Grafting + Implant with mild fibrosys and erectile dysfunction

Hello everyone! Soon I think I will receive a prosthesis. I am scared and excited at the same time. I don't have severe Peyronie's curvature (in fact, my penis is just a little curved to the right only when it's flaccid), but I do have soft fibrosis and have lost about an inch in length due to severe erectile dysfunction and I'm terrified of lose other dimensions after obtaining the implant. My surgeon told me that he can do a graft procedure and then implant me, all in one surgery, so I don't lose any centimeters and try to regain the size I was three years ago. Has anyone ever had both surgeries at the same time? I see many people who do the graft, but they are all people who have a large visible calcified curve, I would like to know if this technique is also valid for those like me who have an aesthetically normal penis but the size has reduced due to fibrosis and of erectile dysfunction. The surgeon says it's possible and honestly I'm delighted. I would like to know if anyone else has similar experiences! Thank you all!

Hopefully, others will respond because my experience is limited.  You do not list Erectile Dysfunction in your signature line.  Can you have sex with PDE5 inhibitors?

Have you lost mostly length or girth?

Do you know what brand/model of implant he wants to use?
The Titan is great for straightening and for girth.  The LGX 700 (AMS/Boston Scientific) might be better for length if you are not significantly above average in size.  My understanding is that AMS/Boston scientific implants sold to other countries do NOT have the inhibizone antibacterial coating.  Find out.  Do your homework.

Usually, if you lost size due to Peyronies Disease without a deformity, it means that plaque is distributed in spots around all sides of the penis.  I am not sure how well grafting works in that situation.  The key is having a great topnotch surgeon, not just a good surgeon.  Then you can trust his judgment with your only dick.

Needless to say, the risk of infection and nerve damage significantly increases with both surgeries since excision and grafting is a much longer surgery than an implant.

I don't understand why he needs grafting when he can have TEP, Tunica expansion procedure like tartao here on this forum to relieve scar tissue and increase length and girth without need for grafting.

Probably because he does not have access to a great surgeon who is skilled by specializing in a high volume of those surgeries.

Yes you're right Hawk, I'll update my signature right away. I suffer from very severe erectile dysfunction after injuring myself with Jelqing. My erect length (when stimulated) has decreased by about 1 cm, but if I stretch with the flaccid penis I can regain my normal size. The circumference has decreased, but I'm not afraid of that, because I will probably be implanted with a Titan. I have an above average penis, around 20cm, but I am still very afraid of losing size. Erectile dysfunction has already caused me psychological damage, even losing length would depress me a lot.
I think I will be operated on by surgeons from the Sava Perovic foundation, who say they are very experienced, and are also very close from my country. The only thing is that when I asked them if I can get the lost length back they said yes, but they mentioned graft + implant. I was very happy with this, but now that you tell me that there are more advanced methods, it makes me doubt and fear. Maybe I should send them another email to ask if they do the tunic expansion technique. I have personally seen a video where the surgeon who will probably operate on me was making small cuts with the scalpel, and I think it is this technique, but I don't know why he mentioned the graft to me. I'll try to ask him, although I don't want to sound rude and question his methods. In Europe I think they are among the best known, I am amazed that they use backward techniques and not right for my specific case

Who would be your surgeon? Dr Djinovic?

I have talked with some guys that had the procedure that you mention with Dr. Djinovic and they had good results. I also asked Dr. Djinovic if he would do this TEP technique without graft and he told me that he can do it as well. The have been doing that for a while but I dont know they offered first grafting + implant and not just TEP + implant. I have the same question.

Hi! I contacted dr. Djinovic, but unfortunately he is very reluctant to implant young boys, unless you have a very serious peyronie that does not allow you to have sex. I have severe erectile dysfunction and fibrosis, but despite this I can't get him as a doctor. But I am in contact with Nikola Stanojevic, who was a student of Perovic and worked for years alongside Djinovic, and always works in Belgrade, he is also one of the founders of the Sava Perovic association, I found him very helpful and it seems to me very up to my expectations! I think I will write to him again to ask him if he does the TEP technique, which I think is much less invasive, and I would rather have it than have the graft. Are you planning to have the implant with Djinovic? In case we keep in touch, I would like to hear from someone like me who will have an operation or have an interest in implanting themselves in Serbia! :)

TEP is a relatively new procedure and very few surgeons have experience doing it. You need to find one that has lot of experience. Frankly speaking, not having to add grafting material and have the small slits relax scar contraction and fill in through self healing sounds a lot better and I bet takes lesser time too in practice. As hawk mentioned, grafting at the same time as putting in an implant takes a lot more time and infection rate only goes up with increased operating time.

I start very cautiously about "new procedures" until proven with a high success rate.  Anything that increases surgical time increases risk, whether it is Grafting, TEP, or any other procedure.  If they do that and add an implant without an antibacterial coating, you must assess the risk. That does not mean rejecting the risk; just accept that the risk is increasing and a number of patients will pay the price.  You have to ask if you are willing to take the chance that you will be one of them.

If you do, you have to take it in stride and move forward if you end up as one of the patients who suffer a bad outcome.

I agree with hawk completely here. If you said you are above average or even around average , you need to consider wether you really want a size increase....instead of getting a simple implant procedure done.
