Early Symptoms, any help much appreciated

Hi everyone, I had what I thought was a minor sexual mishap in early May, wife and I were in a spontaneous position and I think my penis buckled a bit, didn't hurt at all at the time. This also happened around a time when for other reasons my erections weren't as strong as usual. Anyway, I'm seeing some hourglassing towards the tip now when semi-erect - it seems to only happen when I'm sitting or standing. In the morning in bed and/or during intimacy with the wife, my sexual response is much quicker and I don't notice it at all.

I saw a urologist who did a physical exam and said everything felt normal, but the hourglassing has gotten a bit worse since then. I do get that "hard flaccid" sensation quite a bit, not sure why, where the whole shaft feels rubbery, and getting no nocturnal or morning erections at all. I have occasional penis pain, but this seems to coincide with periods of intense emotional stress, so I don't know what to make of it (in high school my girlfriend told me to do routine testicular exams, and my balls burned like hell for months on end after that, so stress induced genital pain is a real thing for me). Anyway Could this rubbery hard  flaccid state cause inelasticity in the shaft (especially since erections are rare now) which explains the hourglassing towards the glans, as that's the last place blood flows to during the erection process? Or is scarring/plaque more likely? So far erections themselves are straight and painless.

In either event, nocturnal erections are important for wound healing, are they not? And so should I try and get my hands on some daily cialis just in case? Seems like it couldn't hurt, and might help slow the plaquing process if that's indeed what's causing my issue. As a precaution in case this is early fibrosis/peyronie's, are there any other steps I should be taking to enhance the healing process and slow the plaquing?

Interesting case! How old are you?

All your current symptoms point to the elusive Hard Flaccid Syndrome that we don't really know much about, medically speaking. There are subreddits with some good information about that https://www.reddit.com/r/Hard_Flaccid/ and https://www.reddit.com/r/Hard_Flaccid_Syndrome/. It seems some men find resolution through pelvic floor exercises, daily Cialis, and rest.

Jumping on Cialis would be a good idea to start with and also consider taking the supplements as listed in the Survival Guide above. Those are good for overall health and bloodflow, and also have some scientific evidence for being anti-fibrotic.

Dr. Levine says it's not Peyronie's Disease but a slow healing wound. Saw him Mar 21 and May 21.
Traction (PMP) and supplements per Dr. Levine's recommendation.

Nice to meet you, and seriously, thank you for all you do here.

I'm 34. The only thing that makes me suspicious of the idea that it's "hard flaccid" is that I don't have that much other pelvic pain. Some bowel issues that are consistent with what some CPPS/hard flaccid people describe, but I don't actively feel much pain or tension in the pelvic floor (which I also understand lots of hypertonic pelvic floor sufferers don't actively feel pain/tightness all the time, so who knows?)

I read the survival guide, it was very helpful! Only thing is I'm on an ACE inhibitor for blood pressure, and so I hear I'm not supposed to take L-Arginine with that as it could cause BP to lower too much. Have you heard of any success with castor oil?

Well, and I don't claim to be an expert on Hard Flaccid Syndrome (I don't think anyone can haha), but your flaccid state is hard which isn't a typical symptom of Peyronie's Disease.

Try laying down on your back for a couple minutes and feel your flaccid penis. It should feel soft and flexible. Now stand up for a couple minutes. Does your penis feel harder now?

For castor oil, feel free to search about it on the forum. I think some people do claim it can help, but it's not thought of as a Peyronie's Disease treatment by doctor.

My only other question for now is - sometimes the hard flaccid is very hard, and right under the glans where my suspected plaque is located since that's what hourglasses is especially hard - but this is less palpable during non-hard flaccid flareups. Would this be an indicator that it isn't plaque? I would think plaque would be consistent and/or worsening regardless of the hard flaccid flares. Then again, during the flareups of HF, the area in question feels especially firm. Not sure what to make of this, but I would assume peyronie's plaque doesn't change its palpability regardless of the flaccid state?

I mean one can definitely have scar tissue in their penis and HF. I'm not saying that's what you have, especially since your only deformity is in the semi-erect state and not in a full erection (this is more typical in HF than Peyronies Disease).

I'll let others who have more knowledge on the topic to chime in, but I think at this point the best treatments you can do are pelvic floor exercises and take daily Cialis. You can begin traction therapy (Penimaster Pro or RestoreX) if you want to be more proactive on the Peyronie's Disease front. Apart from those treatments, it's wait and hopefully that it resolves over time!
