Hi! Tortão here, 28yo from Brazil. My Peyronie started in August 2019. I don't know the reason, I haven't injured my penis. AFAIK, no one in my family has Peyronie or Dupuytren.
In 2019, a 45º curvature to the right and some torsion developed pretty quickly. The pain stopped after 1 entire year. The progression of Peyronie, actually, never stopped. It slowed down after the second or third month but didn't stop. Every 1 or 2 months a new surprise comes.
My curvature went to 80º to the right and then started to straighten after doing some traction. But traction didn't help at all. Actually, I think it created more plaques (this time, at the left side of my shaft). These plaques straightened my penis to something like 30º~45º to the right (this is my current curvature).

Of course, after the development of a lot of plaques, my penis lost size. Pre-Peyronie, my girth was 13.5cm and my length was 15 cm (Note: 15cm but not bone pressed. I never measured it bone pressed before Peyronie, but comparing the way I used to measure and bone pressed measurement, bone pressed adds 0.5 to 1 cm. So I think my penis had between 15.5 cm to 16 cm bone pressed. I'm going to use just bone-pressed measurements from now on). Today, my bone-pressed penis has 13.5 cm. The girth below my glans is 10.5 cm. At the base, my girth is 13.5 cm.

I forgot to tell him to put english subtitles in the videos. He has a lot of good information and from his videos I could see he is a very knowledgeable doctor.
25th march was my in-person appointment. He injected some substance into my penis in order to obtain an erection for a doppler exam. crap, THIS INJECTION HURTS SO MUCH!! I can't believe men inject this crap in their penis. He told me that usually, men don't feel this level of pain. He probably does this injection every day and knows how to do it. When his assistant injected a substance to let my penis down, it hurt a lot too. I think probably they didn't make a mistake, probably they hit a plaque (it's everywhere) or my penis is sensitive to this kind of crap. What I can say is: if I just had ED, and PDEi's drugs stopped working, I'd never inject anything in my penis. I'd rather choose never penetrating again if injections or nothing were my only choices.

Today, I don't have ED. Maybe a very mild ED, cause below my glans my penis doesn't have much rigidity. But when I have an erection, my penis can stay erect for a long time. I really think my situation will get worse if I do nothing to stop this disease, maybe I get more loss of rigidity, or ED start to develop... who knows.

Ok, so he analyzed my penis and the treatment is what we had agreed in February, in our first appointment: Tunica Expansion Procedure (TEP) for size recovery and a Titan 3 piece inflatable implant.
This way I can:
- Recover my size;
- Straight my penis;
- Gain axial rigidity;
- Never worry about Peyronie again.

Tomorrow (27/march/2021) is my surgery. I'm taking an anti-biotic named Zinnat (Cefuroxime axetil) and I will keep taking it for 14 days. Today and tomorrow morning before surgery I'm gonna take a bath using Chlorhexidine. It's an anti-septic.


Summary:
- 30~45º curvature to the right;
- Loss of axial rigidity (buckling);
- Loss of size (length and girth);
- Plaques on both sides;
- Peyronie keeps advancing;
- No ED.

Size pre-peyronie:
Length: 15.5 cm
Girth: 13.5 cm

Size today:
Length: 13.5 cm
Girth: it depends. At the base is 13.5 cm. Below the glans, it's 10.5 cm.

The estimation he gave me is: recovery of the girth (Titan really helps here) and 2 cm of length. He said he tries to put the biggest implant is possible. I like this idea. Dr. Eid does this too.

I'm gonna post pics, but not today.
Tomorrow (day of the surgery, 27/March) probably I'm not gonna respond here. Maybe I'll just stay on the bed for a few days without turning on my notebook.
I pretend to post pics from before Peyronie, during Peyronie, and after surgery. I understand that size can take months to be recovered, so I need patience. I also ask you for patience if you want to see my results.

I assume you have a longer recovery and some more discomfort from the TEP, but I think you will be happy with the final results.  Will he use the scrotal approach for the implant and reservoir?

You are in our thoughts and prayers.

PS: I injected BiMix at least twice per week for 8 months.  I would say the pain was non-existent.  It did not phase me.  If you are describing the needle, then something is definitely wrong.  If it were not for causing fibrosis, I would have been happy to inject for the rest of my life.

@Hawk ; How did you know that injections were causing you fibrosis?

Good question!
Because:
A. It is a listed warning and side-effect of the drug
B. It began weeks after I started injecting
C. The fibrosis started with dents the size of a teaspoon right at the base of my penis right at the site of injections

My surgery went well, I woke up at 14h (5 hours after receiving my anastesia). A bit groggy, but now I'm very ok. Feeling good. No pain at all, but I'm feeling nothing down there lol

Now I'm gonna eat a soup and drink a bit of coffee. I'm still on the hospital bed.

I haven't talked to my doctor and his team yet. Soon they will come to my bedroom to explain everything about the surgery and post-surgery care.

Continuing day 1...
The pain started around 22:00.

Day 2 -28/march
3:00 am I had to ask for Tramadol.

The implant is 90% inflated, more or less. I  will be kept this way for 10 days, then I have an appointment where the doctor will teach me how to cycle. Tomorrow my curative will be renewed and the drain that sucks blood from I don't know where will be removed.
My shaft is numb. It's normal since the procedure requires the manipulation of the neurovascular bundle. I'm feeling ok in the glans, but I haven't touched it that much to be sure.
My catheter was removed at 8:00 am.
I feel the rear of the implant being pressed up against a muscle if I move my penis down. This is painful. Cowgirl position with the woman's back facing you would be a no-no if this discomfort is part of the implant. (Like this https://i.imgur.com/Gydeq78.jpg).

Starting from today, I need to pull down my pump 10x/day, more or less. I pulled it once with the help of the assistant doctor. It's was a bit painful. I pulled one more time here, in my Airbnb, and I was more gentle this time.
Before the surgery, I thought I had no more room in my sack for the pump. My balls are kind of big. Maybe I was right. The pump is not totally behind the balls, it's just a bit behind and separating them. I'm not liking the way it is now, but I think it can get better, right?

When I stand up, my penis points to 2 o'clock, but I'm not using anything to fix it in this position. Should I? The doctor told me to rest my penis in the position I find most comfortable. When I lay down it moves more towards my belly.

The penoscrotal approach was used! EDIT 10/April actually, it was the subcoronal approach.
I really don't know why I reacted so badly to the injections. The level of pain was high.

I'd say the penis pointing at 2:00 is a good angle. It's up to you if you want to manipulate to make it where you want. Whatever is comfortable should be a good choice.

Take the pain meds!! Don't be a hero, it's better to get out in front of the pain before it becomes too much to bear.

My 2 cents worth of advice is to do NOTHING to bend your penis down because I am very convinced it significantly influences the angle of your erection when healed.  The uninflatable portion of the cylinder plus the RTE's are stabilized by a scar capsule that will form in the crus.  Like a fence in a fence post hole, any enlargement of that capsule will alloy the post ( your penis) to tilt more toward the floor when inflated.  It is still very usable but loses some aesthetics, especially if your natural erection pointed upward when standing.

All this pulling on the pump is also a confusing thing to me.  Many surgeons encourage it.  The ONLY time I have ever heard Dr. Eid forcefully express himself was when I said, "many say you should tug on the pump to pull it down."  He made it known in no uncertain terms NOT to do that.  Interestingly, during healing, my pump dropped lower than my testicle on its own.

Hey, guys. How are you doing? I hope everyone is ok.
Counting surgery day as day 1, today is day 4. It's Tuesday evening right now.

Yesterday my bandage was removed and another was placed by the doc in his clinic. The catheter that sucks blood from the ballsack got removed. Today my ballsack is more swollen, maybe because of the blood. I don't know. The pump is better placed behind my balls, but I can touch and feel the tubings between them. Does this get better? The doc recommended and I'm using a "scrotal suspender", but I think it's BS. Anyway, I'm following all the instructions. He knows best than me.

What hurts most is the stitches below my glans. Damn it's like my skin is being torn. Yesterday, after the new bandage was put in, the pain was worse. Today is better. I made another bandage today (on my own) and almost fainted in the bathroom. I did this while standing up (big mistake). My glans are kind of fine, but my shaft is purple and numb. It was expected.

Tomorrow I'm gonna clean the toilet seat using bleach, clean my hands with chlorhexidine, sit in the toilet seat and then make another bandage. I'm very afraid of possible infections. I watched some Dr. Eid videos and listened carefully to the infection symptoms he describes.
Oh, I haven't made number 2 since Friday. I started a laxative this morning, but I'm feeling no desire to go to the bathroom.


Something I want to tell you, guys... and I want to be very honest: be skeptical. It's good, it's healthy. Wait for my results before trying this procedure. It promises a lot, but does it deliver?
I made this procedure to restore my sexual function. I could just do an implant placement (I think). But the best implant doctor I could afford (which is Dr. Marco) recommended me this procedure. My tunica had to receive incisions anyway. My narrowing was bad.
But TODAY (or even before surgery day) if you ask me this: hey Tortão, if you could choose between making TEP with Marco or putting just an implant (without any promise of length gain) with Dr. Eid, who would you choose? I'd for sure choose Dr. Eid. Not that Marco isn't a good doctor. He seemingly is, but I couldn't find any review of any Brazilian implant doctor on the internet. Here, 90% of the implants are semi-rigid and most done by random surgeons. Dr. Eid, on the other hand, is very very well known. I basically took this leap of faith.

2.0 cm length gain was promised, but I see this as a plus. I think if I get my girth back and my pleasure returns to normal, then I'll be happy (my penis is already straight and can't bend to Peyronie anymore, so 2 objectives are completed). Of course, getting no length back would make us question the procedure and its size promises.

Thanks for your words. On day 2, the doctor just bent it everywhere show me (it was painful) but I'm not doing all this bending. I'm not maintaining it glued to my belly button too, as some guys do. I think right now it's in a good angle when I stand up (2:00 or 2:30). Marco Tulio said I'm 70% inflated, but his assistant said I was around 90%. Confusing, but I think Marco was the one who pumped me so probably his estimation is correct.  

This thing of pulling down the pump is really confusing. I've seen some videos of Dr. Eid saying it doesn't help at all. Well, I'm gonna stick to my doctor's recommendation anyway. It is not hurting my balls anymore.

Oh something I forgot... DAMN, I got a bit circumcised. I really used to like my foreskin. While having an erection it would always pull naturally back. Flaccid, it used to protect my glans.

After fully healed, I need to encounter ways to grow it back.

Today is day 5. Wednesday evening.

So far, so good. Size seems ok at this moment. I am probably 70%-90% inflated but the length seems good. Seems reasonable. It will grow more after starting cycling. Pain is much more manageable now, what hurts most is below the glans where part of the foreskin was cut, and where my hardest plaque was. My balls sack is still swollen.
Today I went to the bathroom and did number 2 thrice in the morning and two more times in the afternoon :D I couldn't poop since last Friday. I had to use a mild laxative this morning and I feel much better!

The pump is now located behind the balls, but when I sit or lay on my back the tubings get very palpable in front/between my balls. Is this common in the beginning?

Oh, and today I took my first bath since the surgery day,.. I took 2 hot baths and changed my bandages twice. From today on I'll do it just once a day.

I'm lucky this Airbnb has good curtains cause since Sunday I'm walking here with a boxer short and, coming out of the front side hole, an erected dick full of bandages.

4 days closer to your goal!

I am not sure exactly what "when I sit or lay on my back the tubings get very palpable in front/between my balls." exactly means.  When I feel the right side of my scrotum, I feel the pump bulb down low. Moving up is the valve assembly with my testicle next to it.  If I really probe up high, I can feel an inch or less of tubing that comes out the top of the valve assembly.  There is no excess tube, and it is pretty rigid or rather stationary.

I am surprised that if you still have bandages that you can submerge in water.  Doesn't bandages suggest a wound that is not sealed?

I touch my balls while standing in these positions (laid down or sit still) and I can see/feel a part of the tubings in my ballsack. I think it gets better with time since some people also present some problems with tubing and pump positioning in the early days after surgery. Or maybe it got mispositioned. If I remember correctly, excess RTE can cause some problem with tubing visibility in the scrotum. But I got just 1.5 cm RTE, not an excess.

Regarding bandages, it's just to seal the area below my glans that got cut. I take a bath with them and right after I remove, clean the area and put on a new bandage.

Itis true that RTE's do move the tubing connector on the cylinders forward, but I would not worry.  It is interesting how things readjust and settle down as you heal.  It seemed like my pump was in a bad position for the first few weeks, then everything seemed to adjust itself.

Another day (day 6 or the fifth day after the surgery) and things are going well.

- I'm still pulling down my pump. Gently, but firmly.

- The implant is heavier than a natural dick, but I think my muscles will get used to it. Walking with an erected implanted dick is still causing a bit of discomfort, but I think this is normal. Not even a week has passed since surgery. Ah, and I won't walk around with an erection hahaha.

- I worry a bit if the tubings will keep apparent as they are now.

Ah, something I've noticed: I've been told the penoscrotal approach was done to put an implant into my penis, but I don't have any scar in my scrotum. Wtf? I'll ask the doc about it.

Well you certainly look straight.

It looks like you have an incision in the fat pad over the penis that would indicate a retropubic approach. And at least at this stage have a good upright angle.  Do all you can to preserve that because it will drop as you heal.

Impressive that you did not have Ed but still went for the implant. I remember a few posts here from some trolls and others that were vehemently against this and where saying for you to go this route only if you were "70 years old with a limp penis".

Honestly, so far it looks like you are on track to have a very good outcome with a straight and functional penis. and that is very nice.  8)

I think he made the correct decision. Once I am done with classes and I start working again I will go the same route if i have not fixed things. I'm not saying it's the only way, but for me I think it's probably the best route. I have ocd already and I don't like having to worry about what may injure me what may not etc. it's just not healthy for me to think like that

Not much to add. Yesterday I didn't use Paracetamol, just dipyrone. I think today I'm not gonna need it. Pain is much better now (if I can call it pain... discomfort would be a better word).

Recovering is surprisingly easier than I was expecting! Good!

When my scrotum tightens (when excited, cold, or sometimes randomly) the tubings aren't visible and its aesthetic is much better.

I won't update this journal every day since there's not much to add these early days. Of course, there are more exciting updates coming regarding sensibility, size, sex, etc...

I'm sorry if I sounded like a troll in the past, but I never meant to troll here.
I was never against implants. I just think there are some prerequisites to put one, and it's ok if you disagree with me, we're not here to agree with each other all the time.
Of course, these prerequisites are a bit subjective and people should do what they want with their bodies. In my case, I had curvature, narrowing, buckling, and a non-stop Peyronies progression. Just to clarify, I don't think having Erectile Dysfunction should be necessary to go to the implant route. Each case is different. These are my 2 cents.

yea, man. I think I really made the right choice. The immediate results I can see are the straightness and the increased girth. I'm happy with the results so far! I'm gonna need more months to analyze every aspect of the surgery, though.

Yes, I think I posted about this earlier.  I would not put downward pressure on my flaccid penis at this stage, much less my inflated penis.  Here is my earlier comment.
Tortão,  My 2 cents worth of advice is to do NOTHING to bend your penis down because I am very convinced it significantly influences the angle of your erection when healed.  The uninflatable portion of the cylinder plus the RTE's are stabilized by a scar capsule that will form in the crus.  Like a fence in a fence post hole, any enlargement of that capsule will alloy the post ( your penis) to tilt more toward the floor when inflated.  It is still very usable but loses some aesthetics, especially if your natural erection pointed upward when standing.

Sorry, I understand.  Yes, I think it is your comfort option to wear it up snug, or just give some support to keep the weight of it from pressing down.

I never had to deal with a long-term inflated implant.  I am sure it is a challenge.

8 days since the surgery. So far, so good. Feeling almost no pain. My biggest discomfort is when I sit for a couple of minutes and then get up. I don't know how to exactly describe it, but it's a pain at the side of my balls, almost in the groins. After a few seconds, the pain goes away and a small discomfort takes place.

Regarding size: my penis looks so thick! Its shape is very good too! I know some guys complain about an oval shape after getting an implant, but fortunately this is not my case (at least, not yet). I don't have a ruler with me in São Paulo, but I'm sure the girth at the base is 14 cm or more! And I've not started to cycle yet! ;D I'm happy about this!

I have a question for the doctor (I'm gonna visit him tomorrow) but I'm gonna question here too:
In the early days after surgery, after starting cycling, is there a best position to point my flaccid penis under the pants? I don't know if the position while flaccid also influences the position while pumped up...

Do you have any expectations of what your surgeon will do tomorrow?  Do bandages come off?  Do you get deflated?  I assume you are not totally inflated at this point.

When you deflate the stiff new cylinders, plus the swelling will make you look semi-erect.

So much is difficult to tell at this point.  You surely have some swelling.  You don't know how much more you can inflate.

Your angle will drop but maybe not from where you are now because you are less than fully inflated.  The next week will begin to tell a lot, and it is bound to be good news.
Do you have any "before" photos?

I would wear everything up.  I would put NO downward pressure on the base.  If you are determined to wear it down, I recommend pointing the base up and bending your deflated penis about an inch out from the base downward.

It was quite a bold and brave move to decide to underwent surgery. But at the same time, it was THAT defining moment, of setting yourself free from the erratic and diabolically capricious grip of this evil disease. To not let yourself being defined by this problem, but to rise against and reclaim your own mental, physical and sexual life back. I wish you all the best. In no time you will be facing much different problems, like dealing with all those demanding ladies. Parabéns!

I recently finished my appointment. I got deflated and trained on how to deflate/inflate.
Inflating is not painful, but deflating is very. The reason is I need to squeeze my penis, and it is still sore because the plaques got a lot of incisions. Squeezing the deflating button in my ballsack also hurts.
Tomorrow I'm gonna get an airplane and return to my hometown. I'll deflate and point my penis up under my pants to come home deflated. I foresee problems coming, like security wanting to view what's under my shirt or whatever.

Right now, I'm deflated and looking semi-erect, pointing to 4 o'clock. It gets better, right? When deflated for the next weeks, I'll let my penis pointing up under my pants, but after healed I really want to see it pointing 5~6 o'clock (when deflated).

Right. But is it recommended to point it up for how many weeks? I read Merrix's journal and he has a good-looking flaccid penis pointing down.
crap, I made so many questions to the doctor but I forgot to make this one. He recommended me to wear it up or pointing to the opposite direction of my Peyronie curvature, but I forgot to ask for how many weeks/months.
I fear pointing it up and getting a 4 o'clock flaccid, or wearing it down and getting a below 3 o'clock inflated.

I have before photos, but not with me in São Paulo. It's in a Pendrive in my house. I'll post them as soon as I can.

Tortão, I smile, but I remember all my own concerns that seem a little crazy now,

Wearing your penis up is NOT going to give you a 4 O'Clock flaccid penis.  It will, however, help give your erection a better angle. Your penis will eventually settle down to 5 O'clock or better, NOT because you make a loose scar capsule by putting downward pressure during healing.  It will hang below 5 O'clock because the cylinders themselves soften.  Those cylinders will stretch and become more flexible, and their Hydrophilic Coating will actually absorb fat from your body which will not only make them more limber but slicker and less crinkly.

How long?  I am not sure.  I think the answer is when the penis can hang down without prying down on the crus but because the shaft bends rather than tilting down.  A good rule of thumb is when you are at 5 O'clock or better in the shower, it is safe to wear it down.  My guess, without looking it up in my journal, is 3 months.  I am right at 7" (hard bone pressed) with about a 5" girth on the distal half.  If you have more mass, it might be sooner.  Less mass might take a little longer

Today is the 10th day after surgery day.
I took a plane and bus to get home.
Well, all I can say is I'm definitely a newbie regarding how to keep my penis under my pants. I truly need a little help here.
In my first appointment after surgery (day 3), I was 80% inflated and went to the doctor's clinic. It was embarrassing. I just can't bend my penis while inflated. I remember before surgery I could never bend my penis totally to the left or right when I had an erection. Pointing it upside was the best I could. Now, I can't even point it upside while inflated. Well, on day 3 I tried to keep my penis up but to no avail. When I entered the doc's room, my glans was yellow to white. Circulation problem, of course, because my pants were tight trying to keep my penis up.

When deflated, things are a little easier. Today when traveling I could keep my penis up and a little bit to the left. It's painful and it's obvious there's something down there. Unfortunately, this bending left a small crinkle on it. When I came home, I inflated to 90% but that crinkle didn't disappear and it is still there. It wasn't there days ago, as you can see in my last photo. crap, this is concerning.

The doctor told me to start cycling: 12h inflated,12h deflated. Well, I told him I simply cannot. I'm in my home now, how can I work or see my family with an erection? I can't bend it. I told him I'll keep my penis inflated while sleeping (7 or 8 hours), he answered me it's ok but I need to sleep on my back.

Truly, I need help on how to keep my penis under my underwear when cycling (inflated) and when deflated... I appreciate any tips from you, guys!
