Does Xiaflex occasionally worsen Peyronies Disease symptoms?

I’ve seen this elsewhere on the web—men, in surprising numbers, claiming that Xiaflex has had perverse effects. In other words, their curve has increased noticeably, or has moved without reduction; they’ve lost another inch as a result of Xiaflex; they’ve experienced a five-fold increase in scar tissue; etc. From this forum, I see very little evidence of such horror stories.

Right now I’m less interested in how these accounts got on the web and whether they’re from fake or real people.

I’m curious as to whether anyone here has any knowledge of such perverse effects of Xiaflex as a treatment for Peyronies.

I haven’t tried xiaflex but I can say that verapamil injections gave me more scar tissue. I don’t believe injections are the answer for peyronies.

Every therapy has advantages and disadvantages.  The more aggressive the therapy, the more likely you can get permanent side effects.  The challenge with Peyronies Disease is that it has a 'natural history' of progression, even in men who are otherwise stable for a period of time.  Because of that, any time you intervene using anything which can cause inflammation, you can get some reactivation of the underlying disease.  So, it's always a matter of weighing the risks / benefits with any treatment.

In general, and after performing thousands of Xiaflex injections, I would say that the far majority of men are better off after they started than before.  Typically, in my experience, men who would say that they are worse off after Xiaflex are men who stopped after one or two series and didn't finish it out.  These are the men who get a hematoma or something similar and then stop treatment.  They also typically don't follow the recommended instructions on wrapping, stretching, etc.  However, there are some who will get long-term discoloration of the penis, and less commonly, you'll see some who report new bumps, deformities, changes in erectile function, pain, and other symptoms like that.  The challenge with Peyronies Disease is that most men just want to be the way they were before the onset of the condition.  And we all want treatments which perfectly improve all aspects of the condition without any possible side effects.  However, this is typically not possible with any therapy.  Rather, it's a matter of trying to improve some aspect of the condition (improving curvature, improving indentation/hourglass, improving length) while giving yourself the best odds possible that the final outcomes are better than the long-term side effects.

Dr Trost,
   Curious if you’ve ever seen improvement on narrowing from xiaflex. It seems data only shows improvement in curvature.. I have slight narrowing under the glands down a quarter of the shaft. Was curious if there was any hope for improvement

35 yrs old
Onset 05/01/20 diagnosed 05/14/20. Taking Pentox, trental, multivitamin, Zoloft & Tylenol
Traction was painful and made it worse
Lateral plaque by glands 20 degrees left/ narrowing
Lost about 2cm length
