I think this is causing me most trouble with functioning. Hurts the most too. I'd like it gone!

I read through your earlier posts and it sounds like your plaque has calcified. From my understanding, Xiaflex doesn't work well on calcified plaques.
26 years old. 20 degree upward curve with slight clockwise twist. Symptoms onset Dec 20.
Dr. Levine says it's not Peyronie's Disease but a slow healing wound. Saw him Mar 21 and May 21.
Traction (PMP) and supplements per Dr. Levine's recommendation.

My band under glans isn't calcified. I have 2 minor calcifications from a pop accident 6 years ago. This only have me a mild curve, so they aren't the issue.  My newer injuries are the problem and they aren't calcified yet.

Has anyone had success using xiaflex for hourglass?

I have found that Xiaflex typically doesn't do much for indentations, hourglass, or circumferential bands (like you're mentioning).  It is good at improving curvature, but that's about the limit of what it can do.
