Incision verses Excision Grafting Questions

I have read over the two type of the above grafting and understand the theory.  I am not sure which one as yet my Dr. my decide to do.  If not just a second placation.  (yes i am seeing a different Dr) From all my reading I am getting a little mixed up that one of the two mentioned is more dangerous as far as hitting the nerve that gots to the tip gland and knocking out feeling.  Are these both the same danger?
Also, as far as the grafting material... I did see a video on it, but not sure how that works.  
Is the recovery much longer, and is the pain much more than just a placation?   I am assuming that if my placation is undone and I end up with a grafting, I will get my length back, or am I under the missinterpertation of that as well.
Thank you for any claiarification you can give me on the two graftings.

Maybe I worded this wrong... .and forgive me.... is there a difference between these two?  I did read in the header and thought the proceedures were different.  But I was told to stay away from one because it may cause nerve damage

Yes, there is a difference between the two.  Incision and grafting (which I have had done twice) involves incising the plaque (to release pressure and resolve curvature) and then placing a graft over the area.  Excision involves removing some or all of the plaque followed by grafting.  Excision is generally considered more risky due to the dissection of the plaque rather than leaving it in place.  Both options can result in temporary or possibly permanent loss of sensation.  If you havent already I would highly recommend seeking out one of the specialists recommended on this forum.  Personally Dr Levine (Chicago) is my doctor after a failed procedure in Michigan. Incision/grafting/plication at U-M 9/20 failed.
Implant / graft repair with Dr Levine (Chicago) 9/21, successful so far

Guy, thank you... I had the placation that has failed, and now i fear the grafting is next.  How long did it take you to recover from your second, and did you get sensation back.  I assume there is more pain after than just placation with stitches.

As I recall, after my first surgery (which was incision and grafting only), I had to wait 6 weeks to resume sexual activity and things were fine sensation-wise except that I was still significantly curved (the surgery had failed).  My 2nd surgery occurred on 9/23/2021 and involved a circumcisional incision to deglove the penis, followed by incision/grafting and placement of an implant.  I was cleared to return to sexual activity again after 6 weeks and the sensation seems fine except its still a little tender around the circumcision incision.  I am told this may take up to 3 months to completely get back to normal.  With regard to pain, the first surgery did not create much lingering pain after the first week or so of recovery.  The second was somewhat worse but I was only on prescription pain meds for about a week post-surgery.  After that I was able to manage with ibuprofen, etc.  I would not let the potential for pain dissuade you from getting the help you need.  Worst case, you can have the Dr call in additional pain meds if you have lingering pain.  Much depends on the exact type of surgery you need and what techniques are involved. Again, a highly qualified Peyronie's specialist can guide you on what is best for your particular case and give you an idea of what your recovery might look like.

This is very informative... that second one,, I am assuming that the implant is the one you pump up because I do not know of any other type.  So you pump up when your ready.  I am going to a well known Dr. mentioned in the site.  Boy, this really sucks.  You are much younger than I am and sorry for the problem you are having.

Yes I received a Coloplast Titan 3 piece implant as part of my second surgery.  The concern was that after a second incision and grafting surgery the likelihood of severe ED was high and there was no point in having a third surgery to place the implant.  Yes, you pump up to achieve an erection.  

Four years ago, before this whole thing started, I would have never expected to be where I am but things could be worse.  Once I am healed I am going to have a normal life again and while Peyronie's sucks, there are a lot of other diseases that would be worse.

Thanks for the information.  Sorry to hear your first incision and grafting failed.  I am in the same position, and wondering about my next steps.  

Did you have further loss of length following your second procedure, or were you able to gain any from the failed surgery?  I've lost around 2 inches since pre peyronie's, probably an inch since the first surgery, and significant girth.  I still have residual curve of around 45 degrees, weak erections and constant pain/discomfort.  So I am considering further surgery, although the prospect scares me to death.
