  I would also state for the record that while it may be interesting as an aberration, the entire story is irrelevant to this Forum, because no normal Peyronies Disease patient would ever be treated with medications to remove his sex drive.

I don't know if you missed the last part of the text,or you mean that they don't mention "him in particular" but on the last part of the text it says:

Replication of this treatment intervention in men with Peyronies Disease and new studies of its use in men with paraphilic disorders are warranted.

So yes, you are indeed correct! Impressed that this would be considered but in this particular situation it makes sense!

Nothing to do with paraphilias stated, and based on one case , pretty hard to see how that "conclusion"could pass the least scrutiny let alone an ethics committee

This is hilarious and ridiculous. Making a mockery of us peyronies sufferers. Basically telling us to get ourselves castrated and have 0 sex drive. Lol... if you can't fix it... break it completely lol.

  It's definitely a weird write-up of the study, because it's not clear if the main who suffered painful erections was the same man who suffered paraphilias.  It's almost as if they deliberately wrote it ambiguously to hide facts which might reveal the patient's identity.  
 We don't know if he was a sexually normal Peyronies Disease sufferer who had such intense pain from his erections that he chose never to have an erection again.  Meanwhile, the way it's written, it appears that both he and his wife were cool with some version of sex that didn't involve erections.  It also says his sex drive was decreased with "no undesirable side effects", which makes it sound like both he and his wife wanted him to have less sex drive.  Okay, this is definitely not for me, but even without an erection one can still have intense sexual pleasure and orgasm, so if that's what they wanted, it doesn't sound like a complete disaster.  
 Meanwhile, if you read between the lines, they talk about using this treatment for sex offenders who simply can't stop themselves from raping anything, even children, animals, and inanimate objects -- so it might indeed be the same guy.  And I can see how guys like that would regret their actions later and want their sex drives reduced.  
 In any case I maintain my original assertion that this is only marginally relevant to this forum as the vast majority of Peyronies Disease sufferers don't want to lose the ability to have an erection!
