24yo, I fractured/injured my penis, got diagnosed, very sad. Hope to find help

Hello! I am Tom, a 24 year old male and ran across this forum and have been really pouring over everything. This whole thing is really depressing for me and I hope i don't sound like an idiot but...well...its sad.

About 3 months ago, I accidentally injured my penis during a masturbation session. While masturbating, I pushed my penis slightly down while gripping it in order to aim during ejaculation. When I was ejaculating, the penis tends to force itself up, and given the grip it made a 'cracking' or 'popping' sound on the top side and midpoint of my penis. It was a deep crack noise like when you crack your knuckles. It felt very uncomfortable, but no lasting or significant pain. However, some veins on my penis were immediately swollen which seemed like cords and could be felt. The strange thing was that at first I was still able to get erections at first, but that my penis felt very numb/dead/soft when flaccid. More than usual, as if underneath the skin it is empty. To my memory, my flaccid penis was always somewhat hard and ready to get erect.

I contacted a urologist, whom at first advised me conservative treatment (just waiting, keep on living) for the first two months to let it fully heal on its own. He said, that if it isn't fully healed by then, I should contact him. Since I was experiencing mild pain during the first month, I put ice for 20 to 30 minutes on my underpants just to do some kind of home-treatment which seemed to be standard-advice when Googling for ''treating a fracture''. I did this icing for three to four days. I had different kind of issues (hard flaccid, 'dead' penis, severe erectile dysfunction, swollen veins, bending during urination etc.).

After two months, I noticed the issues were still not resolved on its own. So, I went to the urologist. He gave me a visual and hands-on examination to check for plaque (scar tissue). He did not found any issue. He then performed an ultrasound to check for damages in both the corpus cavernosums and spongosium such as - if I remember the used terms correctly - plaque, fibrosis, mondor, tears etc. He did not found any damages and no significant differences between the corpus cavernosums. He ensured me that this is probably a mild penis fracture or injury that should heal on its own over time. According to the urologist, if the injury was major, I should not have been able to get erections shortly after the injury and not during the first days of the injury.

He however did describe me a treatment to follow: daily Vitamin E 400 IU and daily Cialis 5mg before going to sleep for at least 3 months. The first two days of taking Cialis made a significant difference, I got an erection so hard it was unbelievable. It still wasn't 100% in terms of pointing upwards but it was harder than I had ever had. However, the third day, when my penis was in the process of becoming erect, I felt a sharp pain on the left side of my penis which felt more and more severed. At one point, it felt like something teared like a elastic band or something. Since then, I am having mild burning/throbbing/itching pain on that side of the penis. This pain sometimes increases when my penis is getting an erection. The pain is present in both flaccid and erect state. Sometimes, only during flaccid state. Furthermore, I notice that the left side seems like a weak spot. When I want to get an erection it tends to bend to the left and the left side doesn't seem to have as much strength as the right side. As if there is a blockage to getting an erection and growing to full size. I also can not get a full-erection in te sense of hardness and it pointing upward (which it could do more in the first weeks of the injury).

Given the sharp pain I felt, I just contacted my urologist again and went to the urologist to let it check out again. Yet again, the urologist felt no scar-tissue, plaque and did not find any damages during the ultrasound of my flaccid penis. He again ensured me this should heal on its own and the pain will subsidue over time, and concluded that it is probably a tissue, blood vessel or something similar that was injured/damaged/torn which should heal on its own. He categorized it as a mild fracture. The rings outside the cavernous bodies.

My current issues are as follows.

1. The left side (flaccid) feels more sensitive when I touch it. Not very painful, but sensitive and irritated. Pain present on that side, mainly during flaccid. Sometimes when I try to get an erection, this side seems to hurt and have a hard time joining with the right side causing it to bend to the left (I guess?). What I notice is that what hurts in particular during certain situations: like sneezing, when I tighten my muscles to force my erection or to want to ejaculate. They seem to be the same muscles as when I want to hold back my pee.

2. Erectile dysfunction. It is harder to get an erection, to maintain it. I yet do not had it on full strength (100%). The erection strength varies per time, sometimes it is 30-40%, sometimes 60-70%. It also seems to not point that much upward as it was before the injury or during the first few weeks of the injury. During the first weeks of the injury, while the erectile function was a bit weaker, it was still similar to its old shape (like shape,form, pointing upward etc.)

3. The penis seems to curve/bend to left while flaccid or when semi-erect. It always had a natural curve of 5-20% but now it seems more severe. When full erect, the curve is not present and is not significantly different as before the fracture. However, the curve seems to happen as if signs an issue on the left side. Currently, getting an erection is difficult as the left-side seems to 'hang' and just bend/curve left because it cannot expand. I can straighten it out, but the erection does not seem to happen.

4. Veins are still swollen, much less now but still present. If it is cosmetic, I do not care but it could be an indicator/sign of something. I do not know. At least, it wasn't swollen before the injury.

I already read the Peyronies Survival Guide - Information for New Members. Currently my lifestyle is as follows:

- I exercise 5-6 days per week, each day 2-3 hours (45 min cardio, and 1.5h strength training)
- I cut off meat (just started this month)
- I take daily Vitamin E 400 IU and daily Cialis 5mg pills before going to sleep
- I take multivitamin pills each day before going to sleep
- While I masturbated/had sex on a daily basis (at least 2 times per day), I now minimized it to 2 times a week or sometimes even 1 time per 2 weeks. It was hard at first, but now much better to handle. I did not have sex anymore since the injury, I only do gentle masturbation with almost no grip. I notice that whenever I masturbated/ejaculated the erectile function worsens a bit and puts me in somewhat a earlier stage especially with respect to the curve/bending.

Now I am at a loss, especially the urologist did not find any damage or issue whatsoever and specifically stated he does not know either what it is. He hypothesizes it is probably a tear/injury/fracture of the outside ring of the penis such as a blood vessel, vein or tissue. So ...here i am...dealing with it. I hope I can talk to some of the experienced guys here and see what can be done.  What is going on? Is there any hope? Thank you very much in advance for any help or reply.

Nothing you've said suggests early onset Peyroines to me. That's just not typically how it starts or sounds. I get the feeling it's just some sort of residuals left over from the trauma, which sounded kind of minor. So, I'd just go with the advice of the last urologist and keep seeing him or her if things don't get better on their own or worsen. Maybe repeat the ultrasound after awhile? Seems reasonable.

A cracking sound on an erect penis is the most tell tale sign of a penile fracture. It is going to depend on how severe or not was the injury. I take it since you didn't mention hematoma or swollen penis that it wasn't the case so take it as a good sign. Not all fractures are severe. With this being said.... now you have permanent ED. What level is going to vary on several factors. It may or may not progress to peyronies.

Start of with Daily cialis 2.5mg and take it from there. You can use l-arginine on top of it to help you out with the erection hardness. But, this is not a thing that your doctor will feel from the outside. I fractured mine as well, and fortunately have no plaque but it lead to fibrosis (mild) and arterial insufficiency which is different from Venous leak.

I can go more in depth if you are interested. Bottom line is, don't despair. You have plenty of options for treatment.

According to him, there were no differences between both the corpus Cavernosum and no abnormalities were found. He performed an ultrasound 2 months after the incident and 3 months after the incident (last week). Both were clean.

My main issue is erectile dysfunction. I find it especially strange that the erections were still reasonable in the first week of the trauma. I feel hopeless that I haven't gotten a (good) erection at all this month. My body wants an erection, but at most it is a weak erection which is not full size (length/thickness) and which bends because the left side appears to be weak. It's so strange that erectile dysfunction has deteriorated instead of improved, and that whenever some improvement is noticeable after nofap that whenever I masturbate, that it gets back to an earlier stage.

I am taking Vitamin E + 5mg Cialis on daily basis since the second month of the injury (which is now 1.5 months ago).

I'd try upping the daily cialis to 10mg and if still needed add viagra (59-200 mg) 30 minutes before sex..

Is there any hope at all?

It is so strange that my erectile ability has actually diminished, and that my penis initially responded very well to CIalis (at least for the first two days).

I get depressed when I have to remember to go on like this all my life without erectile ability.

My current issue is mainly mild pain (left side), severe erectile dysfunction and if there is any erection at all (even 40-50%) my penis does not get as big as it was. It is much smaller.

What's strange, or not so much is you not listening at all to what i posted. There have been dozen of cases like yours here in the board throughout the years. After a fracture, even if it's mild, ED will always progress as time goes by. It may take years, or it can be quick. Depends on several factors. Your age, health condition and lack or presence of other diseases....

You should familiarize yourself as much as possible with this condition. The faster you understand that your penis "won't magically fix itself" and that your erections won't magically improve, the better for you. Now, you need to start looking at treatment options. Cialis daily... avanafil.... gel based ed creams... certain types of diets as well as other medications and types of food help you get better erections. BUT, they won't fix your ED. I don't want to sound pessimistic, or send you into a depression pit but i can't lie to you either.

I did see what you posted. You suggested to start taking 2.5mg Cialis. I in fact am doing that, even at a higher dose (5mg).

Thank you for your word of encouragement. Today, I saw my urologist. He advised me to up the dose with Cialis from 5mg daily to just 10 to 20mg once a week. He told me that the erectile function SHOULD fully recover on its own, on which a temporary use of Cialis can help to stimulate it, especially as no damages/plaque/fibrosis or anything deviating has been found.

He told me I should wait another 2 months to see if there are any improvements. While he ensured me that the erectile function will recover fully on its own, he mentioned that the penis can start curving throughout time. If that remains or start worsening, I should consider surgery to try to straighten the curve, which is up to me he said. But I am currently not considering any surgery. I am fine with how it looks, I just need to get the erectile function back at 100%.
