New member, long time peyronies with new symptoms

Hello all,

I'm 28 and I've had peyronies since late 2017. I think the first incidence was caused by a bend that happened during sex in December of 2016. I didn't notice the curving up until fall of 2017. There was slight curvature to the left, and luckily it stabilized rather quickly. It stopped at about 10° and the module on the left side was small. Everything was ok until fall 2019 when I started noticing pain again. I abstained from doing anything, and briefly tried the penis extender from x4 labs (a few days, made pain worse) and the pain  seemed to resolve, with no formation of any new plaques. Fast forward to April 4th of this year, and I had came once during sex, and we were going for round two. The position was painful (her on top) and because of this pain I became flaccid. There was definitely buckling happening. After that I noticed some pain and a nodule began to develop on the left underside of my penis, at the base almost back behind the pelvis (right where the festival's connect. Since then sex has been painful, the nodule seems to be growing. Two weekends ago, doggystyle severely agravated it.
I got in to see a urologist (PA) and she was not the most helpful. Her answer was basically wait and see or get xiaflex. I am starting to experience curvature from the new plaque at the base, and weirdly I am unable to push my penis in that direction when it is erect (so anything other than missionary, spooning, or yab yum would be very uncomfortable). I wanted to do everything I can to halt the progression, and I've been reading the forum posts for a little while and looking at the various options, and reading reviews of peyronies treatments and outcomes. I tried to get the urologist to prescribe cialis, but she would only do 20mg sildenafil. I'm still trying for the cialis, and I think my insurance has approved it. I tried one out yesterday to see if I had any negative side effects (I plan on taking it during the night, but took early to test) and it gave me an amazing erection, but the curvature is starting to become more noticeable. My erections since April had been sub par, I think partially because of my fear of pain during intercourse. That is one reason I want the PDE4i, so I don't reinjure my penis if it starts to become flaccid.
I am going to start taking l-arginine but I am unsure of dose. I started heating with NeoVs rice sock and am doing his stretches. I am thinking about getting the restoreX traction device, as the studies done on it appear fantastic.
Is there anything else I can do the help stop the progression? I understand that reversing the curve is a lot harder to do, but I'd be happy if I could just stop it in its tracks (about 20° in mid bend).

Hello Hawk,

Thank you for the welcome. I found it after I had made the post, I apologize for being quick on the draw. I took initial measurements, including the bone-pressed stretched flaccid length, two girth measurements, and took a picture to measure angle and show to the next urologist. I am starting two of the oral trifecta, a PDEi (sildenafil 20 mg each night, trying to get Cialis 5mg, insurance said they'd cover it, whether the urologist will prescribe is another story), l-arginine (I will shoot for the 1500 mg daily found in the Survival guide), and then I will try to obtain Pentox. I started taking CoQ10 2x200mg daily today. I also made a signature!

I do not drink or smoke, and usually get adequate sleep. I am trying to make my diet as non-inflammatory as possible, and whole foods (plant-based isn't an option as I have too many allergies).  

I think one of my only questions that still remain are about NSAIDs, I've seen some places that ibuprofen helps pulmonary fibrosis, but NSAIDs do inhibit healing. Does the reduction in inflammation outweigh the healing inhibition when it comes to fibrosis?

For starters, I am a little embarrassed and disappointed in myself.  As an information hound, I work to stay generally informed.  But, somehow, this is the first that I have ever heard that NSAID's inhibit healing.  It seems to have a solid basis, in fact.

Thanks to you, I have read 3 articles and a couple of studies.  Here is one https://pubmed.ncbi.nlm.nih.gov/22744434/

It seems to suggest at the date of this article that the jury was out on NSAIDs and soft tissue, but the detriment to bone healing of at least one NSAID is clear.

here is another: "A December 2020 study in the American journal of therapeutics (4) suggests: "There is convincing evidence that NSAIDs administered locally in and around the joint reduce postoperative pain scores and opioid consumption in patients undergoing total joint arthroplasty, yet further research is required regarding the risks of potential chondrotoxicity and the inhibition of bone and soft-tissue healing with locally administered NSAIDs."

I think you will have to sort this out without any more help from me because I am not informed enough to share an opinion.  Maybe some other members are.

Minus further information I would first opt for an antiinflammatory diet, and use of heat and cold.

If your curve is 15 to 20 degree only and usable I wouldn't do xiaflex. Traction should be enough to get you under 10 degrees... if pain is an issue nightly cialis will help or get interferon 2b injections at the point of pain, supposed to help with pain.
26 year old
45-50 degree downward curve with an indentation on one side when erect.
Using RestoreX and Xiaflex injections
Taking coq10 with daily Cialis 5mg.

Thanks Gauss, I had seen the results of the restoreX combined with Xiaflex and was thinking about it. I am trying to get 5 mg Cialis, but right now have 20 mg  Sildenafil. I am going to use that as a nightly PDEi until I can get Cialis.

I would say that watching your diet can help, but I would stick to traction and heat therapy and Cialis/tadalafil first. I would not reccommend hand traction as there is no way to guage your level of pull. You will see if time in traction will help you or not. For pain I know that when I could take it that Celebrex was awesome and worked at both inflammation/pain. Don't despair! Work a plan.

Another thing that many find that helps pain is Acetyl L-Carnitine 1.5 - 3 grams per day divided into 2 or 3 doses.
