Peyronies and Hairloss Treatment

I've seen this topic floating around about, most notably the Peyronies and Finasteride debate, and thought I'd write up what I've found that might benefit us with both Peyronies and hair loss (androgenic alopecia). I personally was advised by Lue not to take oral finasteride, his main point seemed to be impotency / erectile dysfunction.

Now, this is a shame, because anti-androgens are the most effective way at battling androgenic alopecia in the long run. However, people in the hair-loss community who want to avoid the systemic side effects have been exploring topical anti-androgens and while there are no current FDA approved options in the US for topical anti-androgens there are several options.

Low-Mid Level Topical Anti-androgens: These would be supplemental treatments that likely will only slow progression in combination with other treatments, this is because they are not strong enough topical anti-androgen treatments to battle scalp DHT. However, they are effective in conjunction with each other.

- Nizoral ketoconazole 1-2% shampoo. This anti-fungal / anti-dandruff shampoo contains an active ingredient, ketoconazole, which is also a light anti-androgen. This alone is not enough to battle the long term effects of scalp DHT, but will help, its considered to be one of the "big three" of hair loss prevention.
- Other shampoos and ingredients: Regenpure Shampoo is also a good shampoo for a hairloss regiment, or get a shampoo with, pyrithione zinc, zinc ions inhibit 5α-reductase
- He Shou Wu / Fo-ti Root: This has traditionally been used in TCM (traditional Chinese medicine) for hairloss and grey hair. There is now research that demonstrates He shou Wu both extends the growth phase (anagen phase) of hair growth and can counter the androgen in hair follicles. I will be trying this as a topical application.

- Saw Palmetto: People take sometimes natural remedies as an oral supplement, but I've also seen good accounts of it when mixed as a topical solution.

Stronger Topical Solutions:
- Topical Finasteride: There is an increasing amount of people who got finasteride sides and replace it with topical finasteride that they either make themselves or order from a chemist. I personally am quite interested in trying low-dose topical finestaride and will probably consult Lue and a dermatologist before doing. The scalp is quite an effective area for absorption, so its necessary to properly ratio, which you can find on reddit or forums to see what works. There are a couple of studies that discuss the use of topical fin:

- Ru58841: This is a strong anti-androgen that people buy in powder form and mix into a topical solution that they may use alone or mixed with minoxidil. There are some internet users who have reported some sides, such as red eyes and they typically recommend stopping, but there is strong support for it from those who have success.
- CB-03-01: This might be the first topical anti-androgen to be FDA approved, look into it.
- If you look into Ru58841, CB-03-01, and the new topical anti-androgens, I highly suggest the "MorePlatesMoreDates" youtube channel hair loss section, this dude is the internet guru of hair loss and the science of this stuff.


Here is the bottom line about hair loss, especially when you want to use anything other than Finasteride. You need to combine it with multiple treatment options. In an ideal world you would use 1.) an effective topical anti-androgen that doesn't go systemic, 2.) a growth stimulant such as Minoxidil, PRP, or micro-needling, 3.) scalp conditioning/reduction of dandruff, oil, dryness, or inflammation.

I hope this information is helpful to anyone who has Peyronies and wants to have an effective hair loss regime without taking oral finasteride or dutasteride.

So oral finasteride is definitely a no no for us? I'm surprised you didn't mention minoxidil at all

I would never take finasteride. It can shrink your penis and lower your testosterone or incur brain damage
not to mention a list of other bad stuff. Sure there are some men who don't experience this - I would not touch it.

But minoxidil is fine, right?

I have used minoxidil and it does not get into the bloodstream. It worked a little bit and it does not fool with your body chemistry.

Do you think topical azelaic acid is safe? It has been shown to induce hair growth

I have never used azelaic acid. I don't have any knowledge with it.
