  Hi. As you can see from my signature i've had peyronies since december 2017. Acute stage lasted about 16 months and although i attacked it from every angle it progressed to a large dorsal plaque with 80 degree bend, i lost about 1 1/2 inches in length and plenty of girth .  I've carried on with every peyronies supplement known to man and continued with traction and bouts of VED, with very little improvement, although i'm convinced it would've been worse without it. November 2020 i managed to injure myself again when i lost an erection during sex, which resulted in a small pea sized plaque on the left mid shaft, but i managed to keep my erect length to around 6 inches but my girth went from 5 1/4 to 5 inches.

   After a year or so in the nhs seeing uro's and getting let go when i didn't want the nesbitt surgery , i went private last spring and went to see dr ralph in london. We talked about an implant and he measured my bone press flaccid as 14cm. i sent him email pics of my erection with measurements and also a picture of a semi flaccid hang of 16cm. He said after an implant i would end up with at least 14cm straight penis and if we released tissue with incision and grafting i may be able to gain some length, but rigid straight erections are the aim., but with any luck either way i may gain a centimetre or 2.

 I've been really thinking about it over the summer as quite like the feeling of natural erections, but to be honest sex has become harder as my wife is now in menopause and last time we had sex it was uncomfortable for her. She has lost libido and isn't overly worried if i have surgery as she could live without sex, but i'm 59 still a horny bastard, it's awful watching my dick deteriorate and i know at my age it will only get worse.

  What's really made my decision more necessary is an accident at the weekend with the ved where the tube slipped out of the ring and went downwards bending my penis. I'm now in pain at the newest plaque site and had a really really painful erection in the night. I feel i have to decide now as i don't think i can take another few months battling to keep my dick alive. Maybe i should just bite the bullet and do the implant route before i start losing more length and girth.

 Any advice would be gratefully received thanks.

To be honest, it's like history repeating itself reading your post as I've read so many similar stories now on this forum over the past 3 years. Your bend is in the absolutely extreme end of the spectrum and your penis is shrinking from plaque as time goes by. As has been said so many times in all of these other posts, do not waste any more time on a pointless journey to find alternative solutions when it seems to me there are none that will have the same end result as a Coloplast Titan implant. Your bend is extreme, and like me, the implant was the last throw of the dice. As many have pointed out before, you could have saved yourself all of the stress and agony and simply jumped to the final outcome - the implant.

Please, before anyone jumps in and accuses me and others of promoting implant surgery, I can only say it as I see it. Your case sounds extreme and you need an extreme solution. It's not like I'm suggesting you take a healthy dick and implant it. It's the opposite.

As has been said ad nauseam by many, if I had found this site years earlier, and learned about the existence of an inflatable implant, I would have not gone through as much physical and psychological distress as I did because once I had the Titan implanted I honestly cursed the many years of healthy sex and relationship I'd lost.

It's truly a no brainer. From a 90 degree bend to absolutely straight in one 90 minute surgery, I am the living proof.

If you need any advice, just ask.

Don't worry, you will know when you have had enough and are ready for surgery and it sounds to me you are there.  I have no clue how you had sex with an 80-degree bend, however.  That deserves a post of its own.

The facts are I have never known (close to two decades on the forum) a man that went from 80 degrees to 40 degrees without surgery.  I cannot think of any reason why a 60-year-old man with an 80-degree deformity would not be done with this ordeal and get an implant if he could find a good surgeon and I think you found one.  Your wife feels discomfort.  You are at risk of more damage.  Your erection quality will likely diminish in the coming years.  A properly installed implant will fix all of this.  I would push for a Coloplast Titan but it would not be a deal-breaker if the surgeon insisted on another model.

One day I just had it with the tool kit, hours of traction, VED, pre-planning sex and oral meds, supplements.  I started the ball rolling and NEVER looked back.  If you read our journals, you will see that to the man, we all wish we had done it sooner.

  Well i'm booked for a titan implant in london mid february. I'm obviously going to put in the hours with VED and traction before the surgery, But i'm not sure what to do about my ridiculous supplement regime. I take 1 pentox tablet a day,2250 mg arginine/cytruline . 100mg acetyl l carnetine, 30 g pycnogenal, 100 ubiquinol and in small doses ginseng/ginko, maca smoothies, ashwaghanda and a few vits/zinc. I know some of these are blood thinners and probably not good before surgery, but i don't want to stop taking them and lose blood flow to my penis while i'm trying to maintain size. Do you think it's ok to continue taking some of these?  I've cut myself at work lately and it didn't seem to bleed excessively and clotted ok.

I know that even before regular surgery Dr's will have you stop taking nsaids and aspirin products 10 days before to keep you from excessive bleeding. You probably should ask your Dr what they suggest.

Hi Philtered, I'm under prof Ralph as well and have been considering an implant to correct my 45 residual curve and ED following a failed incision and grafting surgery (from different surgeon).

Can I ask what traction device you have/looking at getting?

  So i had my implant surgery on friday by prof ralph and his team. He just put a titan implant in without incision and grafting and he says i can straighten out my 80 degree curve in 6 months with modelling and cycling. The surgery went well and i was left 80 per cent pumped up in the hospital overnight, which was pretty painful to be honest.

One of dr ralphs assistants came the next morning and deflated me which was pretty hardcore but said that was the worst it will ever feel. He said he thinks i was fitted with a 23cm titan with 1 cm rte's. He said it went in easily with no complications.

It took a while to get discharged as i couldn't wee properly  after the catheter was taken out and they thought i may have retention. I drank 2 jugs of water and once i started i couldn't stop, although this burnt quite badly and has been gradually been getting better since being at home for 2 days.

I couldn't get picked up so i had a pretty horrendous trip to the metro station then 2 underground trains, then a  1 hour overground train to my house. i was walking around like john wayne with hemorrhoids getting some funny looks. I managed to tuck my dick sideways in my boxers and baggy joggers but it hurt like hell.

Iv'e pretty much laid in bed since i got home as my dick sticks out at 3 oclock with a residual upward curve and feels really heavy and swollen. My balls have been pretty huge since yesterday as well, it feels like there's alot of fluid in there, but the pain is fine if i don't move around and a bit uncomfortable if i do, but manageable. My scrotum is black n blue and some of the shaft but this is normal i suppose.

I will go back in 2 weeks to start inflating/deflating and was told to leave it alone until then, showers not baths and prof ralph says i'll be able to do some drumming recording i had booked in in 2 weeks, but i find that hard to believe at the moment ha ha.

I'm pretty content at the moment but pissing etc is quite a challenge and i'm bored already with laying around. Will keep posting my recovery as i'm only 3 days post op right now and i'm sure there'll be plenty of twists and turns.

A very positive post too, Philtered. I remember that John Wayne walk very well - it was as awkward as it was funny. Good for you - take it easy, stay ahead of the pain when it comes (Tramadol I was on) and it'll all be good. Playing your drums in 2 weeks may be a tad optimistic in my book but everyone is different, I guess. Stay well and ask as many questions as you need to over the next 4 weeks.

  4th morning since opp and everything seems a bit less swollen and sore. I did actually feel the pump for the first time and tried to push it around a bit which has created some pain. Gould knows how some of you guys start cycling and pumping on the 3rd day, you need some kind of medal for that.

  So this is my 5th morning post surgery and i'm struggling a bit. i was hoping to have made more progress, but i'm still really swollen. My scrotum is huge and black n blue and seems to have loads of fluid inside. i can feel both testicles and the pump separately though which i suppose is a good thing. The pump is higher than the left testicle.

My shaft still pokes out at 3 ockock when standing up and is quite a mixture of black blue and red with yellowish tinges.

I literally lie on the bed and go downstairs slowly to get drinks and food and that's it. Ralphs prediction that i'm very fit and could be back at work within a week and drumming within 2 looks like a dream right now.

I;m trying to stay positive but i'm going through a "what have i done" stage right now. I'm just praying i haven't got an infection which is slowing any progress. The broken sleep lying on my aching back isn't helping.

Hang on in there philtered, I'm two days behind you but had a WTF have I done Moment this morning. I've been in hospital for the last two nights and going back to my air b n b soon and then have a 500 mile drive home on Sunday, it was going to be Friday but due to bad weather and the way I'm feeling I've extended my stay.

Im actually not that bruised, a little swollen around the balls, but my abdomen really hurts, I've not been for a number two since Sunday night so I think I'm just constipated 😂

Same comment to both Philtered and Jimmi as you are both fresh out of surgery and questioning whether you have done the right thing.

Yes, rest assured you have done the correct thing. You have both sought the advice of the top urology surgeons in the UK and both surgeons thought the said course of action was the correct one for you. You are having doubts within the first week of post surgery psychological trauma. I remember it very well indeed. I was sitting on the edge of my bed at home asking myself and my wife WTF I had done to myself. I was at a very low point. The pain was intense - constant, and starting to cycle made the pain unbearable at times. If you read the early part of my journal again you will see what I mean.

However, the guys on here kept me going. I took advice, I took strong painkillers and I stayed ahead of the pain. You need to do the same when that time comes.

Your pissing issue will not be an issue when you are not erect. After all, who does a piss with a hard on. Just now is a massive physical but also psychological adjustment for you.

Stay strong - I know I repeat myself with this, but you are 3 weeks away from having unimaginable sexual pleasure without the worry of PE, inability to gain an erection or how to get your bent dick into positions to have meaningful sex. It's all in the past for you both now.

You'll see. Have faith. All will be good.

Yesterday was exactly a week since surgery and i had my best and worst day so far. It started when i woke with intense pain in my left ball which is vying for position with the pump and tubes. It was so bad when i got up and walked around i convinced myself i must have an infection or something as i thought i should be getting better not worse.

I emailed dr ralph with a picture of swelling and bruising and he got back to me(top man) and said he would phone me mid morning. In the meantime i did something i haven't done yet as i've just been using ice. I heated up a wheatbag i use for my bad back and put that on my nutsack lying on my back with nuts raised. This seamed to loosen everything up and i got instant relief.

Ralph said everything looked ok for one week, although i do have alot of bruising swelling. My chances of infection are low as surgery went very well and i've done my course of antibiotics. He also said i can have a bath which was music to my ears. He also told me to get moving and not just lie around. So i've had a bit of a mindset change and i've felt really good about it ever since, even though i do still get shooting pains when i stand up straight or move around, but they ease the more i walk around. I've got work booked in for next week and i'm not looking forward to that and i go to see ralph on thursday for cycling etc.

 Onwards and upwards.

  Hi i'm at day 10 post opp.  The swelling and pain were definitely less this morning and my walk downstairs to make morning coffe and breakfast was pain free. I did some icing and then a heated wheatbag and my second walk downstairs was far more painful infact the most painful for a few days. This is so weird i thought i'd be back at work and stuff by now but that's totally out of the question.

I sent 2 pics of the incision site and my glands to one of the andrology team and she said it looked to be healing well, but i just feel its taking time.

Did anyone else feel heat in their scrotum or shaft as they were healing? i seem to feel quite hot down there the last 2 days. I just wish i wasn't so paranoid about infection all the time.

You must take your recovery slowly, Philtered. There will be good days and bad days. Heat, as you know, is a fantastic natural painkiller and when you come to cycling (soon) the best way to keep this as pain free as possible is to cycle in the bath with everything submerged in the hot water. This is THE best advice Hawk gave me and the others.

I don't recall my balls/general area feeling hot like you mention tho. I can't offer any advice to you about that being/not being a sign of infection. Keep us posted if you can each day with any observations/questions.

  12 days post opp and things are getting better very slowly. I've put of my visit to london for cycling until next week as i'm just too sore to travel through london and i cant imagine squeezing the pump right now. One of ralphs team said it may set me back and one week won't make a difference to my outcome.

 My main problem seems to be the pump is quite high, so the tubing comes from the left base of my shaft, the base of the pump is central , but the pump kind of lays to the left probably 45 degrees towards the tubing. So at the moment still with a bit of swelling the pump pushes down on my left testicle which is really uncomfortable, to borderline agony at times if i walk around. It then burns for a while when i lie down, but after a while the pain subsides. I just cant see how this is going to resolve itself without the pump moving a bit, but there's no way i'm pulling it around right now. I drive about 400 miles a week in my job and get in and out of the van many times during the day. I'm also a drummer and cant imagine every time i sit on the stool my ball will push up into the pump. I'm hoping all this will resolve once the swelling and bruising around it resolves.

 Do your balls get used to having something rub against them constantly?

Just remember 12 days really is not a long time am given what trauma you go to to get to this stage. I actually saw a video yesterday of dr eid implanting someone - I am so glad I did not see it before I had mine done!

I am surprised you still have swelling after 12 days. I know you are icing and using heat so it does surprise me. I'm not saying it's because there is anything wrong so don't panic but would be good to get others opinion on this.

As you know I am three days behind you and like you I am getting better each day. I can properly feel the pump, but also a lot of tubing which is a little concerning but ultimately I just want it to work.

I honestly can not walk around for more than a minute before I have to collapse on the sofa. And with that I'm puffing and panting for breath. I think / howl this is normal and I am thinking it is because my pump is very low and adjacent to the tubing and might be rubbing against it.

Just remember to support yourself adequately, take pain meds on cue, and do not over do things but try and do a little bit more each day.

You'll get there.

 i don't think it's necessarily swelling which is miles better, It seems to still be very sensitive around the pump and tubes and i think there's still tissue and stuff around them . If i wear tight underwear it pulls my left nut into the pump and if i walk round naked it feels fine until my ballsack contracts with the cold and they all squeeze together ;D. I'm sure all this will resolve but i still get wtf have i done moments.

Both of you describe the same kind of situation. Like I said in my post to you both last night, at this moment in time, so early in your healing, your balls absolutely hate having foreign objects in there with them. It's completely alien to them and all the nerve endings in the area. That is why you feel like you are going through hell just now, how a simple thing like sitting down on a chair is such an issue, and how simply walking causes a lot of discomfort. This all passes in the natural healing process over 8 -12 weeks. One day, who can say when, you'll just stop noticing the pain, the wee razor sharp tingles that catch you and take your breath away, or the dull ache that makes you very breathless like you describe. It just happens in time and you'll not really be aware of the pump, switch or tubing. Like I said, your balls are wondering WTF has happened and who the new guys in their house are.

 Thanks roddy. i think i know whats causing most of my problem now. The pump is actually attached to my left testicle, they just don't move independently . Will this resolve itself?

For that one you really DO need to speak to Mr Ralph. I have been through lots and can offer my advice but I will always stop short of giving actual biological/medical advice that I know nothing about. Best to contact him asap.

 Been to see prof ralph, i've got a hematoma around the pump. It will resolve itself in a week or so, but it's hard to start inflate/deflate until its resolved. I thought my pump was huge ;D.  Everything else looks fine no infection.

A weeks delay isn't going to make any difference at all so I wouldn't worry about that.

I really wish i could come on here and be totally positive but i'd be lying to myself and you tbh. This has been a really tough 3 weeks and i just can't believe how slow the recovery is. I did read journals before i went into this, but maybe i read the more positive ones where people were cycling really quickly and having sex within a month, or just thought that's what would happen with me. Plus dr ralph said he thought i'd be working and drumming again in a couple of weeks as i'm a very healthy 59 yr old with no real underlying issues.

Well the first week was absolute hell with my scrotum a massive purple ball of fire. In hindsight i think i made a massive mistake on release from the hospital the day after the opp. My drain was fullish and dr ralph said it was borderline that they would leave it in ,but they took it out. I really should have got a cab/taxi, but instead i waked quite a long way to the underground, got 2 metros and then an oveground train. This included stairs, escalators and long walks through tunnels etc. I think this could be what has caused my hematoma which is still stuck around my pump and attached to the front of my scrotum, but i could be wrong. I then laid on my back for a week allowing this to solidify. Again this is a guess. But for anyone reading considering an implant, don't do what i did.

2nd week i thought i'd go back to work and would be going to see ralph for inflate lessons, but still way too much swelling and pain for that and when i did go to the clinic it was just to make sure i didn't have an infection which is good news.

3rd week i've actually gone back to work as i'm self employed so don't get paid for lying in bed, have customers who rely on me and have been waiting on my return. I've driven 300 k this week sort of balanced on a cushion in my van seat not knowing which way to have my sticking out dick pointing. When i do arrive at peoples houses i have to arrange myself in my special underwear(still haven't found the best solution) so i don't look like i'm walking in with a stiffy. I'm in pain the whole time during this and when i get home the first thing i do is get naked and lie on the bed for relief while the bath runs.Then i get questions in my head "will i ever be able to wear normal workclothes? rearange my dick etc.

My wife had a breast cancer opp on tuesday for a small lump which they found a couple of weeks ago, so i've had to get up before work and take our 2 big dogs for a walk in the woods for an hour before work. Now i do realise i've made massive progress to be able to do these things after being able to do zero for 2 weeks, but believe me i have my teeth gritted during most of it.

I'm going to the clinic on tuesday but not sure if i'll be able to start cycling as the hematoma is still large, stuck and i just can't make out the deflate valve at all. Because the pump is still quite high and stuck my left testicle is trapped below it, so sitting is hellish. Sitting on a drumstool is impossible and drumming is such a massive part of my life, i've been in a "why have i done this" mode this week. I love drumming, i do hit workouts everyday, i'm a physical guy who chops wood for the fire etc etc etc and i'm just wondering if i've sacrificed all this just to have a straight dick. I'm sure this will change, but at the moment that's where i am. I suppose not actually being able to inflate and see what i've done it for isn't helping, infact with dog ears, a bit of residual bruising and bends my dick looks like roadkill ha ha. I read a few things on FT where guys had there scrotum stuck to the pump and it took six weeks to resolve, that seems ages to have a trapped nut and no cycling, but we're all different i realise that.

I do actually feel better about it all this morning and realise it's one day at a time, i'm just very impatient to get back to some kind of normality. Especially with my wife recovering from her opp at the same time which is for a far more important reason than mine. Tough times .

Firstly, I'm very sorry to hear about your wife. I hope all is well with her long term and you both remain very strong.

I'm also sorry to hear that you've struggled this week. I know you have read my journal but maybe read the first 4 weeks again as you'll see I experienced a lot of pain and had very low mood as a result. It would be good to read that so you can once again appreciate that, in time, everything will be just amazing for you - same as it has been for me and others.

I really empathise with you regarding clothes/trousers. I vividly remember thinking I'd need to throw out all my old jeans etc as none of them fitted me now after my op. The worst thing was that my implant made me look like I had an absolutely massive dick, and a hard on, and there was no way I could go out like that. I had to buy new, baggier, jeans/trousers but am so glad I never threw out all my old tighter jeans.

Just to give you a motivational wee pick-me-up, I've just taken a photo of me, today, in my old skinny jeans that I wear regularly. As you can see, long term, your dick will return to normal and tight trousers/jeans are not an issue. You can't even see my package in the photo as the implant has softened a lot over 2 years.

Hey Roddy. There are normally apps to convert photos that you can download from the Appstore? At least for android they exist, so i imagine it should be the same thing?

So i'm 4 weeks since my implant opp and i'm late to cycling due to a hematoma at the pump which is improving. Dr ralph inflated me this week but i haven't had the full lesson as there was still some swelling and tissue around the pump and deflate button. He said come back in a week or 2 but i could have a go if i like.

Well a couple of days ago i did a semi inflate/deflate just to make sure i could. Today i had a bath and went further, i inflated until my very pronounced dogears straightened and there was a bit of pain , so i left it there for a while. After a minute or 2 the dogears started coming back slightly so i gave it a few more pumps and they disappeared. They came back slightly again before i deflated which was probably 30 mins in total.

My question is, is this slight deflation normal in the early days, is the saline just finding it's place and the tubes finding their place as well? No chance there's a slight leak?

My scrotum is on fire where it's still attached to the pump, but i was expecting that.

  Tomorrow will be 6 weeks (42 days) since my surgery. Slow and steady progress is probably the best description. I went to see dr ralph today and although there's still some swelling and my pump is still partially attached to my scrotum and there's still some tissue around it, he got me to inflate and deflate the implant with no problems. My curve has gone from 80 degrees to 30 and now i can cycle every day we think this will improve further.He showed me a bending exercise to help with the curve and help regain size.

 I voiced my worries about the dog ears, one of which is very sharp and painful and doesn't completely disappear when inflated, although it's just a small nipple like bump at that point. He explained its where the dent from my peyronies scar is and it will improve as the cylinders soften. It's almost a z shape in there when it's fully deflated and he said if i put a few pumps in it will be less pronounced and painful. The problem with that is i'm already sticking out at about 4 0cklock and with a few pumps i'm back at 3 oclock and its almost impossible to conceal. He says all this will get better as the swelling goes down which is taking quite a long time. He says he sized me to the max but in his opinion i'm not oversized and the cycling will help with all the issues. Infact he said sex which he gave me the go ahead for, improves pain and dog ears etc, which seem unfeasible at the moment with the soreness and dog ears, but i'm more than willing to try Ha Ha.

 Anyway i feel much more positive about it all now after a pretty tough few weeks and looking forward to sex n cycling my way to a new future.

My cylinders (especially the one the right) are a "Z" shape when totally deflated.  Since Titan cylinders do not have much expansion and contraction in length that is typical when deflated.  Because Titan cylinders are hydrophilic they bind with body fat and soften so it becomes a total non-issue within 12 -14 months.  At that point, they feel like silicone or rubber and could be mistaken for a large vein if someone was unaware. When laying in bed, I would constantly press and massage the dog-ears in my idle time.  It was more of an obsessive habit than a well-thought-out strategy but it might have softened them sooner.

  Thanks hawk. I'm not really worried about the z shapes and folds, it's just one of them has a very sharp corner that sticks out and is uncomfortable. I was concerned it may damage my tunica but dr ralph said it cant.

The curve is definitely improving even after a short time cycling, but the spiky dog ear is causing me lots of pain all the time when flaccid. I've been told to put a few pumps in, but i have to put alot in to start straightening the fold and to nearly get rid of it i have to pump to full inflation, i can't go to work in peoples houses with an erection. It's started causing pain tucked into underwear so it's a bit of a nightmare all round.

It's obviously making the tunica sore but i just don't know what to do about it. Have a rest from cycling until the inflammation gets better or just keep cycling and hope it softens up or the tunica stretches enough to not be so painful. Has a dog ear ever gone through the side of a tunica?

Hi philtered, sorry to hear you are having so much trouble with the dog-ears.  I had a 20cm Titan implanted last fall and when I am deflated I can press and it "crinkles" a bit but no pain and no sharp edges.  My cylinders are still rather stiff and havent softened up much but I understand that takes time.  All I can suggest from my viewpoint is to ask your Dr.
55 y/o, Peyronies Disease onset 2018, diag 2/19.
Pentox failed, Ref to U-Michigan, doppler showed 45D right curve and 80D dorsal w hinge effect.
Incision/grafting/plication at U-M 9/20 failed.
Implant / graft repair with Dr Levine (Chicago) 9/21, successful so far

The pain will diminish as time goes by. It's a process I am sorry to say. Perhaps some day, they will invent cylinders that are only real stiff when inflated. They will soften. The longer the time, the softer they get.
But I remember mine on the right side as being painful. It felt like they were going to break through the skin. I figured out if I wore tighter underwear to hold my penis more upright, the crinkle(dog ear) was less painful.
It's only been a very short time since your surgery, so be prepared for the long haul. Eventually they will soften, but it's a long long process.
In the meantime, Try to find the positioning of your penis, where they protrude the least.

I've asked my surgeon and a titan rep and they both said it will improve, but i do wonder if i'm a bit oversized.I have 6 dog ears but the others don't really cause a problem , just this one.  hopefully as i stretch and the curve improves it won't be such an issue but it seems to have become more painful with cycling. I'm hoping it's "no pain no gain"

 I wore my penis upright in my underwear at first, but this seemed to make the curve worse as it bent back on itself. I've been wearing it down the last few weeks in seperatec boxers which has been better, but this has now become painful since i started cycling. I realise this is a long haul and things will improve , i'm just a bit impatient for a tad of normality. I'm going to france for a holiday for a couple of weeks, so a break from work and the need to hide my dick from customers the whole time will bring some relief i hope.

I think At your ( and mine ) early stage it is not So much what is comfortable but what is necessary.

Hawk and others have drummed into me and others how important it is to fully deflate to avoid auto inflation issues and maximum inflation ( I think? )

I suspect as times goes on this becomes less important but I am ensuring I am as deflated as possible right now. I see no reason why I wouldn't be fully deflated except when cycling or having sex etc as it is much comfortable.  
37 yr old type 1 diabetic. Implanted with a 18cm Titan by Michael Fraser 14/2/22 using scrotal incision. Has implant due to 70 dorsal curvature, erections were average ore surgery
