Torsion, curve at the base, mild ED

Hi everybody,

I'm a healthy and fit 25 year old, based in the UK, and have been with my girlfriend for 2 years. I had a mild sex injury in Feb'21. During doggystyle, my penis slipped out and was forced to bend down at the base. There were no major symptoms other than some pain, mostly on the right and closer to the base. I did not lose my erection, hear any noises, swelling - none of that. Simply had pain, and gave it a rest for a few days.

Since the injury I've developed some torsion to the right, a curve at the base to the right, and mild ED - mostly venous leak symptoms, such as difficulty maintaining an erection, constantly needing physical stimulation, weaker morning wood that disappears quickly, etc.

I had an appointment with Dr Amr Raheem at International Andrology London, UK. He could not feel any plaques and diagnosed mild ED from trauma to the penis. He prescribed 2.5mg Cialis, VED, Shockwave therapy. He says there is 70% chance I will stay the same or even get better, 30% chance of getting worse (more curve, more unstable erections). He said that when there is trauma to the penis, the penis will take some time to recover, just as would happen with a sports injury. However, it has now been 5 months since the injury and my symptoms have gotten worse over time. Cialis works to get stronger erections, but they still fade away quickly. I am starting VED this week with a Soma Erect Response II. I am also taking Coq10 200mg per day.

I have read most of the key forum documents, including the survival guide. I am now looking for some opinions from you fine folks. You guys have such a knowledgeable forum! I would really appreciate your views, and hopefully I can become a part of the community. I am a PhD student at Cambridge University so I have good access to a lot within the academic world - hopefully I can bring some value to the forum too.

1. I feel that the right corpus Cavernosum is not engorging as quickly or fully as the left one, and is not keeping blood in properly. That seems to be what is leading the erection to be unstable and deflate quickly, and also why it is curving so much. That is also the area in which I had pain after the injury (and still sometimes have mild pain). What do you guys make of this?

2. I am now doing Shockwave Therapy at International Andrology London. I have had one session so far, no improvement. It was quite painful during treatment - like getting electric shocks on my penis. The nurse who did the treatment said that the machine is a Storz Duolith SD1, 3000 pulses per session for 6 sessions, and the intensity is 0.5. Does anyone know what this 0.5 means? And is it normal for treatment to be painful like that?

3. I am trying to access Pentox in the UK and also a second opinion from a urologist/andrologist who focuses on ED. Anyone UK-based have any tips?

4. I'll attach two pictures (erect and semi-erect) so you guys can offer some opinions on what is going on. Bear in mind, before the injury my penis did not use to curve at all whist semi-erect and had only a very slight curve and slight torsion when erect. https://imgur.com/a/dFXwm8P

5. Do you reckon I would benefit from anything else? Perhaps traction?

Regarding your case, you're on a good treatment plan! I'd suggest upping the dosage of Cialis to 5mg, especially since the 2.5mg isn't working too well for you. You should consider daily traction to prevent worsening and hopefully reverse some of the curve that developed. Traction without a doubt is the best nonsurgical treatment we have. Also add in the other supplements as described in the Survival Guide!

If you just showed the pictures, I'd guess it was a congenital curvature. But since you say it's all acquired, there's a chance that it is Peyronie's Disease.

Have you done a flaccid and erect ultrasound? That would be a good thing to look at both for Peyronies Disease plaque detection and to see your bloodflow numbers.
26 years old. 20 degree upward curve with slight clockwise twist. Symptoms onset Dec 20.
Dr. Levine says it's not Peyronie's Disease but a slow healing wound. Saw him Mar 21 and May 21.
Traction (PMP) and supplements per Dr. Levine's recommendation.

SkySun, Sorry to hear about your troubles. For the second opinion in UK - you can try peyronie specialist Professor David Ralph in London or you can also try two american peyronie specialists who know peyronie issue - Dr. Levine in Chicago or Dr. Brian Christine in Alabama, and it's not a promotion, but a real experience according to forum members from this main forum. Don't waste your time on a general urologists. Also you need to accept a new reality and not create a miracle expectations as for example me was living in a miracle during this year sitting in this forum, until yesterday-today I got a personal messages from friends in this forum to accept the new reality and to be realistic. So, now - I'm realist and I don't hope on a magic anymore. Soon, I will travel to american peyronie specialists too in order to get the realistic help and hope, that it will tremendously help me. SkySun, welcome to the most unfortunate "club" in the world! But seriously, welcome. Your academic approach to this is both commendable and will, I think, prove very valuable in the journey ahead, which for you, I hope is very short and reaches a happy resolution.

Regarding your erections, which you say fade away rather quickly without stimulation. ... When you get erect now, do you also have mild anxiety? Are you a bit desperate to see what condition your penis is in, worried "is it worse?," etc. Are you a bit clammy, maybe a little forehead sweat, and slightly racing heart?

Reason I ask, is I know when I'm having a Peyronie's flare (I've had two in my life and have just begun a third), I am indeed a bit panicky when getting erect. And this anxiety is an erection killer, for sure!  

You may not be suffering such anxiety - I'm just asking in order to rule out a psychological component to this.

51 yrs. old, multiple auto-immune conditions. First episode of Peyronies Disease in 2002. Recurred a couple times since. Over the years I have tried Topical Verapamil, Iontophoresis, all the supps and Cialis + Pentoxifylline. Still functional, always worried.

Welcome SkySun to the forum! Here you can find some answers to your situation.  I would say that traction could be helpful in your case. Remeber though that this is a marathon and not a sprint. Gentle traction is always best starting out until you find your sweet spot.  Your penis looks like someone who has a congenital curve and not peyronies/injury at all.  I understand you are use to it looking and appearing more straight.  Taking the supplements and Pentox will not harm you and might be of some benefit. Don't let your mind take you down the bad road to worrying about it - it will consume you if you do.  One thing on your side is that you are young and can heal quicker than most of us "older" gents here.  Use the search function on the forum as you can see other members info regarding their specific case.  Good luck on your studies!  Mikel7  :)

Benraycamp0 - I haven't done an ultrasound. I asked Dr Raheem about it, but he said it wasn't necessary. He said he couldn't feel any plaques therefore it is unlikely that the ultrasound would find any.

Wilson - Dr Ralph sure sounds like a good option. Do you know whether he is keen to prescribe Pentox? I'm struggling to access Pentox in the UK.

Nemo - I'm generally a chilled and positive guy. I certainly have a bit of anxiety nowadays and end up self-examining more frequently than I probably ought to. Dr Raheem also mentioned this: "don't self-examine, it will make you release adrenalin, which is an erection killer". That said, one of the key symptoms is that without Cialis my morning erections are now semi-erect at best. This has happened for the last few months. Could anxiety be affecting erections in my sleep?

Mikel7 - As unusual as this might sound, the curve seems to be happening because the right corpus Cavernosum is not engorging and trapping in blood as well as the left one. It's not a major difference, but still a significant one. Therefore my penis is weaker on the right side and ends up leaning into it. Not really Peyronies (perhaps the beginning of it?), but more likely blood flow issues because of the trauma. Dr Raheem also wrote on my report "Possible early onset Peyronies Disease". What do you think of my current supplements? Cialis 2.5mg at night, Coq10 100mg morning + 100mg night, going to start on Acetyl L Carnitine 500mg morning + 500mg night.

  You could up your CoQ10. I do 200mg in the a.m. and 200mg in p.m.  I take Acetyl L Carnitine at 1000mg in morning and 1000mg in afternoon. I also use natural vit E and boswellia. In addition I also use heat therapy every day before and after traction with PMP.  When I could take Cialis it really helped me out a lot with erections and pain etc...  It was difficult to say if the pentox was helping or not.  You are one step ahead of this disease since you say you are a positive kind of guy.  :)
