I had my implant surgery on the 12th of july. Sent home partially inflated. needed to return to work so dr deflated it enough for me to not look like a pervert while working.  my questions are:

1- is there a best practices regimen like 1 hour full inflate followed by 1 hour deflated etc.
2- i also had my peyronies bend repaired via excision graphting procedure. Does that change my inflation regimen?
3- I hear these stitches should dissolve. How long does it take because they are like sharp pins as the become visible and popping out?
4- How do you know you have reached the pumps maximum inflation?

Hawk is the one for guidance for you.  I know that individuals who undergo implant surgery that there is a special journal that  they post to for future readers to give them hope and info. Hawk will probably set you up regarding this.

The protocol varies greatly by doctor and procedure.  Surely your doctor gave you discharge instructions.  To even discuss typical protocol we would have to know
1. Did you have a Penoscrotal, Retropubic, or Sub-Coronal approach.
2. Did your surgeon use dissolving stitches
3. Did he clear you to start cycling your implant ( that varies from 3 days post-op to 3 months post-op)

Since you have not posted in two years and never posted about getting an implant and since you have no signature line, we don't even know who your doctor is and what implant you have,

Hello,
I've updated my signature line as requested.

I left hospital with 85% inflation.  one week later (one week earlier than scheduled) went in and dr slightly deflated so i could return to work and not look like a pervert or like i was walking around erect.  Since then I have attempted deflation but not very hard as im still sore down there.
today i have made several attempts to deflate and it aint happening. squeesed the button and feel little or no deflating going on. tried squeezing penis while holding button and still no luck.
beginning to feel a bit depressed since swelling has reduced and seeing just how much penis ive lost in 6 yrs. also the pump seems to be very forwardly placed in scrotum which also reduces length between it and the glans.
im not trying to sound ungrateful for the benefits i will get from  modern medical technology but man o man. or should i say not man o not man.
so now my scrotum has been fixed a good inch or two up my shaft putting my testicles out in front of my crotch instead of hanging like normal. between the loss from 5 yrs of peyronies along with my relocated scrotum ive lost close to four inches. not an exxageration either.
the only positive thing is that now it will get hard. yep my teeny tiny new pin dick will get hard.
Ive never expressed my feelings about this disease since getting it to anybody but the couple uros ive seen. and honestly they could care less because i only represent an income source to them.
sorry for ranting but i need to do it one time.

i'm not a whiner or complainer. im a guy who solves problems for a living. this is my official complaint or rant to (God- Allah- Buddah or the universe) that has allowed rapists and child molesters to continue lives with healthy dicks while working in your houses of worship and then condemning some damn good men to a life with peyronies.  F~@< YOU!! now you gotta forgive me. right? or dont. You're fired.

Hey man, you're just a week or two out of surgery right?  One of the most common things I see from guys getting the implant is concern over their post-surgery size, but these are almost always followed by posts months later saying that with repeated use, they gained length as things adjusted and stretched repeatedly.  

Please don't panic over what you're seeing now - you're still dealing with some swelling and other issues, I'm sure. I've always hear you have to give it time to get it to where it's working like it's supposed to.

Don't judge the future based on what you're seeing now.

Hang in there, you've taken a big step ... now just try to have some patience with it. You've seen how a penis can change with Peyronies Disease - it can also change with an inflatable implant, as well.

I believe also that you will regain back size with time.  Healing does take time and seeing our penises different from what we've been accustomed to can be disheartening.  You will make it through this! I agree that it is an injustice that good guys like us get this awful disease and rapist and child molesters get off scot-free.

Just woke up and open the forum to try deleting last nights negative post because I didn't want to be that guy.  Now I'm glad that I didn't make it on time or I would never have read the positive words of wisdom that a couple of you took the time send out and bring me some relief with good info and common sense.  Words can't express how much I appreciate this place and the people in it.  ten minutes into this new day and I'm already feeling like it will be the best day ever just because of a couple paragraphs.

Cardawg, did you not recover any lost size through traction and VED before implant ?  Also, besides the forward placed scrotum, does the implant made you lose any additional length right after post op ? As others mentioned there is no doubt you will regain length with use over time but just curious about your case.

To be 100%.  I can't say that I put in the time that would likely be required to see any significant improvements.  I would see what I thought or hoped was some improvement but that perception would swing the other direction just as fast.  As a single father with full custody of a 10 yr old boy, it wasn't easy, ideal or the behaviors of a what thought a good father should be doing while home in the evening.  I could be a dad and do the things required of a parent or i could lock myself in my room away from him so I could straighten out my penis.  i chose to make dinner, crack jokes, and all the other things I didn't want to cheat my son out of experiencing.  By the time he would go to bed I'd be tired.  Sometimes i would do some traction or ved but more often not.  
I know that whatever results or improvements i might have seen for a moment or two were not occuring as fast as the negative changes.  I never had pain in my bend.  But my crap was shrinking and more and more often it wasn't working well when I would get a chick in the bedroom.  If she behaved really dirty it would work better than if she didnt but the bottom line is that I stopped trying to even be my charming usual self because I knew i wouldnt be able to back it up. that really sucked and I just felt shame finding myself having to secretly fix my johnson so i try again.  If i couldnt do it right with my normal hard big dick then I just figured I wouldnt even do it at all.  Now my kid has his license and is ready to go out and turn into a young man and I'm ready to catch up on lost sex time.

Dont get me wrong, I think that with enough time they can likely bring results.  My implant results are too knew and fresh to even evaluate.  I estimate a four inch loss based on holding on to my erect penis in my right hand while a girl i used to have fun with measured what was remaining above my fist and it was around 4 inches. as of right now if i hold my inflated new implant there is zero inches visible. Very tough reality to face but I've already whined about it last night and i'm done with whining.  I have an appt in four weeks and i will address my concerns at that time and see where I'm at.  I'm hoping that my scrotum relaxes once the incision heals and lets my balls drop back like they used to and maybe the pump will fall lower too. that alone would add an inch.
I grateful foir this place.  If I was half as smart as I seem to think I am, I would have used this place alot more and educated myself more from this resource instead of trying to hope it away. hope is not a good plan.

Just as a side note I would like to point out that trauma to your penis.....yes surgery is trauma, will cause your pelvic floor muscles to contract.  This an involuntary response to trauma by your body.  Once your body heals, guess what?  Those muscles relax.  What does that mean?  Your little guy will slide back into position.  Give it time my man.  Things will get better as things heal and relax

Thank you for the awesome insight.  I've never heard that but there is plenty of stuff I never knew before.  It makes perfect sense and adds to the list of information that helps me become more and more optemistic about the future.

When I reviews my doctors notes on line I was able to see the exact size, model number, etc.

Hi Stepone,

I'm not sure what all of it means but this is cut and pasted from my chart.

Following dilation, total measurement was right proximal 9cm, right total 21cm, left proximal 8cm, left total 20cm. An AMS 700 CX MS implant, total length 21 cm (18 cm cylinder with 3 cm RTE) was prepared on the back table with the scrub tech and AMS representative. Meanwhile, the reservoir (100 cc Spherical) was placed in the right prevesicle space through the same penoscrotal incision.

So i see 18 cm mentioned and 21 cm as well. I know that neither of those lengths represent penis length.  I'd say the 8 and 9 cm measurements are what you are asking.  Maybe someone who knows about this can translate it for me as well because it's all greek to me

In 2011 when you first saw hourglassing did you do anything about it? What was the progress until you saw the bend in 2015? Was there pain? Was there progression or all of a sudden it bent I. 2015?

You have an 18 cm implant with 3 1cm RTE's for a total implant size of 21 inches.  There is no way you will not have a minimum of 6" when inflated.

well i hope you're right.  I also had incision graphting done on the left side to fix my 50 degree peyronies bend.  

When i first noticed hour glassing I'd never heard of peyronies so i just looked at it like i had a half a hard on.  the slight hour glassing would disappear once i would become completely erect which was usually in seconds.  i would just force more blood into the penis by pressing behind my scrotum and then i wouldnt see or notice anything wrong because engaged in sex and it was working fine.  as time passed it became more noticeable more often but still not enough to stop me from sex.  Over the next couple years was the first time that had instances where I had ed bad enough to stop sex in its trax.  I chalked it up as nerves or getting old and even blamed it on the girls not being exciting enough.  
Then while having sex one morning I went in at a bad angle where my penis bent and it hurt. thinking i just hit a bad angle we continued as normal. a few days later i woke up wiyh the normal wood as i went to go urinate. i noticed my urine was shooting to the left of the toilet. i looked down and saw that my penis was bent alot.  It freaked me out and went online to see what i could find out and thats when i learned about peyronies.  over the next week or so i read that there was no known cure but i didnt believe i couldnt get it straight again.  I mean crap, we put a man on the moon so im sure we can get my dick straight.  thats when the official battle began and as you all know.  it sucks

I see. Well that was not what I wanted to hear. I'm like you in 2011. I see prominent hourglassing when semi erect and when I'm fully erect I have some indentations. I'm still able to have sex and no noticeable erectile dysfunction yet. But I'm super depressed because I don't know what the future holds.

I'm no expert but I've always felt that I could have taken preventative measures when I first began noticing hourglassing had I known what I was up against.  Is hourglassing your only indication of peyronies?  have you been diagnosed?  Is there any pain or nodules?

I could be wrong but i always felt like if i just improved blood flow that i could have prevented it from worsening.  But i really don't know.

The good news is that you found this forum during what sounds like a mild stage of the disease.  I recommend reading and asking questions and being all around vigilant so you can possibly stop it from progressing.  
Don't do what i did and go into denial and and just hope it would reverse itself and life would go on.
get educated and ask these guys the hard questions ti improve your chances of getting what you want.  these guys are super helpful and very cool from what ive seen.

the bulb is hard as a rock after the first time i deflated.  then i struggle to get a half squeeze at a time and then i only get around four of those before its hard as cement and wont squeeze at all.  its been 6 weeks and i have an appintment this thursday sep 2nd.  it wont deflate all the way and definately isnt inflating all the way either.  i still have sharp stitches poking out all around the circumfrence that havent dissolved like i was told they would. they are where my skin would normaly fold so they keep stabbing me in my glans.  

im concerned about why the stupid thing is so hard to pump.  i have a strong grip so something has to wrong.  im reading that pumping wont add to length but only to the girth.
Hawk you said based on the details of the equipment id have at least 6 inches.  im not fat at all and i can tell you for sure that it aint 6 inches.  i hoped it would get longer with regular use but the thing itself is useless. it gives me 3-4 half pumps.  ive lost all my girth and half my length.  im really feeling like grabbing the syrgeon by his ears and pounding his head  on every wall of the office. and every desk and computer.  i wouldnt show what ive got to any female.  unless im doing something really wrong i have been left with a tiny tiny cock thats not even long enough to take any kind of stroke of any noticrable length. my anger and anxiety levels are off the charts cuz im faced with the reality that ive had sex for the last time ever. cuz this thing aint happening. sorry for ranting but F~@< these doctors

I am sorry for your difficulties.  Have you started a journal so we can follow the progression of events starting at least with the day of the implant if not before?  Who is your surgeon?  I have 20 questions I need answers to - to give me some background.

From your description of stitches, it seems you had a sub-coronal approach which is probably the least common approach.  It is usually done if a patient is having other surgery such as excision or grafting away from the base of the shaft.

It does sound like something is wrong.  Have you discussed this with your surgeon by phone or in writing?

while you describe the problem inflating you do not say exactly why you can't deflate.

I will try to dig back through some of your posts ti get some background.

OK, I read all of your implant posts, and I recall those I had already seen.

I know it is not easy but be patient.  First, I will merge this topic with your other topic and rename it Cardawg's Implant Journal.  It does no good to have posts about your plant scattered all over.  It denies members easy access to your background so they can keep up with your case.

Please answer:

How old are you?
Where are your stitches?
Do you have stitches anywhere on your shaft?
Can you take hot baths?
Have you discussed any of these inflation-deflation issues with your surgeon?  If so, how (phone, etc.) - what are the details of the exchange?
Does your scrotum still seem "repositioned forward"? - Have you discussed that with the surgeon - what was his response

I was hoping the scrotum was swelling. However, there is some chance that it is associated with the (3) 1 CM RTE's.  RTE's mount at the end of the implant cylinders deep in the base of the penis (crus).  That moves the base of the cylinders forward and moves the tubing connection further forward.  I have heard men complain they could feel the tubing at the base of the shaft with RTE's but never that it "repositioned their scrotum".

Though we are not going to dwell on it at this point, we always urge men to post and get input before choosing a surgeon and going into surgery.  However, I think there is a good chance you will come out of this fine, so relax and let's move forward.

Cardawg sorry to hear about the problems you're having with your CX implant. I have an issue with my CX implant as well that I've talked about in my previous posts.

As for inflating and deflating your CX this is what I've done with mine to get it to work. For inflating I'll put my thumb and index finger on the pump and give it one quick press and release, wait a couple of seconds and then begin to press the pump until it's hard. If you're doing it right after the initial quick pump you should feel what feels like a pop in the pump and the other additional squeezes should start off easy and gradually get harder as the cylinders inflate.
To deflate I'll use my thumb and index finger again positioning my thumb on the back of the bar at the top where the deflate button is and my index finger on the button. I then press the button and keep it pressed for at least 5 seconds. I also feel a slight sensation on my finger when it starts to deflate. If it's quiet enough my wife and I will also hear a slight sound as it deflates. I know this sounds a little weird but that's how mine works.

Hopefully this helps and everything works out for you as you continue to heal.

My pump felt like a rock for awhile after my procedure.
Heat did soften it, but it seems it's been long enough for you to be able to inflate it.
I hope your doc listens to you and gives you some answers.

Where are your stitches? under the glans.  I did also have graphting done to straighten my peyronies bend.

So I am assuming your implant was done through an incision in your scrotum, but you were degloved for excision grafting surgery at the same time??  Is that correct?

At this point, you are less than 4 days from seeing your surgeon.  We are just forming an understanding of your complete procedure.  Here are a few questions you might consider asking in addition to any others that you have prepared.  Do not hesitate to have them written down and read them.  Make sure you understand the answers.  If you have a spouse or other support person who knows about your implant, take them along to write down answers.

1. Why didn't I get a larger implant rather than 3 RTE's on each cylinder (My guess he will say because the AMS only comes in 4cm increments, and you needed 3cm.  
2. Are the RTE's the reason my scrotum is pushed forward, or is it the pump placement independent of the RTE's that caused that?  (if he says the RTE's  I would ask #3
3. Do you ever use Coloplast implants that come in 2 CM sizes to eliminate most of the RTE's?
4. Why is my pump hard after 4 pumps
5. Why can't I fully deflate?  (I suspect he will resolve those two questions by teaching you a technique or by doing a pump reset with repeated pump/release actions)
6. Can you soak in a hot bath while inflating?
7. Any questions you have for him about the stitches.
8. When can I have intercourse?

When we get the answers to these questions, you will know a ton more than you do now.  I am sorry you did not have pages of clear post-op instructions and ready access to chat with your surgeon by phone whenever you had questions.

If you have any pain pills, take some right before you walk in and register with the receptionist.  He will likely be manipulating your pump and release valve, and he will NOT be as careful as you would be.

Thank you for the advice and tips.  I really hope there are solutions that wont require being cut open any more.  I just hope i don't offend him with my dissatisfaction to a point where he doesnt care to help resolve the issues.  I dont want to offend but i wont take it well if he takes an attitude of "it is what it is now F~@< off".  That wont be good

My approach at this point would be to show no anger or prejudgement until he at least has a chance to fully respond to your questions.  For one thing, you want a chance to discuss these issues without shutting down or inhibiting the conversation.  If you want to initially express some disappointment, that is fine, but this is not a time for anger.  It is a time to encourage him to talk as much as possible. Who knows, maybe he will respond that:

1. the scrotum position is a result of the pump placement, not the RTE's, and that it will drop down and back within the next couple of months.
2. He might get your pump working fine, and you will realize you were never even  fully inflated and that you will gain size in the months ahead.

At that point, you come home a happy patient full of hope and optimism.

I would work in one other question.  - How many implants do you do a year?  That will give you a lot of information and is one of the several questions we encourage men to ask when choosing a surgeon.

Cardawg, I have a few additional questions for you:

1. When the pump is "Rock Hard," how hard is your penis compared to the best natural erection you ever had in your life (hardness and angle)
2. When your pump is Rock Hard," and you probe the glans to feel the tip of the cylinders, how far into the tip are they? (1/2 way, 1/3 of the way, the tips do not make it to the glans, etc.)
3. When your pump is Rock Hard," if you stay that way does your penis start to hurt, or burn after 20,40, 60 minutes?
4. When your pump is Rock Hard,"  if you deeply squeeze and probe your shaft to feel the cylinders are they uniform, or can you feel folds, bends, or twists in them?
5. When your pump is Rock Hard" Do both cylinders feel the same?

Answers to these questions will reveal a lot and maybe generate more things to discuss on Thursday with your surgeon.

Cardawg, you'll never get an 8" dick, but like Hawk said, you will get 6" or more.
My implant and RTEs are similar to yours and I got that.
Dammit, I even picked up a little more, but didn't have the conditions you had.
Also, the pump will soften in time and things will look better soon.

Answers to your questions.

The shaft feels very firm probably as firm as it ever was. My glans is not hard at all.  This is another concern and i'm hoping its due to the fact that I've not allowed myself to get horny since i can't have sex. Hopefully this changes when I get aroused. The angle is slightly less than level. i worry is will also get worse since its pointed down all day in my underware.

After messing around with it these last couple days I dont think i've had more than six or seven pumps. Even though its firm the surrounding skin isn't firm and I've not gotten sore or any burning sensation.  The erection doesnt look like an erection. to me it looks like it does when flacid except it lifts away from the body.  the pump does not seem to increase the penis in length or girth.

4. When your pump is Rock Hard,"  if you deeply squeeze and probe your shaft to feel the cylinders are they uniform, or can you feel folds, bends, or twists in them?
I dont feel any folds.  The right side appears to be a little longer than the left side that had the graphting procedure done to it

5. When your pump is Rock Hard" Do both cylinders feel the same?
Yes. they do feel the same.

Well, you are only one day away from getting some answers.  

Based only on your verbal description, It would seem that you have a properly installed implant well into the glans.  The cylinders are not kinked or twisted.  You cannot expect to have your glands respond to implant inflation.  The implant has nothing to do with your glans.  Your glans will do whatever it did before the implant.

It is not uncommon for RTE's to result in a lower angle of an inflated penis because they do not inflate, and they are a smaller diameter than the cylinders, so they are not as tightly planted into the crus scar capsule.  This does not affect size or functionality and is mostly cosmetic.  I am not sure why you point your penis down all day rather than up at an angle.  Most surgeons would say it does not matter, but I think it does, especially when you are not totally deflated.  I think it is just like pushing a post sideways.  It enlarges the post hole in that direction.  With an implant, the scar capsule in the crus represents the post hole.

The confusing part is:
When your pump is rock-hard, your penis is not harder than any natural erection.
That your girth is not as big as before the implant, especially if you did not have an unusually large girth (5 1/2 " or greater) before the implant.
That your Bone Pressed Erect Length (BPEL) is not at least 6" with a 21 cm configuration and no folds in the cylinders.

I think your inability to deflate is just technique (finding the release valve, pressing hard enough, and squeezing the penis simultaneously).  Implant pumps are very stiff initially, and it takes a while to break them in and get the technique down.

Good luck on getting clear answers to your questions tomorrow.  Those of us who are following and trying to support you are very interested in your follow-up report.

PS: I am dying to know how many implants this surgeon does.  The fact that his practice suggests he also has female patients and that he had a rep from the implant company in the operating room would suggest he might not be a high-volume surgeon.

When my pump gets so hard it no longer pumps, my dick shaft is very solid and there is never any pain or discomfort.

My glans never gets solid, but it has a nice size and isn't a problem.

My tips into the glans go nearly to the tips and my right cylinder appears to be slightly longer than the left.
Again, that isn't a problem.

My inflated hang is now about 3:30.
It needs a bit of guidance by me or my wife.

I still have a slight kink in the right cylinder when flaccid.
Sometimes, I put in several pumps to make it more comfortable.

One thing I've noticed is I get some urine leakage at night, usually when partially or completely inflated.
I also seem to peejaculate (my new word) when I should ejaculate.
Maybe urinating prior to sex would prevent that?
I guess beer drinking exacerbates that problem?

Hi All,

Had my six week follow up and it went just like Hawk had said that it might go. The pump operation issues was just technique and practice.  I've been too gentle with it cuz it has slipped from between my fingers and hit one of my testicles and that sucked and made me overly cautious.  Its still a little sore in the scrotum after six weeks but much less.  I have another follow up in november.
He also said the same thing as Hawk and a few other people regarding the tubing pulling the scrotum forward.  Its still a bit swollen under the incision point.  He says it takes about a year for a scar to heal and take its permanent shape.  In other words he says he expects them to drop as the swelling subsides and the rubber tubes get softer and worn in.  But that's where I'm at and Ill add stuff that is of any value into this journal as they come up.  Thank you all for making this place for us to vent and find answers to our questions about this stuff.  God Bless

Good to hear that you got some answers.  

Please keep us posted.  I think there are still a lot of details you need to understand to get the best results.

We are interested in your new take on your inflated and deflated state since you got instructions on using the pump.  Also, if you can soak in a hot tub, it makes the scrotum relax, reduces swelling, and makes pump manipulation much easier until you master it.  It also reduces pain in your inflated penis at this stage.

It is critical to TOTALLY deflate, so you stretch the scar capsule around the reservoir before it fully heals.  You are already near the end of that window.  If you do not do it now, you could end up never being able to fully deflate and stay deflated.  That has some medical consequences as well as obvious nuisance consequences.
