Do I have Peyronies Disease?

Hello there!

I am 34 yrs old and have a leftward bent in the penis since I was 19 (if I remember correctly). It has been a 20 degree bent. Never had issues with Erectile Dysfunction.

Since my divorce about 18 months ago (everything was fine then sexually), I seem to.have issues with Erectile Dysfunction lately.

First time sex with another lady I didn't get hard at all. Second and third time I started to get a 80% erection, afterwards I got 100% hard, but I seem lose it the moment I stop stimulating manually.

Note: With Erectile Dysfunction pills I CAN maintain my erection after getting hard and can go several times a night with my girl. It is exactly as it was in my "NO Erectile Dysfunction times".

My questions:

1) I asked the urologist to do a Doppler. He is supposed to be an expert on Peyronie's. He spotted "a very minor plaque of 1.8 millimeters" on the left side.

Is 2mm of plaque enough to call my condition peyronie's?

2) He said in his view I don't need any treatment. So if I understand peyronie's correctly, this would be a sensible recommendation if I am out of the acute phase, right?

3) I know that my Erectile Dysfunction could theoretically come from Peyronie's. Given my background and the "small" plaque according to the Doc, is it still possible that my Erectile Dysfunction has nothing to do with my bent?

I am sorry to bother you with these questions but I am happy to receive some feedback.

Welcome to the forum EMV.  You must know your questions do not bother us.  We are here to help those who want help and answers.

If you have a plaque at 34 with no known injury, then it likely is Peyronies Disease.  If you are stable with no pain or discomfort, you probably don't need any treatment other than a few supplements mentioned in the guide and a healthy lifestyle.  You also need to be aware that your penis might be a little vulnerable if injured.  I would advise against trusting a woman on top.  Many men are here as a result of injuries in that position.  Also, avoid intercourse with a semi-erect penis that bends (collapses slightly like an accordion) as you thrust.  Make sure you use adequate lubricant to reduce friction.

It is impossible to say if the Peyronies could cause your Erectile Dysfunction symptoms or if it is totally separate.  Was you Doppler Ultrasound with an erection and did he look forr venous leakage?

PS: One final question.  Did you have a curve at 15 or 16 or did the curve develop at 19?  If it did develop at 19, did you have an injury that you know of?

Regarding your question, the only thing I recall in my youth is a small abscess / pimple on the left hand side of my penis,.where there is the bent today. But this is something impacting the skin and thus less relevant, right?

My big concern now is that my erectile issues 3 months ago may made me try to have sex with the lady whilst not fully hard -- and that this may have caused a trauma, leading to the plaque 3 months afterwards...when I first presented to (another) doc for Erectile Dysfunction that Doc also did a Doppler and explicitly said "no plaques". So I am REALLY concerned now that the plaque may have developed over the last three months as opposed to my youth. The bent is pretty much the same 20 degrees to the left as it was 15 yrs ago. Or maybe 3-4 degrees more. Quite unsure now...

If I have no pain (which I never had): is this indicative of NOT being in an acute/inflammatory case?

Doppler readings are far from perfect, so even if you had the plaque, he could have missed it. To worry about when you developed the plaque is not helpful.  If you did it having sex 3 months ago or at 19, you could not know, so it is not your fault, and it was unavoidable.  I was trying to figure out if you had a congenital curve.  The pimples you mention are unlikely to have any connection.

I would guess that it is not in the acute (active) phase, but some men never experience pain.

I do have a follow up questions. I noticed that when I use PDE5 inhibitors, my penis is MUCH less bent, perhaps 10 degrees. Is this something not unusual? What could it mean?

I suspect it just means that your erection is more firm and applying more stretch to the restricted side.  Sometimes the very first signs of Peyronies Disease can be a curve in the flaccid state that vanishes with erection.  I think the greater rigidity from the PDE5 inhibitor gets you close to a maximum erection.

Thanks a lot, Hawk, for your reply.

So is it fair to say that getting back my 100% erections could get back my curve to it's "natural" level that I always used to have? As I said, when I use 5mg of PDE 5 inhibitors I have the  approx. 15° angle that I always had. I seem to get better by with my burnout these days and hope that I can get back my libido which may give me the erections I had a year ago.

As for an update: Interestingly, after 4 weeks I went to the same urologist. He checked again using the doppler ultrasound. He could not find the 1.8mm plaque. He really tried for 15mims (!) and was surprised that he couldn't spot it.

What I also wanted to ask:
1) Can doctors see on the doppler whether the plaque is calcified or not?

2) If the disease is in the fibrotic (inflammatory) stage, can they normally see this or any anomalies in the tissue hinting to this stage?
