My introduction. Recently injured my penis.

Hi all,

Two months ago, I have injured my penis. Since then, I am continously researching what I am going through and what can potentially help me fully recover. During my research, I stumbled upon this forum. Looking for any help on my matter.

First of all, I would like to give some background information:

Gender: Male
Age: 26
Medical history: none
Smoking/Drinking/Drugs: none
Height: 5''10 (1.80m)
Weight: 171 lbs (78kg)
Duration of issue: 2 months.

Two months ago, I injured my penis during masturbation. My penis naturally points slightly left (10-20 degree) during an erection and upward during probably a strong ligament. As usual, I push my erection downwards when I am close to ejaculation in order to aim so it is not making a mess on the wall or on other places. However, this time it was different. Right as I was pushing downwards, and right when I was ejaculating, I heard a quite noticeable and loud cracking/popping/breaking sound that came from the top of my penis, probably around the midpoint of my penis. During the crack, I only experienced an immediate uncomfortable feeling at the place, which thereafter quickly dissappeared within seconds. No pain. It was a deep crack noise like when you crack your knuckles.

Immediately, some veins were quite swollen looking like cords running through my penis which I also could noticeable feel. Also felt some mild pain during the day for a very short period at random occassions at different places of my penis. However, I was still able to get erections but my penis felt completely dead/soft when flaccid as if nothing was inside the skin. At least, the 'flesh' felt very empty or very very soft. As far as I remember, while my flaccid was spongy it always had some kind of hardness. Always ready to go straight up. I also felt some burning/throbbling pain whenever pissing deep down the penis line from base to glance/head.

I became increasingly worried and therefore contacted my General Practitioner who initially preferred conservative treatment while four weeks later (on my request) referred to an urologist. The urologist gave me Vitamin E pills and also prefers conservative treatment for at least the upcoming three months. While no ultrasound/echo was performed or deemed necessary by the urologist, he assured me it could heal over time. He thinks it is some injury and sounds like Peyronies. He saw no reason to have emergency surgery performed. He furthermore gave me some erection pills (Cialis/Tadalafil) which he advised me to use one time a week whenever necessary to support erections. This was the fourth week.

Immediately, I started with the daily Vitamin E pills. Not yet with the Cialis at the time. I have never used Cialis before and given my injury I was concerned that any kind of erections could lead to further tearing. Also, in combination with the urologist's advice (taking a rest), I was not very excited to take Cialis. Also did not want to risk the dangerous side effects (blindness, deafness and priapism). Furthermore, did not take the erection pills as I observed that I was starting to be capable of getting erections on my own and did not want to start to be dependent on erection pills. A very slight form of massaging/masturbation/ejaculation also did work without any issues. For the pain, I used an ice-pack which I placed on my underwear for 30 minutes on three separate occasions. These were 3 days in a row.

While I visited and saw an urologist, I was still concerned. Especially as it remained unclear if there was any damage and to what extent. Therefore, I made an appointment with another urologist which is more specialized in Peyronie. When I visited the other urologist (also for a second opinion), he immediately took an ultrasound/echography. After performing the ultrasound/echography he indicated that both corpus cavernosa are in order. No damage found. The urologist noted that however it does not rule out that there may have been damage, but no deviations are visible at moment of the ultrasound. The urologist indicated that this may mean that the damage to at least the corpus cavernosum healed on its own or that the fracture did not lead to any damage to the corpus cavernosum. This urologist also concluded that I should go on with my life and it should heal on its own. This one did not give me Vitamin E pills, but only Cialis. He stated that I should start using 5 mg Cialis/Tadalafil daily in order to keep the blood flowing. Interesting is, that this urologist advised Cialis on a daily basis instead of a weekly basis which the 'first' urologist advised. The meeting with this second urologist was 1.5 week ago. Still did not take the Cialis/Tadalafil due to worries about Priapism etc.

Now, two months have passed since the injury and 1.5 week after I saw the second urologist. While I in fact see improvements, I still have some issues which is not helping me to be less worried than I am before. At moment of writing this post, the current/remaining issues are as follows (bullet point 3 is new):

1. I can get an erection, but this still needs physical stimulation in order to reach some type of erection. If I get the erection, it is not 100%. It feels less rigid and I would rather scale it on 60-70%.
2. While there were morning woods in the first two weeks, these do not appear anymore since the second week. I feel that however, my body wants to get a morning wood. I get this feeling whenever I wake up early or during the night. However, it also feels my penis gets stuck during semi-erection.
3. The shape of my penis during full-erection did not change. However, when it is trying to become erect or whenever it is in semi-erect it curves more strongly than it did before. My penis had a slight natural curve to the left (10-20 degree), but now the curve is more strongly. Striking is that whenever I reposition my penis during semi-erect state on horizontal level (either straight or right), it seems it can be readjusted which seems weird. The more severe curve only happens when semi-erect.
4. The swollen veins are getting less noticeable and are seemingly disappearing each week passes. But, whenever I masturbate/ejaculate, the veins are yet again appear swollen to an earlier stage.
5. Mild pain/sensitivity occurs throughout the day at one specific place of the penis, which is the left side near the base.
6. No hard flaccid anymore, but still some kind of spongy very soft flaccid. It feels weak. So, overall, both the flaccid and erect penis seem to be weaker than it werebefore the injury.
7. My erection size seems to changed. It got 1 to 2cm shorter in length (it was 19.5cm and hovers now between 17 and 18.5cm. Maybe because I my stimulated erections are still weak but it is interesting.

Guess that's about it. I start to get more and more hopeless. Is this type of incident likely to cause any soft tissue injury/damage or is the penis pretty resilient?

What do you guys advise? Should I start with the Cialis/Tadalafil? If so, what are the best times? So, my first urologist advised it for only 1 time per week to start supporting erections on the midday or whenever I want to have sex/masturbation while the second urologist advised to take it daily before I go to sleep in order to keep blood flowing during the 'healing process' when I sleep. Furthermore, would I be still too late if I start Cialis/Tadalafil after two months since the injury?

The second urologist is right on.  First, he did an ultrasound and then prescribed daily cialis (take at night).  This will do multiple things: increase blood flow including morning wood to help combat any fibrosis that may have occurred or may develop.  Def follow this doc's advice.  Meanwhile read the survival guide here for more info.  https://www.peyroniesforum.net/index.php/topic,3180.msg44057.html#msg44057

I will be brief since I am in a time crunch.  First thing is follow Tony's advice and read CAREFULLY and THOROUGHLY.

You may have lost length because you are at 70% erection.  That is why you need to get objective measurements as indicated in the survival guide.
I find it interesting that you paid two urologists who both told you to take a prescribed drug and you defied their recommendations. When I consider you are risking masturbation that makes it even more puzzling because it is not like you are afraid to hurt your penis, only afraid to follow your specialist's recommendations.  If you are paranoid take a half a pill 2 different days.  Oral ED drugs need mental or physical stimulation to cause an erection.

Do you still get Night Time Erections NTE's ?  If not then definitely take the Cialis.  If you must masturbate during the healing process to so VERY Gently.  You would be more wise to pamper your dick for now so you have a functional dick for years to ahead.

Thank you for your respons. It makes sense to increase blood flow indeed.

I have now taken the pill for the first time this morning. As prescribed by the doctor, 1x 5mg. I did it this first time in the morning in order to test for negative side effects.

I soon noticed a change. The effect seemed to have started within 3 to 5 minutes of taking the pill. The penis started to get fuller and was soon ready to have an erection. No side effect, only feel significant more relaxed than usual (even compared to before the injury/fracture).

I was able to get a hard erection right away without touching it at all. I also didn't even have to fantasize, watch porn or use physical stimulation. Just relaxing was enough. Interestingly, beforehand I was able to get an erection, but so I needed physical stimulation and be focused to get an erection. Plus, I had to flex my muscles to get the erection, and those are the same muscles (as when you want to hold your urine) that make you cum faster. So when I had an erection, I was near ejaculation (severe premature ejaculation) That does not seem to be the case with the pill. I don't need to use those muscles to get an erection.

Remarks:
1. The curve / bend to the left also appears to be present with the pill. But after a manual support / fix / straighten out, it's straight again. With a full erection, no shape difference.
2. Sometimes the left side (towards which the penis bends) appears to be slightly painful. Painful in the sense of a burning / tearing / nagging pain. Pain rating: 1/10.
3. Erection is significantly stronger with the pill. I think from 60% to 90%.

Thanks for your reply. As written in my opening post, I refied the recommendation with respect to Cialis given my worries about the potential side effects. What you state, makes fully sense. As I mentioned in the previous post, I started with Cialis as of this morning. More details in the post above this one.

The first two weeks I could get NTE's, not every night. Thereafter, it has been very quite. However, since previous week. My body seemingly want to get erect during night and mornings but it gets stuck during semi-erect. The stage at which it there is a severe bending (like it is broken/crooked) due to the injury. I hope Cialis will help to fully recover.

Do you think I am doomed in the sense that I lost valuable time by not starting immediately 1.5 week ago? And is there any chance that Cialis (PDE5) can help me fully recover and become no longer reliant on it?

A week and a half won't make a difference.  Glad to here no side effects and it's helping!

I can not believe that we are already more than 2 weeks further. It feels like an eternity.

These two weeks were again full of depression. After the first day of Cialis, erection quality decreased more and more. Last week, I got an erection but suddenly felt something tear inside and it hurt terribly and burned at that moment. It felt as if something teared completely.

Currently I take Vitamin E 400IE and 5mg Cialis daily (before going to bed). After the first day, the erections weren't as strong as it was that day.

Unfortunately, I am still struggling with severe erectile dysfunction. It is not possible to get an erection, sometimes the tendency is there and it does not seem to go beyond, say, 30-40% in terms of erection strength/quality. When I want to tighten my muscles (the ones when holding pee) to strengthen/stimulate the erection, it hurts on the left side like something is about to tear or is wounded.

My current issues are currently:
- Severe erectile dysfunction (cannot get an erection), pain with any kind of erection
- Weakness / pain on the left side of my penis where the erection curves / curves. It is a burning pain
- Swollen veins. I don't think this is a problem and in my opinion mainly cosmetic. I think the first two problems are the worst.

So interesting that the erection quality has seemed to decrease over time while increased..

Note: I did not orgasm for at least 2 weeks. My last masturbation/ejaculation/orgasm was on the 17th of August after taking the Cialis pill.

Erection worse on Cialis?  Doesn't make sense but anyone can have a unique reaction to a medication.  It's probably coincidental but try stopping it for a week and see if EQ returns.

I don't think the erection quality worsened due to Cialis. I think it is coincidental for sure.

What I am worried on though, is that my erectile dysfunction got more severe. Whereas I could get erections more easily in the first month of the fracture, the quality and presence seem to worsened over time.

Now, my main issue is that my penis feels very weak and tends to bend when flaccid. Before the fracture, my penis always tended to be somewhat hard or rubberish. Now, it feels very weak and spongy. I even cannot get an erection.

I find it weird that over time erectile (dys)function got more severe instead of improving. I feel miserable and very depressed.

What is interesting is that I never had any swelling, eggplant deformity or hematoma since the beginning of the fracture or injury.

Similar injury happened a couple of times during sex many years ago. I think I got some peyronies from it where the root of the penis is at the pelvic floor. 5 months ago I injure my penis again using a vacuum device. The pain you describe seems like the pain I experience. Urologist say it is most likely not peyronies but most likely the injury healing or a nerve damage injury. I can feel some plaque for sure but uro says thats healing and not peyronies. I have had sore and pain now for 5 months and getting tired of it all. But no severe new curvature I think (Had 10-20 degrees to the right to start with. Can your case be something similar. Have you been diagnosed and how is the pain now?
Slight curvature to the left when erect (10-15) More curvature when flaccid. Severe pain. Anxious. Got diagnosed with Peyronies Disease chronic phase in 2016. New injury 5 months ago. Been seeing several urologists wich all say its not peyronies. 38 years old.

Thank you for your reply. I've seen two different urologists. They both diagnosed me with a ''penile fracture'' but indicated there is a difference between a fracture and a "fracture". They indicated my fracture is probably more similar with a ''penile bruise'' or ''penile contusion''.

For the past two months I have been able to get erections, although not at full size and hardness (mostly varying between 50% and 70%). I started from the second month with a 5mg daily Cialis treatment as prescribed by my urologists. The first day of using Cialis, I got an erection to my satisfaction (at least 85%). However, the second day, I also got an erection but suddenly felt a terrible pain with the feeling that something literally tore and my erection disappeared immediately.

Since then, my penis feels super soft when it is flaccid. Softer than I'm used to. Normally it was always a bit hard during flaccid. Now it seems completely lifeless. Whenever I feel some blood flow going, the penis no longer gets bigger or erect (harder), but rather only bends severly to the left. And since then, also not able to get an erection. My last erection dates from 20 August 2020. During night, I feel my body wants to get an erection, but it just doesn't happen. Last orgasm/ejaculation/masturbation dates from 16 August. Since the beginning of the injury, I have avoided sex completely.

The pain I am currently feeling is a burning pain and mainly the left side. The pain is light and not very noticeable. For example, during exercise, I don't feel any pain.

The flaccid size also seems much smaller (especially girth).

I see. It does seem similar to my problem. My injury is also on the left side and when flaccid and semi erect i bends å lot to the left. I have some burning pain, some throbbing and some tingling. The first couple of months I had almost no erection. Now it's a bit better but rarely full. Bear in mind the psychological pressure we guys are going through, it might be the reason. Also know that men have about ten erections per night, some of them are only semi erect. I experience the same as you very often, as you describe the penis want to get erect but doesnt. It's normal during night. You might not notice your erections as you sleep. Every night I sleep and dont wake up from erections is a blessing cos they can be very painful. But I know theyre there. Did you get a new curvature after your injury? And if you did did it happen quickly? If not it might not be peyronies and youre lucky. And maybe the loss of erection is more psychological. Im no expert on this matter but this is what Ive been told from urologists.

Also remember that flaccid penises change a lot in firmness, shape, size etc. In my experience sometimes after the injury it's totally numb and then it goes away. I think also we guys pay so much attention to it due to what we experience that we notice things we didnt before injury and label them as abmormal when they might not be. I språk mostly for myself of course.

Before the injury, I already had a natural curve to the left. This was about 10 degrees and hardly noticeable.

In the beginning of the injury there was only a (new) aggravated curvature that occurred during flaccid (eg during urination) and semi-erect and happend to vanish away over time. Maybe because it was injured at that point in time. During erections, the penis was as it used to be. No changes.

The situation after 2 months, which I am currently in, is somewhat worse. I have pain more often, almost continously (throbbing, burning, itchy pain) and cannot get an erection. If some blood flows to it which induces a semi-erection (10-20%) then it bends severly to the left and seems to get 'stuck' there. With this 10-20% semi-erection, I see that the penis is not growing in length or thickness.

The worsening coincidentally started after the painful feeling of 'tearing' my penis during the second day of using Cialis. The erection grew to approx. 40% and then it felt like something 'teared' inside as described in one of my posts earlier.

But up until now, no changes in curvature when my erection was at least 70-75%. Sometimes it seemed to curve a bit more (20-25 degrees instead of 10 degrees) but it was not consistent, and did not look severe to me.

My flaccid penis is numb since mid-August. Before the fracture, it always felt more like a rubber and ready to be erect whenever wanted.
