Hi all,

The past year has been rough which Covid has exacerbated greatly. My wife had her ovaries out. My mother died. Both of her parents died. We each started new jobs that are intense. And our young children are doing virtual learning which is tough. Sex has been lacking for the past year primarily due to her surgery but with everything else going on (I work nearly 16 hrs daily) and we both pass out by end of day. I did notice at one point I no longer had morning erections. I actually said this to my primary doctor during a physical in Nov 2020. We chalked it up to all the stress and everything else my family has going on. I still masturbate several times a week non-aggressively. I noticed I wasn't getting fully erect and again chalked this up to everything going on and also the fact that I was often rushing to even fit that in.

At one  point earlier this year I notice my erection was pointing off to the left with a bend/dent at the base. Penis all in all is very straight but pointing off slightly to the side starting about a cm down the shaft away from my body. I figured I just wasn't fully erect and filled out, but I kept an eye on it. I later noticed that this could happen either way, left or right, with a bit of hourglassing at the base. Again, I thought this was just because I wasn't fully erect and filled out. Then I woke up one evening in the middle of the night with a full blown erection. It was painful and I felt the hourglassing which was fully hard and seemed like hard dents on both sides and a band/groove across the top connecting the two, all right at the base very close to my body. It looked like I had a rubber band around my base for 15 minutes and had just taken it off. Underside felt normal though and it's only top and sides. I immediately thought Peryonies. I did some research online, freaked out even more, and then set up an appt with my primary who I managed to get an appt with the following day. I had also seen a urologist a few years ago so I called his office and also got an appt with him the following week since I was an "existing patient". I saw my primary who again, brought up all the stress in my life and everything else I have going on. He looks at my holistic all-encompassing health, as he should, also mentioning some recent bloodwork he wanted to retest. (Just slightly over normal cholesterol.) He did examine my penis and didn't notice anything but said I should see one of the urologists at this hospital system who is very well versed in this. He recommended the Dr I already had an appt setup with. I live in a major metropolis so we have several really good/big research hospitals in the area.

I saw the urologist the following week and told him the whole story. He did a physical exam and though completely flaccid he could feel the scarring at the base immediately where I and my primary doctor could not. He said all in all I wasn't that bad and he felt confident in my prognosis. He had me start Vit E, Pentox, and recommended RestoreX. I'm also scheduled for an ultrasound this upcoming Monday where he will induce an erection and perform the test. I started Vit E and Pentox the following day and ordered the RestoreX which I started as soon as I received it. I've eased into the RestoreX per the recommended schedule. As far as tension, I'm taking it easy and haven't buried the white lines often. The tension seems strong and as it was, the first few days I was sore and it felt like there was significant stretching at the base. It almost felt like things were ripping, but in a good way. I was actually sore during the in between times and felt better while doing the traction and after for a while. It's been nearly two weeks of Vit E and Pentox and about a week and a half of RestoreX at this point. I woke up last night with a painful erection where the pain was across the top of the base of my penis where I have the indented band. I'm not sure if that is "hinging" but I feel a definite groove across the top of the base from side to side. I do feel like the dents in the sides are not as pronounced. I still see hourglassing but I don't seem to feel the hard denting on the sides on either end of the groove that I did before. Hoping I'm not just imagining that and I have some early improvement.

So that's where I'm at. I guess depending on what I'm told Monday, I was going to also ask about Cialis and VED to assist with the groove and hourglassing and to try to pop all that out from the inside, so to speak. I could no doubt have sex but I definitely do not want this to get worse and would obviously love improvement. Hourglassing, grooving, and denting at the base seem like a recipe for further injury/issues.

Lastly, I also made an appt with the head of the men's urology and the genitourinary trauma program at another local well-known research hospital. He is supposed to also be well versed in Peyronies so I just want a second opinion as far as what he/they recommend as a treatment protocol.

I welcome any feedback and advice.

Welcome to the forum.  I want you to know that you are among those who understand every aspect of the things you are facing, both medically and emotionally.

Thank you for your great first post and for filling out your signature line.  I broke your post into paragraphs to make it a little easier to read than a block of text.  Many read these forums on small phons.

It looks like you at least have a decent urologist who is coving the bases.  One additional thing I would recommend is heat when you can, and I think it has the most effective before traction.  It is important that you not overdo the heat.  It is possible to kill cells.  We are not grilling hotdogs  ;), only encouraging blood flow and relaxing plaque.

Thanks, Hawk.

You mentioned a similar prostate cancer treatment path. I haven't had prostate cancer, though. I hope you don't have a crystal ball!

I did read through the survival guide and actually just ordered the supplements mentioned there and around the forum. L-arginine seemed interesting and as I was ordering that, I just added on the other 2. I'm not expecting miracles but if they offer any benefit, complementary or otherwise, I don't want to leave that stone unturned.

I have tried heat but had forgotten to mention that in my original post. I have a heating pad on low just before some traction sessions, if time permits, for a bit and I feel it does exactly what you stated.

I am liking my urologist thus far. He zoomed right in on the scarring or plaque or whatever when I, nor my primary, could feel it. Also, the fact that he recommended the RestoreX unprompted, I took as a progressive and well-informed sign.

I will report back after my ultrasound tomorrow.

Welcome Dorian ! You are in a great forum with a lot of info. It is great news that you like your Dr. Many of us here have had to visit several Dr's before we found the one for us  :)  Mike

Quick update: I had my ultrasound today. No calcification. Doc said everything else he looks at/for like bloodflow and whatnot was good and healthy. I did show him a picture the first time I saw him but with the induced erection he could see everything for himself. He mentioned the band/hourglass at base when talking to his resident. At this point I asked about Cialis. He said I didn't really need it. I mentioned folks taking it at night for more fruitful NTEs and the possible benefit in that regard. He said OK and called it in. I then asked him about VED. He said it wasn't necessary since I was doing the RestoreX. I said much like the Cialis to help things stretch and hopefully fill out at the base, it seems potentially beneficial in the girth aspect. He held steady that RestoreX is superior and is all I need to do diligently. So, obviously, I don't need him for that should I choose to give it a go but I just wanted to get his thoughts. He wants to see me again in 4 months.

It seems you got some good news.  You have a jump on your plan of action.

Make sure you have objective measurements as mentioned in the survival guide so you don't have to guess about progress.

I saw the urologist I had setup for a 2nd opinion, not on the Peyronies diagnosis but rather on the protocol the first doc prescribed. I was hoping it would be the same outlook or perhaps different where I could integrate both recommendations.

He's at one of the other major top-ranked hospitals (nationally) that happens to be in my city. He was able to see my tests and notes from the other urologist/hospital and also did his own physical exam.

1st doc, as mentioned, has me on pentox/vit E/tadalifil and doing RestoreX 30 min twice a day. Did an ultrasound and said no calcification. Wants to see me in 4 months.

2nd doc said his preferred cocktail is pentox and a long acting nsaid but since I'm not inflamed, he would not recommend the latter for me. He said he only does it for a month anyway. I told him the supplements I'm on to which he said was fine but the reality is that no oral treatment, pentox included, is likely to be of benefit. Likewise he said traction/ved offeres little value and he is give or take on doing it at all but I can if I want to. Lastly, he said the ultrasound at this juncture was meaningless. He basically said it's a matter of waiting and seeing if I improve, stay the same, or decline... and that's when decisions need to be made. Said injections would not be suitable for me since I do not have a curve and just the hourglassing at the base. So it would be a surgical conversation if I get worse. Wants to see me again in 4 months. He said that is usually how long he does Pentox for and not the 9-12 months my other doc said.

It's a bit of a bummer to hear the above from a doc at another prestigious institution. I like that my 1st doc is proactive and more positive in general but I'm hoping not erroneously so.

Don't get too down about it. All of us gents here have had our disappointment with Dr's. It's time to shake it off and at least heed some of the advice of your 1st Dr's visit. It depends on you and how you feel about him treating you.  Does he really listen to you or is he starring into outer space when you talk to him? In the meantime you could still explore other "peyronie" specialists. Remember you are paying them to treat you.  You also have the survival guide to reflect upon and countless articles in our resource library to gain more knowledge yourself.  :)  Mike
